The impact of body mass index on post resuscitation survival after cardiac arrest: A meta-analysis
Background: Observational studies examining the association between body mass index (BMI) and the outcome of cardiac arrest (CA) shows controversial results. Methods: We reviewed literature for studies assessing the impact of BMI on survival and neurological outcome following CA. Eligible studies were subsequently meta-analyzed and pooled odds ratios and their corresponding 95% confidence intervals for post CA survival and neurological status were derived. Results: A total of 7 studies with 24,651 patients were evaluable for this meta-analysis. The studies were also categorized by location of the CA and the use of therapeutic hypothermia. Our results suggested that BMI between 25 and 29.9 kgr/m2had a favorable impact on survival after CA (OR = 1.172, 95% CI, 1.109-1.236) in comparison to normal weight subjects. Likewise, overweight patients presented increased odds for a favorable neurological outcome after CA (OR = 1.112, 95% CI, 1.020-1.213). On the contrary, underweight subjects presented decreased odds of surviving after CA as compared to normal BMI subjects (OR = 0.781, 95% CI, 0.652-0.935). Finally, BMI >30 kgr/m2was not associated with improved survival or neurological outcome as compared to BMI 18.5-24.9 kgr/m2. Conclusions: Overweight patients have a favorable prognosis after CA in terms of both survival and neurological outcome. This effect was amplified when the analysis is restricted in in-hospital cardiac arrest and in patients non-treated with therapeutic hypothermia
Organization and legal basis of Karlis Ulmanis regime
Šajā darbā tiek pētīta 1934. gada 15. maija valsts apvērsuma rezultātā radušās autoritatīvās valsts pārvaldes formas radītās pārmaiņas, to iespējamā nepieciešamība un tiesiskais pamatojums. Tiek secināts, ka Kārļa Ulmaņa un viņa līdzgaitnieku darbībai apvērsumā nebija tiesiska pamatojuma un tā bija krimināli sodāma saskaņā ar 1933. gada 24. aprīļa Soda likumu. Darba gaitā tiek izdarīti secinājumi par Kārļa Ulmaņa autoritārās varas sekām un to ietekmi uz Latvijas valsti un latviešu tautu, tostarp secinot, ka pēc 1936. gada 16. aprīļa Latvijas tautai vairs nepastāvēja iespēja ietekmēt likumdošanu un tā ir maldināta par tās iespējām ietekmēt likumdošanu, izmantojot Kameru sistēmu. Tiek vērsta uzmanība valsts varas pārņemšanas procesam un autoritatīvā vadītāja Kārļa Ulmaņa lomai jaunajā valsts iekārtā, kā arī procesam, kā tika organizēta jaunā valsts vara, secinot, ka tautas atbalsta gūšanai izmantoti neesoši draudi par pilsoņu kara izcelšanos, un ka tobrīd pastāvošās atsevišķās radikālās organizācijas patiesībā neradīja draudus demokrātiskas Latvijas pastāvēšanai. Tiek identificēti konkrēti cilvēki, kuri bija atbildīgi par valsts varas dalīšanas principa pārtraukšanu, apvienojot lēmējvaru un izpildvaru. This study determines that the actions of Karlis Ulmanis and his accomplices did not have legal justification and were criminally punishable according to the April 24, 1933 Penalty law. The author draws conclusions about the consequences and influence of Karlis Ulmanis authoritarian rule on the Latvian state and Latvian people, and finds that after April 16, 1936 citizens of Latvia no longer had any effective influence on the legislation of Latvia and were misled about their influence over legislation through the Camera system. The study examines the process of how the coup took place and how the new state power was organized and finds that the usurpers used nonexistent threats of a civil war break-out in order to gain public support, and that the few existing radical organizations did not pose a substantial threat to the existence of a democratic Latvian state. Additionally, individuals are identified that were responsible for the abolishment of the division of executive and legislative powers.
