    Digital videomicroscopy and image analysis with automatic classification for detection of thin melanomas

    The aim of our investigation was to evaluate the usefulness of a system composed of a digital videomicroscope equipped with a dedicated program for the quantitative characterization of various parameters of the clinically significant features of pigmented skin lesion (PSL) images, forming the basis for automatic differentiation of naevi and thin melanomas. In total 424 naevi and 37 melanomas (including 23 thinner than 0.75 mm) were considered. All the digital images were acquired, framed and analysed using the DBDermo-MIPS program (Biomedical Engineering DeIl ́Eva-Burroni), which calculates different parameters related to the geometry, the colour distribution and the internal pattern of the lesion. We also assessed the efficacy of an automatic classifier, trained for 100% sensitivity using a subset of PSL images (59 naevi and 19 melanomas), on a test set including 365 naevi and 18 melanomas thinner than 0.75 mm. Significant differences between values from benign and malignant PSLs were observed for most of the numerical parameters. Values from the training set underwent elaboration by means of multivariate discriminant analysis, enabling the identification of variables that are Important for distinguishing between the groups in order to develop a procedure for predicting group membership for new cases (test set) in which group membership is undetermined. Going on the training set data, a threshold score was established, enabling each melanoma to be attributed to the right group. When the same threshold value was employed for discriminating between benign and malignant lesions in the test set, all the melanomas were correctly classified, whereas 30 out of the 365 benign lesions were attributed to the wrong group. Thus the specificity of the system reached 92%, whereas the sensitivity was 100%. Our data suggest that elaboration of videomicroscopic images by means of dedicated software improves diagnostic accuracy for thin melanoma. Since elaboration of an image requires only 60s using our system, all the parameter data are available in real time and can be immediately examined by the classifier, providing an instant aid to clinical diagnosis

    Assessment Of Topical Corticosteroid Activity On Experimentally Induced Contact Dermatitis - Echographic Evaluation With Binary Transformation And Image-Analysis

    A new echographic evaluation method employing a B scanner and a dedicated software (Dermavision 2D, Cortex Technology, Hadsund, Denmark) was used in assessing the potency of three different corticosteroids. Experimental lesions were induced by patch tests with nickel sulfate 5 % in petrolatum in 10 sensitized subjects and treated with two medications of different steroids (clobetasol propionate, fluocinolone acetonide or clobetasone butyrate) performed 16 and 40 h after the application of the nickel patch tests. Clinical and echographic evaluations were carried out at the beginning of the experiment and 64 h after the induction of the reactions. After obtaining echographic images, these were processed by software, enabling the selection of amplitudes of interest, the highlighting of parts of images and their assessment by a value corresponding to the number of pixels (picture elements). For evaluations a low reflecting band was chosen, marking edema and inflammatory infiltration. At positive patch test sites we observed a progressive increase in the number of low reflecting pixels, in accordance with the intensity of the reaction. Therapeutic response was assessed as the difference between values of treated and untreated test sites. The rank order of the efficacy of test substances as determined echographically was identical to the rank order generally accepted for these steroids. This evaluation method of topical corticosteroid activity could be usefully employed besides traditional evaluation methods

    ALLERGIA/INTOLLERANZA AL GRANO: VALUTAZIONE DELLA TOLLERABILITA DI UN PRODOTTO DIETETICO

    Tra i diversi alimenti il grano e frequentemente responsabile di reazioni allergiche cutanee, sia di tipo orticarioide che eczematoso, oltre che di risposte respiratorie e gastrointestinali fino al quadro severo dell'anafilassi. Una volta posta la diagnosi di allergia alle proteine del grano sulla base dell'anamnesi, della clinica e dei test allergologici (prick test, atopy patch test, RAST, challenge orale) e necessario che il paziente segua una rigorosa dieta priva di grano. La pasta dietetica da noi esaminata* e un prodotto alimentare a base di farina di riso, farina di mais e farina di grano saraceno, sottoposto a rigorose analisi volte ad escludere la presenza di glutine e dei piu diffusi allergeni alimentari. Allo scopo di valutarne la tollerabilita' in pazienti con allergia al grano, nel periodo dicembre 2005-marzo 2006, sono stati arruolati 20 pazienti. Della popolazione in studio facevano parte anche pazienti con dermatite erpetiforme che necessitavano di dieta rigorosa priva di glutine.Si trattava di 12 maschi e 8 femmine di eta compresa tra 2 e 45 anni. I pazienti erano affetti da dermatite atopica (11 casi), dermatite erpetiforme (5 casi), orticaria cronica (3 casi), rinite/asma (1 caso). Nel caso dei pazienti con dermatite erpetiforme la diagnosi era confermata dall'esame in immunofluorescenza diretta (IFD). Tutti gli altri pazienti presentavano test allergologici positivi per grano e/o glutine e/o gliadina e/o challenge in aperto singolo o ripetuto positivo per grano. All?anamnesi 10 pazienti riferivano peggioramento dei sintomi per consumo di cereali. Dei pazienti in esame 9 erano gia a dieta priva di grano. Tutti i pazienti arruolati, previo consenso informato, sono stati invitati a consumare, al proprio domicilio per 1 mese consecutivamente, la pasta in esame come sostituto della pasta di grano. Ai pazienti e stato fornito gratuitamente il prodotto in quantitativo sufficiente in base ai consumi dichiarati. Ai pazienti e stato richiesto di evitare assolutamente tutti gli alimenti con farina di grano, mentre e stato consentito il consumo di riso e mais.Dopo test d'uso nessuno dei pazienti in studio ha riferito eventi avversi correlabili al consumo della pasta dietetica in esame o peggioramento del quadro clinico. Tale prodotto e risultato essere ben tollerato da pazienti con sintomi cutanei e respiratori conseguenti al consumo di grano. * WELLFOOD past

    Neuropeptides And Skin Inflammation

    Neuropeptides (NP) are protein compounds contained both in the central and peripheral nervous system. They can be antidromically released from sensory nerves and are implicated in the so-called neurogenic inflammation. They also exert a number of functions within the immune system and are thought to act as trophic as well as mitogenic substances. Several NP have been detected in human skin by immunohistochemical and radioimmunological techniques, and recent reports have demonstrated that NP could be involved in the mechanisms of certain dermatoses. The involvement of NP in either physiological or pathophysiological skin conditions is discussed. Moreover, a few questions, which still need to be addressed, are raised, and future directions this field of research should take are outlined
