    Psoriasis is a chronic, immune-mediated, genetic disorder characterized by skin lesions and musculoskeletal system injuries and affected by external and internal factors. Currently, one of the primary challenges in modern dermatovenereology is managing patients with chronic dermatoses including moderate-to-severe psoriasis. This article presents a case narrative concerning the use of secukinumab, an IL-17A inhibitor, in an HIV-positive patient with severe psoriasis and psoriatic arthritis. The disease course was characterized by significant cutaneous and articular syndromes along with resistance to disease modifying anti-rheumatic therapy. Secukinumab treatment of this patient helped to improve both dermatological and musculoskeletal symptoms. The safety and high efficacy of secukinumab enable complete disease control and improve the quality of life in patients with moderate-to-severe plaque psoriasis emphasizing the need for the use of genetically engineered biological drugs.Псориаз — хроническое иммуноопосредованное, генетически обусловленное заболевание, которое характеризуется поражением кожи и опорно-двигательного аппарата и зависит от внешних и внутренних факторов. На сегодняшний день одной из главных задач в современной дерматовенерологии является лечение пациентов с хроническими дерматозами, в том числе с псориазом среднетяжелой и тяжелой степени тяжести. В статье представлен клинический случай применения препарата секукинумаб, относящегося к группе ингибиторов IL-17А, у ВИЧ-инфицированного пациента с псориазом тяжелой степени тяжести и псориатическим артритом. Течение заболевания характеризовалось выраженным кожным и суставным синдромами, а также резистентностью к базисной противовоспалительной терапии. Применение в лечении данного пациента препарата секукинумаб помогло достичь снижения клинических проявлений со стороны как кожи, так и опорно-двигательного аппарата. Безопасность и высокая эффективность препарата секукинумаб позволяют полностью контролировать течение заболевания и улучшают качество жизни пациентов, страдающих бляшечным псориазом среднетяжелой и тяжелой степени тяжести, а также свидетельствуют о необходимости применения генно-инженерных биологических препаратов

    The contribution of the organization theory to the neoinstitutionnalist conception of the firm: a rereading of the works of O.E. Williamson In this text, the author makes a reflection about the contribution of the organization theory to the neoinstitutionnalist conception of the firm, by proceeding to a rereading of the works of O. Williamson. This borrowing, in contrast to others theories of the firm, permits to highlight numerous aspects of the firm, namely: the contractual nature of the firm, the nature of intrafirm conflict and the organizational dynamic of the firm. Moreover, the author studies the complementarities and the differences between this borrowing and another current of the organization theory, the strategic analysis one. This comparison leads us, as Williamson suggest, to build a "science of organization".L'apport de la théorie des organisations à la conception néo-institutionnelle de la firme.     The effectiveness of self-supervised representation learning in zero-resource subword modeling

    For a language with no transcribed speech available (the zero-resource scenario), conventional acoustic modeling algorithms are not applicable. Recently, zero-resource acoustic modeling has gained much interest. One research problem is unsupervised subword modeling (USM), i.e., learning a feature representation that can distinguish subword units and is robust to speaker variation. Previous studies showed that self-supervised learning (SSL) has the potential to separate speaker and phonetic information in speech in an unsupervised manner, which is highly desired in USM. This paper compares two representative SSL algorithms, namely, contrastive predictive coding (CPC) and autoregressive predictive coding (APC), as a front-end method of a recently proposed, state-of-the art two-stage approach, to learn a representation as input to a back-end cross-lingual DNN. Experiments show that the bottleneck features extracted by the back-end achieved state of the art in a subword ABX task on the Libri-light and ZeroSpeech databases. In general, CPC is more effective than APC as the front-end in our approach, which is independent of the choice of the out-domain language identity in the back-end cross-lingual DNN and the training data amount. With very limited training data, APC is found similar or more effective than CPC when test data consists of long utterances.Accepted author manuscriptMultimedia Computin

    Analytic Solutions of Elastic Tunneling Problems

    The complex variable method for solving two dimensional linearly elastic problems is used to obtain several fundamental analytical solutions of tunneling problems. The method is used to derive the general mathematical representation of problems involving resultant forces on holes in a half-plane. Such problems are encountered in geomechanics during the excavation of tunnels. When tunnels are excavated the removal of the weighted material inside the tunnel causes the ground under the tunnel to rebound. This in turn causes a resultant force, referred to here as the buoyancy force, to act on the tunnel until the stresses return to a state of equilibrium. The mathematical representation derived in this thesis shows that the displacements will usually be unbounded at infinity in solutions of two-dimensional problems involving resultant forces acting on holes in a half-plane. A general solution is derived for a single, circular tunnel in an elastic halfplane that includes a buoyancy force. The boundary conditions in this solution are that the surface of the half-plane is stress-free and that the displacements are specified along the tunnel boundary. This general solution is used in the rest of the thesis as the framework for considering three fundamental tunneling problems: the problem of a rigid, buoyant tunnel, the ground loss problem, and the ovalization problem. The three fundamental solutions are combined to illustrate their possible use as tools for the prediction of settlement troughs. The inclusion of buoyancy effects in the solution is shown to have a possibly considerable narrowing effect on settlement troughs. Neglect of the buoyancy effect might well be a reason why settlement troughs calculated in theory are often wider than those measured in practice.Civil Engineering and Geoscience

    Using cross-model learnings for the Gram Vaani ASR Challenge 2022

    In the diverse and multilingual land of India, Hindi is spoken as a first language by a majority of its population. Efforts are made to obtain data in terms of audio, transcriptions, dictionary, etc. to develop speech-technology applications in Hindi. Similarly, the Gram-Vaani ASR Challenge 2022 provides spontaneous telephone speech, with natural back-ground and regional variations in Hindi. The challenge provides: 100 hours of labeled train-set, 5 hours of labeled dev-set and 1000 hours of unlabeled data-set. For the 'Closed Challenge', we trained an End-to-End (E2E) Conformer model using speed perturbations, SpecAugment techniques and use VTLN to handle any unknown speaker groups in the blind evaluation set. On the dev-set, we achieved a 30.3% WER compared to the 34.8% WER by the Challenge E2E baseline. For the 'Self Supervised Closed Challenge', a semi-supervised learning approach is used. We generate pseudo-transcripts for the unlabeled data using a hybrid TDNN-3gram LM model and trained an E2E model. This is then used as a seed for retraining the E2E model with high confidence data.
