    AUTISM Jean Steyaert, MD, PhD Katholieke Universiteit Leuven, departement Neurowetenschappen & Psychiatrie Academisch Ziekenhuis Maastricht, Afdeling Klinische Genetica Correspondentie: [email protected] Einde 2007 en in 2008 waren de belangrijkste nieuwe wetenschappelijke inzichten m.b.t. autismespectrumstoornissen -verder ASS- vooral op het vlak van fundamenteel onderzoek naar de etiologie en neurobiologie van de aandoening. Ook op het vlak van het klinisch beeld op heel jonge leeftijd werd relevant nieuw onderzoek gebracht op de verschillende bezochte congressen (IMFAR, WCPG, Nationaal Autismecongres Rotterdam, e.a.) en in de wetenschappelijke literatuur. Vroege detectie In de voorbije jaren zijn veel pogingen ondernomen om ASS op zo jonge mogelijke leeftijd te diagnosticeren, liefst voor 2 jaar. De reden hiervoor is o.m. dat in toenemende mate wordt aangetoond dat vroege interventie een significant positieve invloed heeft op het verloop en de prognose van de aandoening. De voorbije jaren werden verschillende screenings- en diagnostische instrumenten ontwikkeld voor de leeftijdcategorie 12-24 maanden (Zwaigenbaum and Stone 2008) en richtlijnen ontwikkeld (Johnson, Myers, and Council on Children With Disabilities 2007). In grootscheepse en goed opgezette onderzoeken zoals het Early Screening for Autism Traits, waarin meer dan 30.000 kinderen uit de algemene populatie gescreend werden (Dietz et al. 2006), bleek echter dat tot de helft van de kinderen die later ASS bleken te hebben, gemist werden bij de vroege screening. Het succes hiervan is dus beperkt, en één van de mogelijke redenen is dat de gebruikte instrumenten te sterk focussen op het beeld van ASS vanaf de leeftijd van 4-5 jaar: met name deficits in de sociale wederkerigheid, deficits in de communicatie, en repetitieve activiteiten en interesses (RIB’s: repetitive interests & behaviours). Interessanter waren de prospectieve studies bij zuigelingen met een verhoogd risico voor ASS o.m. broertjes en zusjes van kinderen met ASS (Brian et al. 2008; Ozonoff et al. 2008). Dergelijke studies tonen o.m. aan dat de diagnose niet volledig stabiel is beneden de leeftijd van 3 jaar: sommige kinderen lijken nog “in” of “uit” de diagnose te stappen. Daarnaast blijkt ook dat op zuigelingenleeftijd, vooral tot 12 maanden, atypische motorische kenmerken meer voorspellend zijn voor het later ontwikkelen van autisme dan sociale en communicatieve kenmerken, terwijl deze laatste kenmerken op peuterleeftijd (18-36 maanden) meer voorspellend zijn dan RIB’s. Tenslotte bleek dat bij nagenoeg 1 op 4 kinderen met de aandoening er voor 18 maanden helemaal geen kenmerken van ASS waren, en bijgevolg dat het zogenaamd late onset autism –eerste tekenen tussen 18 en 36 maanden - veel vaker voorkomt dan eerder gedacht. Genetica De meest merkwaardige ontwikkeling m.b.t. de erfelijkheid en genetica van ASS op dit ogenblik komt wellicht uit de epidemiologische hoek en gaat over de validiteit van het klinisch concept ASS als een unitaire aandoening. Al jaren zijn clinici het erover eens dat er wel een fenotypische heterogeniteit is bij ASS, zoals wellicht bij alle neuropsychiatrische aandoeningen, maar dat de drie kerndomeinen die ook als diagnostische criteria worden gebruikt echt wel de kern van de aandoening vormen en ook in hoge mate aan elkaar gecorreleerd zijn in een klinische populatie. Uit onderzoek in de algemene populatie (Happe 2008) blijkt echter dat de kenmerken in de 3 kerndomeinen van ASS zwak tot matig gecorreleerd zijn aan elkaar. Concreet betekent dit dat in de algemene populatie personen die bijvoorbeeld zwakke scoren op het vlak van sociale wederkerigheid niet noodzakelijk zwak scoren op het vlak van communicatievaardigheden en/of RIB’s. De auteurs concluderen hieruit dat er in de algemene populatie ten minste drie clusters van genetische factoren zijn die vrij onafhankelijk van elkaar bijdragen tot de symptomatologie van ASS. Een geheel andere visie komt van J. Constantino (Virkud et al. 2008; Constantino et al. 2004) die geen duidelijke factorstructuur vindt in ASS-kenmerken in de algemene populatie (traits) of symptomen bij patiënten. Hij vindt daarentegen een duidelijke correlatie tussen de verschillende kenmerken van ASS, die weliswaar ook in zijn onderzoek continu verdeeld zijn in de bevolking. Hij acht een samenhang binnen het fenotype niet in tegenspraak met een etiologische