Factores desencadenantes de lipohipertrofia en pacientes usuarios de insulina en una unidad de primer nivel de atención
Get PDFLa diabetes mellitus es una enfermedad crónica que se
desencadena cuando el páncreas no produce suficiente insulina, o cuando el
organismo no puede utilizar con eficacia la insulina que produce. La lipodistrofia
(lipohipertrofia, lipoatrofia) es una complicación dada por la técnica inadecuada
de inyección subcutánea de insulina. Objetivo: Determinar los factores
desencadenantes de lipohipertrofia en pacientes usuarios de insulina en una
unidad de primer nivel de atención. Metodología: Es un estudio observacional,
transversal y descriptivo; se utilizó recolección en hoja de datos conforme a
factores desencadenantes de lipohipertrofia, con una muestra conformada por
374 pacientes. Resultados: Se encontró predominio de pacientes del sexo
femenino (65.8%), con una edad media de 55 años, con diagnostico de
diabetes mellitus de mas de 10 años (73.8%), uso de glargina como tratamiento
(46%), utilizándola por mas de 10 años (60.4%), siendo la zona de aplicación
mas frecuente en abdomen seguida de los brazos, esto ocasionando la no
rotación de zonas de aplicacion (90.6%), con reutilización de la aguja en bajo
porcentaje (7%); encontrándose datos de lipohipertrofia en 19 pacientes (5.1%)
Conclusiones: Se encontró como el determinante más prevalente la no
rotación de áreas de aplicación de insulina en la mayoría de los pacientes
(90.6%); en cambio, respecto a otros estudios, sólo 26 pacientes (7%) reutilizan
las agujas. Los pacientes con datos de lipohipertrofia sólo se encontró en 19

No full textTrack & Field meet statisticsHead Coach: Kyle KeplerTrack & FieldThe Utah 4x1600 relay captured sixth place as the Ute track and field team compete in the first day of the Drake Relays. The relay comprised of Hannah McInturff, Nicole Rietz, Becky Sarmiento and Sarah Feeny improved 12 seconds faster than what the Utes recorded last year at the meet and placed sixth overall. McInturff led off the race with a split of 4:55, followed by Rietz\u27s time of 4:58. Becky Sarmiento took the reigns on the third leg and clocked a time of 4:56 and Feeny finished strong in 4:46. In the sole event of the evening, the Utes competed in the 4x400-meter relay. The relay comprised of Grace Englund, Christine Van Brocklin, Alissa Atisme and Allison Aafedt recorded a time of 3:46.30 to finish 22nd overall. The Utes competed in two events on the final day of action, including a top-ten finish in the distance medley. Getting the action underway for the Utes was the distance medley relay team, who earned Utah its second top-ten performance of the weekend. The relay comprised of Sarah Feeny, Alissa Atisme, Grace Englund and Hannah McInturff clocked a time of 3:53.61 to claim ninth overall. CanScreen5, a global repository for breast, cervical and colorectal cancer screening programs
No full textAbstract: In the first results from an ongoing global cancer screening data repository, screening program organization was better overall in Europe compared to other continents; however, there were substantial gaps in implementation across both high- and low-resource settings. The CanScreen5 project is a global cancer screening data repository that aims to report the status and performance of breast, cervical and colorectal cancer screening programs using a harmonized set of criteria and indicators. Data collected mainly from the Ministry of Health in each country underwent quality validation and ultimately became publicly available through a Web-based portal. Until September 2022, 84 participating countries reported data for breast (n = 57), cervical (n = 75) or colorectal (n = 51) cancer screening programs in the repository. Substantial heterogeneity was observed regarding program organization and performance. Reported screening coverage ranged from 1.7% (Bangladesh) to 85.5% (England, United Kingdom) for breast cancer, from 2.1% (Cote d'Ivoire) to 86.3% (Sweden) for cervical cancer, and from 0.6% (Hungary) to 64.5% (the Netherlands) for colorectal cancer screening programs. Large variability was observed regarding compliance to further assessment of screening programs and detection rates reported for precancers and cancers. A concern is lack of data to estimate performance indicators across the screening continuum. This underscores the need for programs to incorporate quality assurance protocols supported by robust information systems. Program organization requires improvement in resource-limited settings, where screening is likely to be resource-stratified and tailored to country-specific situations
Delayed colorectal cancer care during covid-19 pandemic (decor-19). Global perspective from an international survey

