A direct A.C. to A.C. regenerative frequency and voltage converter
Includes bibliographical references.The reliable variable speed operation of an a. c. machine over a wide speed range is a problem that has received attention for some time. In this thesis a system to permit such operation from a fixed frequency fixed voltage supply is proposed, under the name of the asynchronous modulation converter. This converter is of the cycloconverter family, but is force-commutated and is not synchronised with the supply frequency. The power switching element comprises a power transistor in a diode bridge, coupled to the control circuitry by opto-isolators. The output of the a-mod converter is produced by a "chopper" modulation process and this results in the harmonics present being at frequencies well removed from the fundamental. A feedback system to ensure smooth commutation of the output current from the power switches is described. This feedback system operates by sensing the rate of rise of the inductive voltage surge on the load when a switch is opened to operate a freewheel path switch

Hamilton Stewart Nephroplasty
In 1947, Hamilton Stewart described a method of dealing with those patients with hydronephrosis in whom lower polar vessels were a factor in the ureteropelvic obstruction. Subsequent reports in the British literature indicated that other surgeons accepted the procedure. The operation has been almost completely disregarded in North America. We present our experience with the Hamilton Stewart nephroplasty because, in selected, cases, it fulfills the ideals of surgery in the relief of obstruction at the ureteropelvic junction associated with lower polar vessels

Augmentative transcranial magnetic stimulation (TMS) combined with brain navigation in drug-resistant rapid cycling bipolar depression : a case report of acute and maintenance efficacy
The efficacy of transcranial magnetic stimulation (TMS) has been poorly investigated in the acute and maintenance treatment of bipolar depression. The present case supports the efficacy of low-frequency repetitive TMS (rTMS) of the right dorsolateral pre-frontal cortex (RDLPFC) combined to brain navigation in a drug-resistant, bipolar depressed subject with rapid cycling. While continuing the pharmacological treatment at stable doses, the patient was stimulated for 3 weeks at 1 Hz, 110% of motor threshold, 300 stimuli/day showing a significant improvement on the Hamilton Depression Rating Scale (HDRS(21)), the Montgomery-Asberg Depression Rating Scale (MADRS) and the Clinical Global Impression, improvement scale (CGI-I) total scores. On completion of the 3-week rTMS, the patient was treated with periodic maintenance sessions of rTMS at the same parameters of acute phase for an additional 6 months, at the end of which the therapeutic gains were maintained according to rating scales scores. Larger controlled trials assessing the acute and maintenance efficacy of rTMS in bipolar depression are needed
