Fatigue assessment and its predictors in pediatric patients with chronic kidney disease stages III to V
No full textAbstract Background Chronic fatigue is an intense subjective feeling of mental or physical exhaustion. It influences patients’ daily functioning and quality of life, delays recovery, and increases mortality, especially in chronic kidney disease (CKD) patients. The aim of this study is to assess and determine predictors of chronic fatigue in children with ESRD, dialysis, and pre-dialysis stages that can affect the patients’ quality of life (QOL). Methods We conducted a cross-sectional study on 114 patients diagnosed with CKD stages III to V, following at the nephrology outpatient clinic and hemodialysis (HD) unit of Cairo University Children’s Hospital during the period September 2020 till April 2021. Demographic and laboratory data of patients were gathered, and dialytic analysis in the form of frequency, duration of dialysis sessions, and adequacy of hemodialysis was calculated. The fatigue severity score (FSS) questionnaire was used to assess fatigue’s effects on daily functions, querying its relationship to motivation, physical activity, work, family, and social life. Results The mean age in the current study was (8.8 ± 1.8) years, with 62% being males. The median FSS score was 5.8, with a higher FSS score in stage V CKD patients. High e-GFR, serum sodium, folic acid, and L-carnitine supplementation all reduced the intensity of fatigue, while prolonged HD duration, acidosis, hypertension, and non-compliance to vitamin D replacement increased tiredness severity. Conclusion Routine fatigue assessment and measures to reduce it is a fundamental issue in pediatric CKD patients for better QOL
A comparative study of multilingual Pakistanis in Amsterdam and Birmingham

Amsterdam and Birmingham. It seeks to show that the research subjects' use of the many languages and
language varieties at their command is neither random nor illogical, but rather, aids both community formation in the diaspora and the attainment of individual goals. This attainment of goals involves a use of language which may be pragmatic or affective but, particularly when addressing a heterogeneous audience, is often both simultaneously. The primary importance of context is illustrated. This context is both transnational, in the sense that the Pakistani migrant-settlers studied form part of a world-wide diaspora, and bounded by conditions in the two European metropolises of Amsterdam and Birmingham. There is also a local context, defined by the particular areas of town where most of the respondents lived. Finally, there is a context of power relationships operating through community networks, where gender and religion, in this case Islam, play a significant role.
The Introduction discusses some historical and political aspects of language issues in the Indian subcontinent
and their continuing influence. The first part of the Literature Review and much of Chapter Four, which examine previous studies of the Pakistani communities in England and the Netherlands and give background information respectively, concentrate more on the influence of place than time. Chapter Four also contains a discussion of how the concept of "diaspora" may be applied to the Pakistanis here studied. Many of the languages spoken by the respondents form part of a continuum: hence, the repeated use of terms such as "the Punjabi family of languages", or, "the Pahari group". These and other terms are defined in the Methodology section, which is Chapter Three.
Issues of methodology are not restricted to Chapter Three but are also strands which run through the whole work. The highly multilingual nature of the research subjects rendered some techniques favoured by sociolinguists, such as matched guise tests, inappropriate. The mobility of many individuals posed both practical and analytical difficulties. Some questions of particular social relevance to the group studied influenced research design, such as the separation of the sexes and, in Amsterdam, the presence of undocumented workers in the community. Language issues which are of great concern to researchers in Pakistan and northern India have less relevance in the Netherlands and England, and the reverse is also true. This became apparent to the researcher through conversations with scholars in Pakistan during her fieldwork visit there. More co-operation by scholars in the regions of origin and in the regions of settlement on the interface between ethnicity and language is needed. Any such collaborative research could have important applications in the areas of language planning and language rights.
Chapter Five discusses findings from the individual interviews and Chapter Six deals with the analysis
of the observation of group interactions. They are separate sections for clarity of exposition but this separation is artificial. All of the interviewees also participated in at least one and typically several of the group events. Hence, it was possible to observe and analyse the differences and similarities between individuals' reported and actual language use, and how individuals, with their distinct histories and statuses contribute to and benefit from group multilingual encounters.
