    Lettere diplomatiche e lettere familiari: alle origini dell’epistolario di Baldassarre Castiglione

    Baldassarre Castiglione (1478-1529), uno dei letterati cinquecenteschi più noti della penisola come autore del Libro del Cortegiano, ricoprì numerosi incarichi diplomatici: oratore dei marchesi di Mantova e poi dei duchi d’Urbino, infine nunzio in Spagna presso l’imperatore Carlo V tra il 1524 e il 1529, anno della sua morte. Nell’arco della sua vita e in virtù del suo ruolo di ambasciatore, Castiglione produsse un’ingente quantità di lettere che abbracciava un arco cronologico di oltre trent’anni (1497-1529). Si tratta di una fonte straordinaria, che costituisce una miniera di informazioni, di natura sia pubblica sia privata, e segue pressoché l’intera parabola biografica e professionale di un personaggio che attraversò, con ruoli da protagonista in ambito politico, culturale, artistico, alcuni decenni cruciali della storia dell’Italia e dell’Europa moderna.Questo articolo si propone di ricostruire la vicenda travagliata del progetto di pubblicazione dell’epistolario di Castiglione negli anni ’70-’80 del XVI secolo e di indagare le diverse ragioni del suo esito fallimentare tra interessi privati, esigenze morali e censure di Stato.Baldassarre Castiglione (1478-1529), one of the 16th-century best known man of letters and author of the Book of the Courtier, held many diplomatic posts: orator of the marquises of Mantua and later of the dukes of Urbino, and finally, nuntius in Spain to Emperor Charles V between 1524 and 1529, the year of his death. During his life and by virtue of his role as ambassador, Castiglione produced an enormous quantity of letters that spanned more than thirty years (1497-1529). This is an extraordinary source, a mine for both public and private information covering nearly the entire personal and professional trajectory of a figure who played a leading role in the fields of politics, culture and art during several decades in the history of Italy and modern Europe.The article aims to reconstruct the complicated story of the project to publish Castiglione’s correspondence during the 1570s and 1580s and the reasons why it failed, which included private interests, moral expectations and State censorship

    Arrhythmias in COVID-19

    Coronavirus pandemic has led to the accumulation of substantial evidence about the arrhythmogenic potential of COVID-19. Arrhythmias in COVID-19 develop more frequently in severely and critically ill patients. Hypoxia, direct or indirect myocardial injury as well as the role of immune response to the virus are among the possible mechanisms of arrhythmia. Although management of these arrhythmias during the pandemic does not differ signifi cantly from the currently accepted standard of care, the potential for drug interactions and the impact of some antiviral medications on the QTc interval should be taken into consideration. The current review aims to summarize the available evidence and provide practical guidance on the management of arrhythmia patients with COVID-19

    Percutaneous coronary interventions of unprotected left main stenosis

    Significant stenosis of the trunk of the left coronary artery (LCA) or left main (LM) stenosis is a serious injury that puts a large myocardial area at risk, being found in 4-9% of patients referred for selective coronary angiography. Revascularization in patients with severe left main stenosis significantly improves their prognosis, with bypass surgery being the standard of care. Percutaneous coronary intervention (PCI) has proven to be an effective and safe alternative, especially in patients with less complex lesions or unsuitable for surgery. The aim of this article is to review the topic and to analyze the frequency of adverse cardiovascular events in a two-year follow-up of patients with PCI of unprotected LM stenosis from real practice. Material and methods: Since March 2013, all patients with an interventional procedure for LM stenosis in the Cardiology Clinic of Acibadem City Clinic Tokuda Hospital are included in a register. For the period from 06.2013 to 06.2018, 209 PCIs of LM lesions were performed, and in the present study a series of 136 consecutive patients with PCIs of unprotected LM stenosis were analyzed. The mean Syntax Score (SS) of the study group was 26.3 ± 10.09 (11-58.5) with a median of 24.00. The mean Euro Score is 2.61 ± 4.19 (0.50-34.42) with a median of 1.22. Maximum revascularization was achieved in all subjects, and patients with STEMI and/or cardiogenic shock, previous LM PCI and protected LM PCI were excluded from the analysis. Patients were followed for 2 years for the combined indicator of adverse cardiovascular events, which included death from any cause (so-called overall mortality), cardiac mortality, stroke and revascularization. Results: 22 adverse events were registered (16.2%). The overall mortality was 9.6%, cardiac death – 8.8%, stroke – 0.7% and restenosis and revascularization – 8.1%. Conclusion: PCI of LM stenosis in patients from real clinical practice and achieving maximum revascularization is a procedure with minimal risks and favorable follow-up results, and the operator‘s experience and the capabilities of the respective medical institution to conduct such complex interventions are important

    Revascularization in patients with severe stenosis of left coronary artery (LCAS) trunk signifi cantly improves their prognosis. Modern clinical studies, registries and meta-analyses have identifi ed percutaneous coronary intervention (PCI) of LCAS as a safe alternative to aorto-coronary bypass (ACB) in patients with low and intermediate lesion complexity. Aims: To confi rm the safety and effectiveness of PCI and implantation of second generation drug eluting stent (DES) in patients with unprotected LCAS and concomitant complex coronary pathology. Material and Methods: For the period March 2013–October 2018 we performed 225 PCIs of patients with LCAS. 170 of patients who received PCI were divided into 2 groups, according to their SS-1 (ST elevation excluded). We analyzed the major adverse cardio-vascular events (MACE – all-cause mortality, cardiac mortality, stroke and ischemia driven TLR) rate and time-to-fi rst MACE during follow up. Results: 103 patients had SS-I < 32 and 67 patients had SS-I ≥ 32 and their mean age was 67,25 ± 11,03. The median follow-up was 26,6 ± 19,1 months. MACE rate was 12,4% and there was no signifi cance between groups (p = 0,118). Conclusions: PCI of unprotected LCAS has high procedural success rate and good mid-term results, even in pts with complex anatomy. High anatomical complexity of coronary lesions defi ned by SS-I ≥ 32 is not predictive for poor clinical outcome after PCI

    As part of a larger project on the history of relations between Christians and Muslims in the modern age, this essay aims to shed light on the figure and work of Francesco Sansovino (Rome 1521- Venice 1583), writer, translator and curator of texts as well as founder of print shop which over a period of roughly twenty years published more than fifty works. Sansovino was author of two texts (Historia universale, 1560-61 and Annali turcheschi 1571) which deal with the history, customs, culture and religion of the Ottoman civilization and which enjoyed a wide circulation. The essay aims to show how within the panorama contemporary publications these works of Sansovino’s were intended, above all, to disseminate knowledge about the Ottoman Turks: an approach that was quite unlike that of ideological confrontation against the infidels following the footsteps of Paolo Giovio’s Commentario de le cose de’ Turchi , a work dedicated to emperor Charles V and published in 1531 and which served as Sansovino’s main source and model

    Successful selective arterial thrombolysis in patient with acute abdominal thromboembolism

    AbstractThe paper reports successful thrombolysis conducted in 64 years old woman admitted to the clinic with clinical and angiographic data for acute surgical abdomen caused by acute tromboembolia of arteria mesenterica superior (AMS). The therapeutic approach required to undertake lifesaving decision on i.e. surgical vs. invasive treatment in conditions of emergency. Finally, it was decided to undertake invasive treatment with successful restoration of blood flow in the related artery. The patient was discharged from the clinic with considerable clinical improvement on the fifth day of her stay. The case report includes discussion on issues relating the consequence of the diagnostic and interventional procedures in such patients, opportunities for conducting emergency thrombolysis in acute embolia of AMS and preventive measures in patients with high tromboembolic risk
