    A faster technique for intraoperative assessment of resection margins in breast cancer

    Objective: Breast cancer is considered the most common cancer among females. Breast-conserving therapy (BCT) is the preferred treatment option for females presented by early breast cancer. BCT is equivalent to modified radical mastectomy as regarding the overall survival. Intraoperative assessment of the lumpectomy margins is required, the best methods are frozen section (FS) and imprint cytology (IC). Methods: This study was conducted on 40 female patients admitted to the Surgical Oncology Unit, Alexandria Main University Hospital in 2017, who were eligible for BCT. Excised specimens of breast conservation surgery were sent to the pathologist for both IC and FS to assess safety margins. The study compared results of IC for the 259 margins with the results of paraffin section for the same number of margins. Results: Sensitivity of IC was 91.35%, and its specificity was 95%. The overall accuracy rate for this method was 94.21%. The sensitivity of FS was 96.91%, and its specificity was 94%. The overall accuracy rate for this method was 94.59%. Conclusion: Breast conservation therapy must be done in the presence of intraoperative safety margin assessment to improve survival and prevent recurrence. Imprint cytology is a fast and accurate method for intraoperative margin assessment in breast cancer. [Arch Clin Exp Surg 2018; 7(4.000): 159-165

    Reliability of Different Surgical Oncoplastic Techniques in the Management of Breast Cancer

    Breast conservative surgery (BCS) followed by radiotherapy has become the standard treatment of breast carcinoma. The cosmetic defects after breast conservation and radiotherapy are technically challenging and the outcome of their correction is disappointing. Oncoplastic surgical procedures evolved with the concept of applying plastic reconstructive techniques to reshape the breast after conservative resection. Thus, breast deformity will be prevented. This work aimed at studying the feasibility of applying different oncoplastic surgical procedures in the treatment of operable breast cancer located in different breast quadrants. Between July 2009 and February 2011, 72 female patients with operable breast cancer were selected to undergo different types of oncoplastic procedures according to the location of the tumor and breast size, with a follow-up period ranging between 32 and 51 months. The application of oncoplastic procedures in different quadrants was associated with an acceptable cosmetic outcome (variable between excellent and good) in 81.9% of the studied patients. The complication rate was 16.7%. During the period of follow-up the incidence of local and systemic failure was 2.8 % and 6.6 %, respectively. As they allow wider excision volumes and better margins without cosmetic penalties, we can conclude that oncoplastic techniques are a valuable option in the armamentarium of a devoted breast surgeon aiming at conserving the breast and maintaining the female's body image.     Plasma long non-coding RNA <i>HOTAIR</i> as a potential biomarker for gastric cancer

    Background: Long non-coding RNAs (lncRNAs) Hox transcript antisense intergenic RNA ( HOTAIR) has been suggested to be implicated in gastric cancer tumorigenesis and progression; however, little is known about the role of the plasma HOTAIR in gastric cancer diagnosis and prognosis. Objective: The current study was aimed at investigating the clinical relevance of plasma long non-coding HOTAIR as a non-invasive diagnostic biomarker in gastric cancer. Methods: Plasma HOTAIR expression was measured in 50 patients with newly diagnosed gastric cancer and 50 age- and sex-matched healthy controls using quantitative reverse transcription polymerase chain reaction. Results: Plasma level of HOTAIR was significantly higher in gastric cancer patients compared with healthy controls ( P &lt; 0.001). By using receiver operating characteristic curve analysis, it was found that plasma HOTAIR could diagnose gastric cancer with 88% sensitivity and 84% specificity. Furthermore, increased HOTAIR expression was associated with advanced tumor stages, higher grades, and metastasis. Conclusion: Plasma HOTAIR might serve as a potential non-invasive biomarker for diagnosis of gastric cancer. </jats:sec

