    Prospective evaluation of clinical symptoms of chemotherapy‐induced oral mucositis in adult patients with acute leukemia: A preliminary study

    Objective The objective of this study was to prospectively evaluate the clinical features of chemotherapy-induced oral mucositis (CIOM) in adult patients with acute leukemia and the aggravating factors for such symptoms. Subjects Thirty-seven prospective patients aged >= 19 years with acute leukemia undergoing chemotherapy were enrolled. Oral and clinical investigations were performed at baseline and on Day 14 after starting chemotherapy. The presence and severity of cancer-induced oral mucositis were demonstrated using the World Health Organization (WHO) scoring system. Results On Day 14, we found that oral mucositis had developed in eight patients (21.6%). Hematopoietic stem cell transplantation (HSCT) in patients was a predictor of increased WHO scores (beta = 1.937, p < .001). Regarding oral sites, ventral tongue (beta = 1.670), soft palate (beta = 1.242), and buccal mucosa (beta = 0.593) were predictors for increased scores. In addition, the increase in WHO scores was positively correlated with the number of oral lesions (r = .521), the difficulty in eating (r = .250), and the overall oral health (r = .534; all p < .05). Conclusion The main factors affecting the severity of CIOM symptoms were the treatment with HSCT and the location of oral lesions. The incidence of CIOM and WHO scores were not significantly different between the subgroups of disease. Our findings will help clinicians investigate the oral findings after chemotherapy in adult patients with acute leukemia.Y

    Strong association between herpes simplex virus-1 and chemotherapy-induced oral mucositis in patients with hematologic malignancies

    Background/Aims A link between oral cavity infections and chemotherapy-induced oral mucositis (CIOM) in patients with hematological malignancies (HMs) undergoing intensive chemotherapy (IC) or hematopoietic stem cell transplantation (HSCT) has been suggested. However, conclusive data are lacking, and there are no current guidelines for the prophylactic use of antimicrobials to prevent CIOM in these populations. Methods The relationships between herpes simplex virus (HSV) reactivation and Candida colonization in the oral cavity and CIOM in patients with HMs undergoing IC or HSCT were evaluated. Patients aged ≥ 19 years with HMs undergoing IC or HSCT were enrolled. Each patient was evaluated for HSV and Candida in the oral cavity along with CIOM at baseline and during the 2nd, 3rd, and 4th weeks. Results Seventy presentations among 56 patients were analyzed. CIOM was observed in 23 presentations (32.9%), with a higher incidence associated with HSCT (17 of 35 presentations, 48.6%) than with IC (six of 35 presentations, 8.6%). The reactivation of HSV-1 was significantly associated with an increased incidence of CIOM after adjusting for age, sex, type of disease, and treatment stage. A higher HSV-1 viral load was associated with an increased incidence of CIOM. The presence of Candida was not associated with CIOM. Conclusions HSV-1 reactivation in the oral cavity was highly associated with CIOM in patients with HMs undergoing high-dose chemotherapy

    주류 국제정치이론들에서 외재적인 독립변수로만 취급되어 왔던 정보기술체계가 국제정치적으로 구성되는 모습을 이 글은 추적한다. 특히 오늘의 인터넷이 가지고 있는 이중적인 커뮤니케이션 구조가 만들어진 국제정치적인 과정에 주목한다. 인터넷은 매우 분산적인 커뮤니케이션 경로와 이를 관리하는 프로토콜이라는 집중의 관리방식이 결합된 이중적인 커뮤니케이션 구조를 가지고 있다. 이 이중적인 구조는 냉전체제라는 제약 속에서 만들어진 미국 핵전략의 기술적인 필요를 만족시키기 위해 구상되었다. 이러한 기술적인 필요는 이중적 커뮤니케이션 구조를 갖는 컴퓨터 네트워크의 개발로 충족되었으며 동시에 이 개발과정은 정보기술이 가지고 있는 물리적 속성에 의해 다시 영향을 받았다. 이 글은 1950년대 반자동지상환경(SAGE: Semi-Automatic Ground Environment) 대공방어체계 개발 사례, 1960년대 ARPANET 개발 사례를 분석하여 이와 같은 냉전의 국제체제, 미국의 전략적 필요, 정보기술의 속성이라는 삼자의 구성적인 상호작용이 인터넷의 이중적 커뮤니케이션 구조를 창출하는 과정을 분석한다.For many years, scholars of International Relations have paid much attention to the effect of Information Technology to world politics. However, few explored how international political processes have influenced the formation of IT. This paper tries to fill this gap by examining the Cold War origin of the communication structure of the Internet. Contrary to the popular myth of the inherent liberal characteristics of the Internet, its structure has both decentralizing and centralizing effects. On the one hand, the Internet has decentralizing architecture that promotes free communication among individuals. On the other hand, this decentralized communication is governed by the centralized control of protocols. This double structure of the Internet communication was developed to fulfill technological needs of the nuclear strategy of the U.S. during the Cold War, and its developmental process, in turn, was affected by the inherent characteristics of IT. This paper shows how the constitutive relationship of the Cold War international system, the nuclear strategy of the U.S. and material features of IT gave birth to the unique structure of the Internet through the studies of two critical junctures of its development: Semi-Automatic Ground Environment project in 1950s and ARPANET in 1960s

    45 pg.Tillie Olsen's life experiences and self-identification as a working class woman provide a strong basis for analyzing her fiction as partly autobiographical. As she wrote, she developed her position as a recognized and award winning author into that of a literary mediator for socially marginalized subjects, actively working to represent certain conditions of exclusion due to social, racial, economic, and sexual factors during the 1970's and 1980's. Through analysis of her fiction and non-fiction texts, her use of modernist writing techniques, her purpose as a writer, and her impact on the literary canon, it becomes possible to see how she has altered the literary landscape and has made those who suffer exclusion visible and legible.Advisor(s): Choi, Helen .
