    Nello spirito operativo del volume è interessante sottolineare due aspetti: il vuoto come agente critico nei confronti della ideazione formale del progetto; la riflessione sul vuoto come pienezza del progetto. According to the operating spirit of the book it is interesting to point out two aspects: the void as critical agent against formalism in design process; reflection on empty as fullness of the project. Entrambi i temi sono presenti nel Crystal Palace, prima versione, epifania del vuoto nella architettura europea industriale. L’edificio nasce da una crisi dei linguaggi storicisti e dall’emergere di una cultura progettuale – i costruttori di serre – estranea alla storia delle forme architettoniche e che utilizza il principio costruttivo antifigurativo del montaggio a secco. La comparsa di una “substantial manner” nel cuore di Londra rende evidente un sistema di qualità architettoniche legate al vuoto: il mito del Palazzo di Cristallo, prototipo di una architettura liberata dal limite umano; la possibilità di “plasmare” non solo la materia solida, ma l’aria stessa. L’aspetto che, tuttavia, rimanda più direttamente alla idea del vuoto come pienezza del progetto è la costruzione di uno spazio individuato ma che non vincola la collocazione degli oggetti e non determina univocamente il movimento delle persone o le loro azioni. Both themes are present in the Crystal Palace, early version, the epiphany of the vacuum in European industrial architecture. This building comes from a crisis of historicist languages and by the emergence of a design culture - the builders of glasshouses - alien to the history of architectural forms and which uses the constructive and anti-figurative principle of the dry assembly. The appearance of a "substantial manner" in the heart of London shows a system of architectural qualities related to empty: the myth of the Crystal Palace, the prototype of an architecture liberated from human limitation; the option of "shaping" not only the solid matter, but the air itself. The aspect which, however, refers more directly to the idea of emptiness as fullness of the project is the construction of a space that is identified but not constraints placing objects and not uniquely determine the movement of people or their actions. La crisi dei grandi linguaggi rifondativi messi a punto dai maestri del primo Novecento che segue la Seconda Guerra mondiale riporta il tema del vuoto ai suoi compiti di critica della forma e pienezza del progetto. The crisis of the great re-founding languages developed by the masters of the early twentieth century, following the Second World War, reports the theme of emptiness to his task of critique of the shape and fullness of the project. La riflessione sulla crisi del modello creativo dell’architetto artista che Fernando Távora pone al centro della sua ipotesi di una “terza via” tra tradizionalismo salazariano e internazionalismo modernista porta alla “visione” di una architettura in cui vuoto e pieno sono un “continuum”. The reflection on the crisis of the creative model represented by architect-artist that Fernando Távora put at the center of his idea of a "third way" between the salazarian traditionalism and the modernist internationalism leads to the "vision" of an architecture in which emptiness and fullness are a "continuum ". Se ci spostiamo nell’Italia dell’immediato secondo dopoguerra ritroviamo il vuoto come agente critico in diversi ambiti di ricerca. If we move in Italy in the immediate post-war period we find the vacuum as a critical agent in several research fields. Il vuoto come critica della forma e pienezza del progetto è tra i motivi centrali nelle architetture che commemorano l’esperienza della deportazione politica e razziale. Le Fosse Ardeatine si caratterizzano per un “vuoto” di forma di potente assolutezza, la “pietra tombale” in cemento che rappresenta, o testimonia la non rappresentabilità, della componente “immemorabile” dell’eccidio e, sovvertendo il principio del monumentale, aprono il luogo della commemorazione – le cave – a un percorso in cui il visitatore è chiamato a costruire, con un notevole grado di libertà, il proprio cammino di comprensione in un atteggiamento conoscitivo e non contemplativo. Il Museo-Monumento al Deportato Politico e Razziale di Carpi si incentra anch’esso sul principio di un libero percorso, la “misura del respiro” del visitatore, in un luogo, un castello ricco di memorie storiche stratificate e altre, in cui tutto l’impegno progettuale è volto a costruire il vuoto di una “pagina bianca” su cui costruire il racconto della deportazione. The void as a critique of form and fullness of the project is among the central themes in those architectures that commemorate the experience of racial and political deportation. The Fosse Ardeatine are characterized by a "void" in shape with powerful absoluteness, the "tombstone" made of cement that represents, or witness the non representativeness, the "immemorial" component of the massacre and, subverting the principle of monumental, they open the site of the commemoration - the cave - to a path where the visitor is called to build, with a considerable degree of freedom, its own journey of understanding in a cognitive (not contemplative) attitude. The Museo-Monumento al Deportato Politico e Razziale (Museum-Monument to Political and Racial Deported) in Carpi also focuses on the principle of a free path, the "measure of the breath" of the visitor, in a place, a castle rich in historical and stratified memories, where all the design effort is aimed at building the void of a "blank page" on which to build the story of the deportation. Anche la relazione tra autorialità architettonica e Anonimo, che il vuoto come costruzione di uno spazio relazionato e non ricomposto in unità pone, trova pieno sviluppo nell’Italia dell’immediato dopo guerra. Si pensi solamente a Leonardo Ricci e alla sua riflessione sull’ Anonimo del Ventesimo Secolo (1962). Also the relationship between architectural authorship and Anonymous, that the void as a construction of a space related and not recomposed in unity offers, is fully developed in Italy in the immediate post war. Consider Leonardo Ricci and his reflection on Anonimo del Ventesimo Secolo (Anonymous of the Twentieth Century) (1962). Certamente l’uso critico del vuoto, l’idea di una architettura il cui obiettivo non consiste nella costruzione di un oggetto architettonico da contemplare ma nella capacità di raccordare e radunare presenze – animate e inanimate – non è risultato una linea vincente nella architettura italiana, soprattutto trascorso il ventennio fortemente sperimentale che segue la fine della guerra. Ma le recenti ricerche architettoniche testimoniate nelle pagine che seguono mostrano intatta la vitalità del vuoto come agente critico con obiettivi non differenti dai casi storici sommariamente richiamati. Certainly the critical use of the void, the idea of an architecture whose goal is not the construction of an architectural object to be contemplated but in the ability to connect and gather attendance - animate and inanimate - was not a winning line in Italian architecture, especially after the highly experimental twenty years following the end of the war. Despite this some current architectural researches presented in the following pages show how intact is the vitality of the void as a critical agent with objectives which are not different from those historical cases summarily recalled

