Necrotizing fasciitis following a monkey bite
Necrotizing fasciitis (NF) is rapidly spreading bacterial infection of a facial plane. It has a higher mortality rate in patients with diabetes and immunocompromised state. The initial presentation mimics cellulitis and most of the cases are misdiagnosed. Delayed diagnosis and treatment increase mortality by nearly 100%. Our case developed NF with septic shock following a monkey bite. The patient underwent prompt fasciotomy along with extensive wound debridement. Later, the patient underwent secondary suturing with a skin graft for wound cover. NF following money bite is not reported in the literature till now. Hence, early diagnosis and prompt surgical debridement in monkey bites will reduce the incidence of mortality due to NF
A case of synchronous primary gastric cancer and primary hepatocellular carcinoma
Synchronous occurrence of primary gastric cancer and hepatocelluar carcinoma is very rare. It is mandatory to differentiate synchronous two primary lesions from metastatic lesion, because treatment modality is different for each disease. We report an interesting case of double malignancy, synchronous primary gastric cancer, and primary hepatocelluar carcinoma, who never had a cirrhosis, family history of cancer as well as hepatitis B or hepatitis C infection. Our patient managed with palliative chemotherapy due to multicentric hepatocelluar carcinoma with primary gastric cancer
Uncommon location of verrucous carcinoma
Verrucous carcinoma is an uncommon locally invasive slow growing squamous cell carcinoma of the skin and mucous membrane. It usually appears as a raised, fungating, white cauliflower-like mass. It usually occurs in oral cavity followed by genital region. Human papillomavirus, betel nut chewing, alcohol consumption, and schistosomiasis are the accepted causes for verrucous carcinoma. The recommended treatment is wide local excision with a negative margin. Even after wide local excision, local recurrence is very high compared to squamous cell carcinoma. Until now, only one case was reported with presternal verrucous carcinoma which is a rare occurrence
Diagnostic difficulty of liver lesion
Liver abscess is mostly either pyogenic or amebic. Fungal and mycobacterial liver abscesses are rare and mostly associated with immunosuppression. The occurrence of fungal liver abscess with hydatid cyst was never reported previously. This case created diagnostic difficulty, whether we are dealing with liver abscess or hydatid cyst. Sometimes, it may be possible that concurrent occurrence of hydatid cyst with liver abscess or hydatid cyst become infected and mimic like liver abscess
Adult jejuno-jejunal intussusceptions due to gastrointestinal stromal tumor
Intussusception is telescoping of proximal segment of the intestine within the lumen of adjacent segment of intestine. In children, most of the intussusception is idiopathic, whereas in adults, it is always associated with pathological lead point. In adults, jejuno-jejunal intussusception is very rare compared to children. Preoperative diagnosis is difficult in most of the cases, so adequate negative margin is mandatory when a gastrointestinal stromal tumor (GIST) is suspected as the cause of jejuno-jejunal intussusception. This case is reported because jejunal intussusception is one of the rare presentations of GIST
An update in the management of malignant melanoma
Malignant melanoma is one of the most dangerous forms of cutaneous malignancies. It is classified as cutaneous melanoma and non-cutaneous melanoma based on the location of the lesion. Ultraviolet (UV) radiation exposure is a predisposing factor for cutaneous melanoma, although in other forms such as mucous and anorectal malignant melanomas, ultraviolet radiation does not play a significant role. Human papilloma virus and human retroviral infections are commonly associated with non-cutaneous melanomas.</jats:p
Cutaneous malignant melanoma and targeted therapy based on the biomarkers
Malignant melanoma is the most aggressive form of cutaneous malignancy. It accounts for more than 75% of cancer-related deaths among cutaneous malignancies. It accounts for <5% of cutaneous malignancy. Numerous biomarkers are used in malignant melanoma with varying clinical applications, including diagnostic purposes, prognosis, therapeutic purpose, and targeted therapy against melanoma. Systemic chemotherapy in malignant melanoma has little benefit compared to immunotherapy and targeted therapy. The observed overall survival with systemic chemotherapy is much less compared with targeted therapy in advanced or metastatic melanoma. Various targeted therapies are currently used in melanoma treatment including BRAF inhibitors such as vemurafenib and dabrafenib; MEK inhibitors such as trametinib; anti-CTLA-4 antibodies such as ipilimumab; and anti-programmed cell death 1 antibodies such as nivolumab, pembrolizumab, and pidilizumab. Bilateral Breast Hamartoma
Breast hamartoma is a rare benign tumor of the breast, and it is classified based on the variable tissue components. It is more common during the lactating period and premenopausal period. Bilateral breast hamartoma is rarely reported in the literature. It might create diagnostic difficulty based on fine-needle aspiration cytology and core needle biopsy. Radiological investigation sometimes might have overlaying findings with malignant disease. It is always better to undergo an excision biopsy for the confirmation of the diagnosis