The Conclusion shows how the Pakistani communities studied - and the researcher believes this must be true of many other migrant groups - came from multiethnic, multicultural and multilingual societies and settled in similarly heterogeneous environments in Europe. Hence, far from suffering crises as a result of being "between two cultures", they were able to use tried and tested strategies in the new environment to their advantage. The Informal Economy in Lebanon: Dangers and Benefits
Get PDFThis thesis investigates through situational and empirical analysis the beneficial and detrimental characteristics of the informal economy in Lebanon and its impact on the public and state interest through its socio-economic associations. The informal economy is a polymorphous entity, and in order to determine its versatile contribution, has been separated into four different variables or key drivers. The four key drivers constitute the determinant variables of the informal economy.
The main method used to explore the four determinant variables is the Force Field analysis. The first key driver is informal remittances as they represent a significant fraction of the informal economic activity in Lebanon and make a major contribution to Gross Domestic Product. The second driver is corruption, as this involves massive economic transactions on a daily basis, with an enormous impact at both the microeconomic and macroeconomic levels. The third key driver is the informal employment and commerce sector, as the unpaid VAT and corporate and income tax evasion results in a huge annual loss of government revenue in Lebanon. The fourth and last key driver is the illegal networks, as Lebanon represents an international hub for smuggling, drug and people trafficking and money-laundering, with strong links with, and implication for, its sectarian constituents. The current situation in Lebanon fuels the creation of a chaotic socio-economic environment where it is impossible to estimate accurately the significance of the informal economy, or indeed the size of the overall economy.
As far as possible in this research all the key drivers have been independently and collectively evaluated through the data collected from the primary sources (users/public opinion, government officials and academics) and secondary material in order to assess each key driver’s input to the informal economy. Subsequently the thesis provides an estimation of the beneficial and detrimental contribution of the informal economy in Lebanon, as well as the overall perceptions of each of the respondent groups. Lastly, the primary and secondary materials are collectively assessed from a single perspective to build, using an inductive approach, a theoretical model of the factors which fuel and perpetuate the informal economy in the country. The present thesis may constitute the foundation for future analysis of the informal economy in Lebanon by providing unconventional recommendations. It is an attempt to present the possibility of an alternative approach to the informal economy, by stressing its merits and advantages, while also recognising the dangers and challenges it poses for both the state and the society

No full textBackground: This study assessed the potential cost-effectiveness of high (80–100%) vs low (21–35%) fraction of inspired oxygen (FiO2) at preventing surgical site infections (SSIs) after abdominal surgery in Nigeria, India, and South Africa. Methods: Decision-analytic models were constructed using best available evidence sourced from unbundled data of an ongoing pilot trial assessing the effectiveness of high FiO2, published literature, and a cost survey in Nigeria, India, and South Africa. Effectiveness was measured as percentage of SSIs at 30 days after surgery, a healthcare perspective was adopted, and costs were reported in US dollars (216 compared with 6 (95% confidence interval [CI]: −1) difference in costs. In India, the average cost for high FiO2 was 195 for low FiO2 leading to a −15 to −1164 compared with 93 (95% CI: −65) difference in costs. The high FiO2 arm had few SSIs, 7.33% compared with 8.38% for low FiO2, leading to a −1.05 (95% CI: −1.14 to −0.90) percentage point reduction in SSIs. Conclusion: High FiO2 could be cost-effective at preventing SSIs in the three countries but further data from large clinical trials are required to confirm this. © 2023 The Author
Global age-sex-specific mortality, life expectancy, and population estimates in 204 countries and territories and 811 subnational locations, 1950–2021, and the impact of the COVID-19 pandemic: a comprehensive demographic analysis for the Global Burden of Disease Study 2021