    Plasma triglycerides predict ten-years all-cause mortality in outpatients with type 2 diabetes mellitus: a longitudinal observational study

    BACKGROUND: Cardiovascular disease (CVD) is the leading cause of death in type 2 diabetes mellitus (T2DM). American Diabetes Association standards of care set a series of targets recommended for the CVD prevention: blood pressure, LDL and HDL cholesterol (LDL-C and HDL-C), triglycerides and HbA1c goals. The aim of this study was to evaluate cardiovascular risk factors in a T2DM outpatient population in order to estimate their specific clinical value in predicting long-term overall mortality. METHODS: Our study population was composed of 1917 T2DM outpatients attending the hospital-based Diabetes Clinic of Ferrara for a mean follow-up period of 10 years; recorded information included personal, clinical and biochemical data, and pharmacological treatment. RESULTS: A Cox proportional hazard analysis was performed, pointing out as age (HR:1.08; IC95%: 1.06-1.11), sex (males: HR:1.97; IC95%: 1.26-3.07), mean triglycerides levels during follow-up (III vs I tertile: HR:1.87; IC95%: 1.12-3.12) and lipid-lowering treatment (HR:0.56; IC95%: 0.35-0.90) were significantly associated with all-cause mortality, independent of confounding factors such as mean values of LDL-C, HDL-C, HbA1c, blood pressure, BMI, fasting glucose, and antihypertensive and antidiabetic treatment. CONCLUSIONS: This finding suggests that more attention should be given to the management of cardiovascular risk in type 2 diabetic patients with high triglycerides levels

    Acute monoparesis onset in recent aortic valve replacement: A case report

    Vertebral metastases are frequent in patients with cancer. They are much more frequent in higher age groups (> 50 years); the lesions can be asymptomatic despite a setting of widespread metastatic disease and may become symptomatic due to bone pain, pathological compression fractures, or extension into spinal canal with cord compression ensuing neurological symptoms. We report the case of a patient, without known history of malignancy, in which shoulder pain was attributed for few months to sequaela of recent cardiac surgery for ascending aortic aneurysm. She was admitted to our ward because of acute onset of lower limb monoparesis which evolved in several days in paraparesis. Only a dorsal magnetic resonance study revealed the presence of a bulky vertebral lesion at D1-D2 level involving the peri-dural space. Neuro-surgical decompression was performed obtaining specimens for histological analysis, which suggested the presence of metastatic adenocarcinomatous lesions of gastrointestinal origin. A computed tomography study partially supported this hypothesis showing only a thickening of rectal wall, even if endoscopic exploration did not show macroscopic mucosal abnormalities. Surgical and medical therapies did not improve the patient’s clinical course and she died few months later

    Gene expression profiling of gluteal adipose tissue after prolonged bedrest

    Adipose tissue is a complex and highly active metabolic and endocrine organ, with an important role in immune and inflammatory processes. Its dysfunction has been linked to development of insulin resistance, type 2 diabetes and cardiovascular disease. A sedentary lifestyle is associated to a quantitative and qualitative alteration of adipose tissue mass and function. Bed rest has been utilized as an experimental model to simulate physical inactivity. The aim of this study was to investigate the impact of physical inactivity on adipose tissue gene expression in healthy subjects. A total of 7 healthy subjects underwent gluteal adipose tissue biopsies before and after 14 days of bed rest; RNA was isolated and hybridized on RNA microarray chips in order to detect gene expression differences. A total of 308 genes were differently expressed after bed rest intervention; these genes were analyzed with bioinformatics database support (DAVID gene functional classification tool), to find potential functional-related gene groups and gene-disease associations. In silico analysis showed that 14 genes in our list have been previously involved in cardiovascular diseases, 8 genes in metabolic disorders and 12 genes in both. Among these, UCP3 (Uncoupling Protein 3) and BMP7 (bone morphogenetic protein 7) have been shown to be involved in energy expenditure and adipose tissue “browning”; TTR (transthyretin) is a known transport protein that carries the thyroid hormones (T4) and retinol-binding protein bound to retinol.
