    La sindrome di Ekbom: uno studio su due casi

    Introduction Ekbom's syndrome is a rare disease characterized by the patient’s belief to be invaded from parasites. It causes itch and/or pins and needles with intense scratching and excoriated lesions. Aim We describe two cases where the psychotherapeutic treatment has allowed pharmacological one, removed the causes and produced the disappearance of symptoms. Clinical Case Patients, aged 67 and 70 years, after the traumatic death of a family member showed intense itch and excoriated lesions on scalp, trunk and limbs. They attributed these symptoms to an infestation. The patients were submitted to Rorschach and Machover tests, psychotherapy and anti-psycotic. Resulted The patients present compromised the examination of reality and go toward a psychotic pathology. Affective compulsion, depressed humor, extraneousness to the environment and polarization of the thought, were present. The interviews pointed out the delirium was the conversion of an uneasiness psychic. The psychotherapeutic treatment has allowed the patients to accept the pharmacological one and the regression of the psychosis. Discussion The delirium and the tactile hallucinations are been induced by an inevitable psychic trauma. The content of the delirium is the death and it is connected to the bugs. The skin lesions are the proof of the delirium. Besides, the itch and the scratching could be determined by emotional regressive reactions. They determine a reassuring role and stabilize the emotional conflict. Conclusions We retain that in the previous cases the delirium of infestation can occur by an event perceived as terrifying, because it is negative,immediate, unpredictable and unstoppable

    Il disagio psichico nei soggetti con malattia di La Peyronie o di pseudo-Peyronie: uno studio su 55 pazienti

    Introduction Peyronie’s (PD) and pseudo PD’s deseases are erectile dysfunctions with recurvatio penis and pain. They devalue the identification of the erection with the perception to be male for the reduction and/or the loss of the first one. Purpose The possible psychological damages and the methods employed for appraising the psychic state of the subject with PD or pseudo PD have been studied and described Materials and Methods 26 patients with PD and 29 with pseudo PD, aged 35-72 year (DS 9,2088) have been studied with the MMPI-2 test and 3 interviews psychologist-patient. The results have been appraised with the studies coorte, control-case and Sperman’s correlations. The patients have been treated with medical therapy and submitted to weekly sessions, for 9 months, of cognitive-behavioral (all 55) and relationships couples (15 PD and 22 pseudo PD) psychotherapy. Results The test recorded points T raised in 21 subject MP and 11 pseudo MP in the staircases D, Si, D1, D3, D5, Hy2, Hy3, Si1, Anx, Frs, Dep, Hea, Ang, Sod, Trt, Lse and MDS. The uneasiness shows - in the PD patients: RA% 95,94-99,91 and RAP% 91, 21-100 – in the pseudo PD patients: RA% 91,30-93,82 and RAP% 79,38-100. The Sperman’s correlations between the clinical signs and the psychological uneasiness are meaningful in the PD patients p = 0,0017 in the pseudo PD patients p = 0,0044. The communion between the cognitive-behavioral therapy and relationships couples one has improved the uneasiness. Discussion The test and the interviews have underlined in 21 patient PD and 11 pseudo PD, anxiety, depression, loss of the autostima, worry for the health. The state of uneasiness showed a 80,76% incidence in the patients with PD and a 37,93% incidence in the patients with pseudo PD. Conclusions Our experience, develops in 9 months, suggests that in patients with erectile dysfunction, the psychotherapy cognitive-behavioral is the elective treatment of the uneasiness, while the relationships couples psychoterapy is able to prevent the changes brought by the pathology to the dynamics of couple