No full textBackground Estimates of demographic metrics are crucial to assess levels and trends of population health outcomes. The profound impact of the COVID-19 pandemic on populations worldwide has underscored the need for timely estimates to understand this unprecedented event within the context of long-term population health trends. The Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2021 provides new demographic estimates for 204 countries and territories and 811 additional subnational locations from 1950 to 2021, with a particular emphasis on changes in mortality and life expectancy that occurred during the 2020–21 COVID-19 pandemic period. Methods 22¿223 data sources from vital registration, sample registration, surveys, censuses, and other sources were used to estimate mortality, with a subset of these sources used exclusively to estimate excess mortality due to the COVID-19 pandemic. 2026 data sources were used for population estimation. Additional sources were used to estimate migration; the effects of the HIV epidemic; and demographic discontinuities due to conflicts, famines, natural disasters, and pandemics, which are used as inputs for estimating mortality and population. Spatiotemporal Gaussian process regression (ST-GPR) was used to generate under-5 mortality rates, which synthesised 30¿763 location-years of vital registration and sample registration data, 1365 surveys and censuses, and 80 other sources. ST-GPR was also used to estimate adult mortality (between ages 15 and 59 years) based on information from 31¿642 location-years of vital registration and sample registration data, 355 surveys and censuses, and 24 other sources. Estimates of child and adult mortality rates were then used to generate life tables with a relational model life table system. For countries with large HIV epidemics, life tables were adjusted using independent estimates of HIV-specific mortality generated via an epidemiological analysis of HIV prevalence surveys, antenatal clinic serosurveillance, and other data sources. Excess mortality due to the COVID-19 pandemic in 2020 and 2021 was determined by subtracting observed all-cause mortality (adjusted for late registration and mortality anomalies) from the mortality expected in the absence of the pandemic. Expected mortality was calculated based on historical trends using an ensemble of models. In location-years where all-cause mortality data were unavailable, we estimated excess mortality rates using a regression model with covariates pertaining to the pandemic. Population size was computed using a Bayesian hierarchical cohort component model. Life expectancy was calculated using age-specific mortality rates and standard demographic methods. Uncertainty intervals (UIs) were calculated for every metric using the 25th and 975th ordered values from a 1000-draw posterior distribution.Research reported in this publication was supported by the Gates Foundation, UK Department of Health and Social Care, the Norwegian Institute of Public Health, and the New Zealand Ministry of Health. Validation of the OAKS prognostic model for acute kidney injury after gastrointestinal surgery

Published by Oxford University Press on behalf of BJS Society Ltd.Background: Postoperative acute kidney injury (AKI) is a common complication of major gastrointestinal surgery with an impact on short- and long-term survival. No validated system for risk stratification exists for this patient group. This study aimed to validate externally a prognostic model for AKI after major gastrointestinal surgery in two multicentre cohort studies. Methods: The Outcomes After Kidney injury in Surgery (OAKS) prognostic model was developed to predict risk of AKI in the 7 days after surgery using six routine datapoints (age, sex, ASA grade, preoperative estimated glomerular filtration rate, planned open surgery and preoperative use of either an angiotensin-converting enzyme inhibitor or an angiotensin receptor blocker). Validation was performed within two independent cohorts: a prospective multicentre, international study (‘IMAGINE’) of patients undergoing elective colorectal surgery (2018); and a retrospective regional cohort study (‘Tayside’) in major abdominal surgery (2011–2015). Multivariable logistic regression was used to predict risk of AKI, with multiple imputation used to account for data missing at random. Prognostic accuracy was assessed for patients at high risk (greater than 20 per cent) of postoperative AKI. Results: In the validation cohorts, 12.9 per cent of patients (661 of 5106) in IMAGINE and 14.7 per cent (106 of 719 patients) in Tayside developed 7-day postoperative AKI. Using the OAKS model, 558 patients (9.6 per cent) were classified as high risk. Less than 10 per cent of patients classified as low-risk developed AKI in either cohort (negative predictive value greater than 0.9). Upon external validation, the OAKS model retained an area under the receiver operating characteristic (AUC) curve of range 0.655–0.681 (Tayside 95 per cent c.i. 0.596 to 0.714; IMAGINE 95 per cent c.i. 0.659 to 0.703), sensitivity values range 0.323–0.352 (IMAGINE 95 per cent c.i. 0.281 to 0.368; Tayside 95 per cent c.i. 0.253 to 0.461), and specificity range 0.881–0.890 (Tayside 95 per cent c.i. 0.853 to 0.905; IMAGINE 95 per cent c.i. 0.881 to 0.899).