heterogeniteit, die de jongste jaren nagenoeg algemeen aanvaard wordt (Geschwind 2008). Een boeiende bevinding op moleculair genetisch vlak is de hoge prevalentie van autisme-specifieke copy number variations of CNV’s , vooral in simplex families (1 persoon met de aandoening) maar ook in multiplex families (verschillende personen met de aandoening) (Marshall et al. 2008). CNV’s zijn heel kleine structurele afwijkingen in chromosomen, zoals deletie of duplicatie van één of hooguit enkele genen. CNV’s komen vaak voor in het genoom van alle mensen, maar Marshall et al. vonden een aantal auti-specifieke CNV’s: o.m. van het SHANK, neuroligine4 en neurexine1 gen, 3 genen met een functie in de synapsvorming, en bij verschillende individuen ook CNV’s in de 16p11 regio, een regio die al langer in de belangstelling staat in de moleculaire genetica van autisme. Een geheel andere genetische bevinding is de ontdekking van mutaties in het ASMT-gen bij een klein aantal personen met ASS, maar bij geen enkele controlepersoon (Melke et al. 2008). Deze mutaties leiden tot een sterke beperking in de aanmaak van melatonine. De rol hiervan in de pathogenese van ASS is echter niet duidelijk, maar interessant omdat een subgroep kinderen met ASS zeer hardnekkige slaapstoornissen heeft. Brain imaging Via Diffusion Tensor Imaging (DTI) -een MRI-techniek die toelaat om specifiek tracti in de witte stof in beeld te brengen- werd in een eerste onderzoek aangetoond dat bij volwassenen met ASS de korte subcorticale witte stof banen meer volume innemen, terwijl lange, dieper liggende banen minder volume innemen dan bij controlepersonen (Geschwind and Levitt 2007). Het boeiende van deze bevinding is dat ze in de lijn ligt van de moleculair-genetische bevindingen die een belangrijke etiologische rol toekennen aan stoornissen in de synapsvorming en/of –plasticiteit. Aangezien het onderzoek bij volwassenen werd uitgevoerd is het echter niet duidelijk of de vastgestelde afwijkingen een vroege pathologische ontwikkeling weergeven, of eerder het eindstadium tonen van een aanpassingsmechanisme in het brein met ASS. ASS als systeemziekte Een theorie die aanvankelijk werd beschouwd als aan de rand van de wetenschap wint steeds meer veld: m.n. dat ASS een aandoening is die verschillende systemen aantast, en niet alleen het centraal zenuwstelsel. Een langer bestaand argument hiervoor was dat verschillende van de kandidaat-genen die gevonden zijn bij ASS ook in andere systemen tot expressie komen. Recent empirisch onderzoek toont o.m. aan dat gastro-intestinale symptomen, in het bijzonder gastro-oesofagale reflux met letsels (Buie 2007) en niet-allergische voedselintolerantie vanaf jonge leeftijd vaker voorkomen bij ASS. Ook voor afwijkingen in het immuunsysteem is er meer empirische evidentie (Enstrom et al. 2008; Cabanlit et al. 2007), niet alleen bij het individu met ASS, maar ook bij zwangere moeders van kinderen die later ASS ontwikkelden (Pardo 2008). Behandeling Een eerste highlight hier is de richtlijn van de American Association of Pediatricians, www.aap.org/healthtopics/autism.cfm , om bij peuters reeds bij een vermoeden van ASS een interventie te starten. Een tweede, maar louter hypothetische idee, is het geven van SSRI’s op een leeftijd beneden 3 jaar omdat serotonine op zeer jonge leeftijd een rol speelt in de synaptogenese (Bethea and Sikich 2007). Conclusie Meer en meer argumenten wijzen in de richting van ASS als een diffuse stoornis van het centraal zenuwstelsel, met een duidelijke impact op de synapsvorming en –plasticiteit, waardoor de connectiviteit in de hersenen verstoord kan verlopen.     The Effects of Four Weeks of Intranasal Oxytocin on Social Responsiveness and Repetitive and Restricted Behaviors in Autism Spectrum Disorders: A Randomized Controlled Trial. Sylvie Bernaerts, Claudia Dillen, Jean Steyaert, and Kaat Alaerts Background: Autism spectrum disorders (ASDs) are characterized by impairments in social interaction and repetitive and restricted behaviors (RRBs). To date, no pharmacological treatment exists targeting the core symptoms of ASD, yet the pharmacological use of the neuropeptide oxytocin (OT) has gained interest from the research community to explore its potential for elevating social deficits in ASD. Methods: A double-blind randomized placebo-controlled trial with thirty-four young adult men with ASD (17 OT/17 Placebo) was conducted to