    Wafer classification using support vector machines

    Increasing yield is a primary concern to integrated circuit manufacturing companies as it dictates the readiness of a new process for high volume manufacturing. In order to expedite the process of discovering yield issues, companies have started looking for ways to perform early prediction for such issues. This paper suggests the use of the support vector machines (SVMs) for early wafer classification. The choice of SVM is motivated by the model's ability to effectively classify multivariate, multimodal, and inseparable data points. This model uses multidimensional hyperplanes to separate and classify wafers into low-yield and high-yield classes. This paper includes a proposal for how the classification model can be applied for yield classification and how it can be adaptively updated in a manufacturing environment. We show how the values for the SVM parameters can be selected for best yield classification. Furthermore, performance evaluation is conducted on real manufacturing data, comparing the proposed SVM classifier to state of the art. Results show that in all cases, SVM consistently outperforms other methods with and without adaptive model updates. The experiments also show that all classifiers' performances depend on yield thresholds.     A Systematic Review and Meta-Analysis on the Efficacy and Safety of Concomitant Laparoscopic Cholecystectomy and Sleeve Gastrectomy in Patients with Morbid Obesity

    Background/Objectives: Rapid weight loss after bariatric surgery is linked to a higher risk of symptomatic gallstone disease, which could require a cholecystectomy. This meta-analysis aimed to assess the efficacy and safety of conducting laparoscopic cholecystectomy concurrently with sleeve gastrectomy in morbid obese patients. Methods: Scopus, PubMed, ProQuest, Web of Science, and Google Scholar were searched for this review. Quality assessment was conducted using the Newcastle-Ottawa Scale for observational studies and ROB2 for randomized clinical trials. Eight of thirteen studies were high-quality. Pooling for BMI difference and LOS was used to measure efficacy, and the pooled proportion was utilized to assess safety through bleeding, wound infection, and leakage. Heterogeneity was explained via leave-one-out analysis and meta-regression. Results: The pooled standardized mean difference in BMI peri-operation was 3.26 (95% CI: 2.51&ndash;4.01, p &lt; 0.01), where the age and initial BMI explained 98% of the heterogeneity through meta-regression. The pooled mean of LOS was 3.18 days (95% CI: 2.23&ndash;4.14, I2 = 99%, p &lt; 0.001), where age explained 33.22% of the heterogeneity through meta-regression. The pooled proportion of bleeding was 0.03 (95% CI: 0.02&ndash;0.04, I2 = 2%, p = 0.42). The pooled wound infection was 0.04 (95% CI: 0.02&ndash;0.08, I2 = 80%, p &lt; 0.001), where age accounted for 99% of the heterogeneity. The pooled biliary/gastric leakage was 0.02 (95% CI: 0.01&ndash;0.05, I2 = 0, p = 0.52). Conclusions: These findings indicate that the combined procedures can be executed with an acceptable safety profile. The heterogeneity in outcomes underscores the necessity of personalized patient care, standardized perioperative practices, and continuous research to enhance results and mitigate dangers

    IN-VITRO ANTICOAGULANT EFFECT OF AERIAL PARTS EXTRACTS OF YEMENI FAGONIA SCHWEINFURTHII HADIDI

    Background: Blood coagulation is a quick and effective process that results in the creation of clots, which demands to monitor. Many illness disorders include an abnormality in blood coagulation. This study examined the in vitro effects of methanol, ethyl acetate, and n-hexane extracts from aerial parts of Fagonia schweinfurthii Hadidi on healthy human volunteers' blood coagulation. &#x0D; Methods: The Secondary metabolites were extracted from dried and crushed F. schweinfurthii aerial parts using n-hexane, ethyl acetate, and methanol, respectively. Additionally, the extracts were tested in vitro at different concentrations (10-100 µg/ml) on the blood coagulation profile, prothrombin time (PT), and activated partial thromboplastin time (a PTT) of apparently healthy human volunteers.&#x0D; Results: Methanol, ethyl acetate, and n-hexane extracts of F. schweinfurthii aerial parts significantly (p˃0.05) prolonged PT and PTT in the blood of healthy human volunteers with Ethyl acetate and methanol extracts recorded the largest prolongation of PT and PTT correspondingly.