The widespread nature of coronavirus disease 2019 (COVID-19) has been unprecedented. We sought to analyze its global impact with a survey on colorectal cancer (CRC) care during the pandemic.
Methods
The impact of COVID-19 on preoperative assessment, elective surgery, and postoperative management of CRC patients was explored by a 35-item survey, which was distributed worldwide to members of surgical societies with an interest in CRC care. Respondents were divided into two comparator groups: 1) ‘delay’ group: CRC care affected by the pandemic; 2) ‘no delay’ group: unaltered CRC practice.
Results
A total of 1,051 respondents from 84 countries completed the survey. No substantial differences in demographics were found between the ‘delay’ (745, 70.9%) and ‘no delay’ (306, 29.1%) groups. Suspension of multidisciplinary team meetings, staff members quarantined or relocated to COVID-19 units, units fully dedicated to COVID-19 care, personal protective equipment not readily available were factors significantly associated to delays in endoscopy, radiology, surgery, histopathology and prolonged chemoradiation therapy-to-surgery intervals. In the ‘delay’ group, 48.9% of respondents reported a change in the initial surgical plan and 26.3% reported a shift from elective to urgent operations. Recovery of CRC care was associated with the status of the outbreak. Practicing in COVID-free units, no change in operative slots and staff members not relocated to COVID-19 units were statistically associated with unaltered CRC care in the ‘no delay’ group, while the geographical distribution was not.
Conclusions
Global changes in diagnostic and therapeutic CRC practices were evident. Changes were associated with differences in health-care delivery systems, hospital’s preparedness, resources availability, and local COVID-19 prevalence rather than geographical factors. Strategic planning is required to optimize CRC care

No full textBackground Neurocritical care is devoted to the care of critically ill patients with acute neurological or neurosurgical emergencies. There is limited information regarding epidemiological data, disease characteristics, variability of clinical care, and in-hospital mortality of neurocritically ill patients worldwide. We addressed these issues in the Point PRevalence In Neurocritical CarE (PRINCE) study, a prospective, cross-sectional, observational study. Methods We recruited patients from various intensive care units (ICUs) admitted on a pre-specified date, and the investigators recorded specific clinical care activities they performed on the subjects during their first 7 days of admission or discharge (whichever came first) from their ICUs and at hospital discharge. In this manuscript, we analyzed the final data set of the study that included patient admission characteristics, disease type and severity, ICU resources, ICU and hospital length of stay, and in-hospital mortality. We present descriptive statistics to summarize data from the case report form. We tested differences between geographically grouped data using parametric and nonparametric testing as appropriate. We used a multivariable logistic regression model to evaluate factors associated with in-hospital mortality. Results We analyzed data from 1545 patients admitted to 147 participating sites from 31 countries of which most were from North America (69%, N = 1063). Globally, there was variability in patient characteristics, admission diagnosis, ICU treatment team and resource allocation, and in-hospital mortality. Seventy-three percent of the participating centers were academic, and the most common admitting diagnosis was subarachnoid hemorrhage (13%). The majority of patients were male (59%), a half of whom had at least two comorbidities, and median Glasgow Coma Scale (GCS) of 13. Factors associated with in-hospital mortality included age (OR 1.03; 95% CI, 1.02 to 1.04); lower GCS (OR 1.20; 95% CI, 1.14 to 1.16 for every point reduction in GCS); pupillary reactivity (OR 1.8; 95% CI, 1.09 to 3.23 for bilateral unreactive pupils); admission source (emergency room versus direct admission [OR 2.2; 95% CI, 1.3 to 3.75]; admission from a general ward versus direct admission [OR 5.85; 95% CI, 2.75 to 12.45; and admission from another ICU versus direct admission [OR 3.34; 95% CI, 1.27 to 8.8]); and the absence of a dedicated neurocritical care unit (NCCU) (OR 1.7; 95% CI, 1.04 to 2.47). Conclusion PRINCE is the first study to evaluate care patterns of neurocritical patients worldwide. The data suggest that there is a wide variability in clinical care resources and patient characteristics. Neurological severity of illness and the absence of a dedicated NCCU are independent predictors of in-patient mortality