assess behavioral effects of OT therapy (i) at baseline; (ii) after four weeks of therapy; and (iii) four weeks post-treatment (follow-up). Doses of 24 IU oxytocin (Syntocinon®, Sigma-tau) or placebo nasal spray were administered daily for four weeks. Primary outcome measures included the social responsiveness scale (SRS) and the repetitive behavior scale (RBS-R). Secondary outcome measures included questionnaires assessing attachment, mood state and quality of life. Results: After four weeks of OT administration, self-reports on RRBs showed tentative reductions in the OT group (F(1,31)=3.83, p=0.06) and this effect persisted until one month after treatment F(1,31)=4.02, p=0.05). No significant changes in social responsiveness were observed immediately after the four-week OT treatment, but interestingly, at the 1-month follow-up session, significant improvements were revealed for the informant-based SRS report, indicating clear improvements in reports of social motivation (F(1,22)=4.71, p=0.04) and social communication (F(1,22)=7.39, p=0.01). For the secondary outcome measures, only tentative effects were revealed.     Problem behavior in young children referred with language difficulties: Relations to language and intentional communication

    BACKGROUND AND AIMS: This exploratory study aims to examine the relative contribution of language and intentional communication to internalizing and externalizing problem behavior. METHODS: Twenty-nine Dutch-speaking children (age range 24-46 months) referred with language difficulties participated in this study. For the majority of children, these early language difficulties appeared to be part of a broader neurodevelopmental disorder, mainly autism spectrum disorder. Parent ratings on the Achenbach Child Behavior Checklist 1½-5 were predicted from children's language level and intentional communicative abilities, the latter being assessed by both parent report and direct observation. In all series of hierarchical regression analyses, chronological age and nonverbal mental age were included as covariates. RESULTS: Parents commonly reported withdrawal, emotionally reactive behavior, attention problems, and aggressive behavior. Parent-rated intentional communication was the most important predictor of internalizing problem behavior and played an important role in the prediction of aggressive behavior as well. However, chronological age and/or nonverbal mental age also predicted parent-rated levels of externalizing problem behavior, especially attention problems.     Local and global visual processing in autism spectrum disorders: Influence of task and sample characteristics and relation to symptom severity

    Local and global visual processing abilities and processing style were investigated in individuals with autism spectrum disorder (ASD) versus typically developing individuals, children versus adolescents and boys versus girls. Individuals with ASD displayed more attention to detail in daily life, while laboratory tasks showed slightly reduced global processing abilities, intact local processing abilities, and a more locally oriented processing style. However, the presence of these group differences depended on particular task and sample (i.e., age and gender) characteristics. Most measures of local and global processing did not correlate with each other and were not associated with processing style.     Gaze-contingent displays as new window on analytical and holistic face perception in autism spectrum disorder

    The strength of holistic face perception in children with autism spectrum disorder (ASD) was evaluated by applying the gaze-contingent mask and window technique to a face matching and discrimination task in 6- to 14-year-old children with (n = 36) and without ASD (n = 47), and by examining fixation patterns. Behavioral results suggested a slower and less efficient face processing in the ASD sample compared with the matched control group. Comparing the moving mask and window conditions revealed a reduced holistic face processing bias in the younger age group but not in the older sample. Preferential viewing patterns revealed both similarities and differences between both participant groups.sponsorship: We thank all the participating children and their parents. De stripcolleges zijn getekend en geproduceerd door Martijn van Gemert. Elk college duurt niet langer dan 10 minuten.
