Behavior of Soil Reinforcements in Slopes
AbstractIn geotechnical and transportation engineering, especially road constructions in cuts and deep excavation problems are solved by using different supporting excavation methods. Soil reinforcements are chosen due to easy and economic application. In this study, behavior of supported slopes with geotextiles and geogrids were analyzed by performing experiments on slope models in the laboratory. In the experiments, a static loading was applied to find failure surface and deformations for each case. Additionally, slopes were designed by using Plaxis program. At the end of the study, experimental and analytic models were compared and also behavior of models were presented
DC/DC Converters for Multiterminal HVDC Systems: Based on Modular Multilevel Converter
The increase in the energy demand has resulted in searching for new energy sources. Due to the increased prices of fossil fuels and the environmental issues, renewable energy sources has become popular in Europe. Renewable energy generation such as wind, solar and wave has point-to-point connections with the main grid. In order to minimize the disadvantages of renewable energy sources like energy fluctuations, multi-terminal systems are favoured interconnecting energy generation stations among them with the main grid. The long distance interconnection especially for offshore wind farms and intercontinental connections is not possible with traditional alternating current (AC) technology because of the limited power transfer. Therefore, high voltage direct current (HVDC) technology is considered as the main element for the future multi-terminal grid. The conversion between AC and DC is preferred via voltage source converters (VSC) as they offer more flexible power control compared to traditional current source converters in other words line commutated converters (LCC). HVDC power transmission schemes have been constructed depending on the technology of the time, which means that there is no standardization in voltage levels and configurations of the HVDC schemes. Therefore, the connection of such systems operating at different voltage levels and/or in different schemes such as monopole and bipolar systems for the future multi-terminal DC grids requires DC/DC converters. Although the voltage level and the configurations are the same for both DC systems, DC/DC converter may be required for the power flow control in multi-terminal DC grid. The modular multilevel converter (MMC) seems as the most suitable converter in this application due to its advantages such as low switching losses, high scalability and modularity. MMC control structures are introduced and front-to-front connection of two MMC forming a DC/AC/DC converter is modelled and simulated for different applications such as interconnecting systems with same voltage levels, different voltage levels or different configurations in this study. The passive element sizes of MMC submodules are also compared for different AC side frequency as the component sizes can be decreased thanks to the increase in the AC frequency which has a disadvantage of higher switching losses. Systemic Immune-Inflammation Index as a Predictor of Asymptomatic Organ Damage in Patients with Newly Diagnosed Treatment-Naive Hypertension
Background: Low-grade inflammation is known to facilitate the development of hypertensive organ damage. The systemic immune-inflammation index (SII) is a new inflammatory index based on circulating immune-inflammatory cells. Objectives: The objectives of this study were to investigate the relationship between the SII and asymptomatic organ damage (AOD) in patients with newly diagnosed treatment-naive hypertension (HTN). Methods: A total of 500 participants (>= 18 years) were enrolled in the study, including 250 patients and 250 healthy volunteers. Microalbuminuria of > 30 mg/day or proteinuria of > 150 mg/day, left ventricular mass index of > 95 g/m(2) in women and > 115 g/m(2) in men, and carotid intima-media thickness of > 0.9 mm or the presence of plaque in the carotid were evaluated as AOD indicators. AOD grade was classified as follows: Grade I - One organ involved, Grade II - Two organs involved, Grade III - Three organs involved, and Grade IV - Four organs involved. Results: SII values were higher among patients with HTN than in the control group. Positive correlations were found between the SII and AOD indicators and C-reactive protein levels. Increasing SII values were a common independent predictor of the presence and severity of AOD. The gradually increasing threshold values of the SII from no AOD to Grade III-IV exhibited high diagnostic performance. Conclusions: High SII values were independent predictors of the presence and severity of AOD in patients with newly diagnosed treatment-naive HTN. Considering the role of inflammation in HTN, the SII, which can be easily evaluated using blood parameters, can be an effective prognostic screening tool. ( REV INVEST CLIN. 2022;74(5):258-67
TCT-1023 Comparative Effectiveness of DES, DCB, and BMS for the Treatment of In-Stent Restenosis in Carotid Artery Stents: A Systematic Review and Meta-Analysis
Background: In-stent restenosis (ISR) following carotid artery stenting is a challenging complication associated with an increased risk of cerebrovascular events. Various endovascular approaches—including drug-eluting stents (DES), drug-coated balloons (DCB), and bare-metal stents (BMS)—are employed for ISR treatment. This study compares the clinical outcomes of these treatment modalities. Methods: A systematic review and meta-analysis of studies from 2005–2024 was conducted. Outcomes included restenosis rate, stroke, target lesion revascularization (TLR), and mortality. Subgroup analyses by patient comorbidities and lesion characteristics were also performed. Results: A total of 12 studies comprising 1,284 patients were included (DES: 478; DCB: 412; BMS: 394). DES had the lowest rate of recurrent ISR (7.2%), followed by DCB (12.8%) and BMS (22.4%). TLR was lowest in the DES group (5.1%) compared to DCB (10.6%) and BMS (17.3%). Stroke and mortality rates were similar across groups. Conclusion: DES appears to offer superior efficacy in preventing recurrent carotid ISR and reducing the need for revascularization. DCBs are a viable alternative in high-risk patients where stenting is undesirable. BMS should be reserved for select cases given higher restenosis and TLR rates. TCT-1231 Comparison of Micropuncture Versus Ultrasound-Guided Arterial Access Techniques: A Systematic Review of Patient Outcomes and Procedural Safety
Background: Arterial access is foundational to many endovascular procedures. Micropuncture and ultrasound-guided techniques are increasingly used to minimize complications. This study compares these two methods in terms of access success, procedural complications, and clinical outcomes. Methods: A systematic review and pooled analysis was conducted on studies comparing micropuncture and ultrasound-guided arterial access in femoral and radial procedures. Key endpoints included access success rate, access time, hematoma formation, pseudoaneurysm, and need for surgical repair. Results: Seven studies with 2,840 patients (micropuncture: n=1,420; ultrasound-guided: n=1,420) were included. Ultrasound guidance showed superior first-pass success (93.8% vs. 87.1%, p\u3c 0.001) and reduced access site complications, especially hematomas (1.8% vs. 4.7%, p=0.02). Micropuncture offered shorter mean access time in radial access procedures. Pseudoaneurysm formation was significantly lower with ultrasound guidance (0.2% vs. 1.1%, p=0.01). Conclusion: Ultrasound-guided arterial access demonstrates a superior safety profile and higher success rate compared to micropuncture techniques, especially for femoral access.
