    Three-dimensional ultrasound fetal lung volumes and infant respiratory outcome: A prospective observational study

    Objective: To determine if fetal lung volumes (FLVs), determined by three-dimensional rotational ultrasound and virtual organ computer-aided analysis software (vocal), correlated with neonatal respiratory outcomes in surviving infants who had a high risk [fetuses with congenital diaphragmatic hernia (CDH)], lower risk [fetuses with anterior wall defects (AWDs)] and no risk (controls) of abnormal antenatal lung growth. Design: Prospective observational study. Setting: Tertiary fetal medicine and neonatal intensive care units. Population: Sixty fetuses (25 with CDH, 25 with AWDs and ten controls). Methods: FLVs were measured and expressed as the percentage of the observed compared with the expected for gestational age. Main outcome measures: Neonatal respiratory outcome was determined by the duration of supplemental oxygen, mechanical ventilation and dependencies, and assessment of lung volume using a gas dilution technique to measure functional residual capacity (FRC). Results: The infants with CDH had lower FLV results than both the infants with AWDs (P = 0.05) and the controls (P < 0.05). The infants with CDH had longer durations of mechanical ventilation (P < 0.001) and supplementary oxygen (P < 0.001) dependence, compared with infants with AWDs. The infants with CDH had a lower median FRC than both the infants with AWDs (P < 0.001) and the controls (P < 0.001). FLV results correlated significantly with the durations of dependency on ventilation (r = )0.744, P < 0.01) and oxygen (r = )0.788, P < 0.001), and with FRC results (r = 0.429, P = 0.001). Conclusions: These results suggest that FLVs obtained using three-dimensional rotational ultrasound might be useful in predicting neonatal respiratory outcome in surviving infants who had varying risks of abnormal lung growth.     An evaluation of the delivery of the Eskom Customer Safety Education Programme in the Queenstown Customer Services Area.

    The aim of this research is to evaluate the Eskom customer safety education programme in the Queenstown Customer Services Area. The research focuses on the implementation of the programme with the aim of assessing whether there is need for optimisation in the way that the customer education and training is delivered. The paper consists of three sections. Section one is directed to Eskom and is made up of (i) a review of the literature that is relevant to the Eskom safety education programme, (ii) the findings made from the study and (iii) recommendations from the author. Section two is a review of the key concepts which were identified for this paper and these are: (i) education and training, (ii) consumer education, (iii) citizenship education, (iv) training programme development, and (v) training programme evaluation. Section three provides detail on and justifies the research methodology that was followed for this paper. The author adopted the deductive approach and collected data from literature sources and Eskom documents as well as face-to-face interviews with the Eskom trainers and customers. Qualitative data was collected from the interviews and is presented in section one of the report in the form of (i) narratives, (ii) graphs and (iii) tables. The research has identified some shortcomings in the delivery of the training and recommendations have been made on how these shortcomings can be addressed. Trainer skills, training evaluation and training tools are amongst the issues that require attention in the Queenstown Customer Services Area. The results revealed that 67 percent of the trainers have not received training related to conducting customer forums. Inconsistencies with regard to the evaluation of the training interventions were also identified and there were differences in terms of the training tools that are utilised at the forums. The recommendations made by the author are based on the literature that was reviewed as part of this study and are aimed at addressing the delivery of the training. The respondents all displayed an understanding of the objectives of the Eskom safety education programme and made recommendations in terms of how Eskom can reduce the tampering of Eskom equipment. The recommendations made included the involvement of the community as well as unannounced door-to-door audits aimed at identifying damaged and tampered Eskom equipment. These recommendations are in line with the guiding principles of safety education presented in McWhirter (2008: 32) which include the involvement of young people in real decisions to help them stay safe, as well as the use of active approaches in addressing safety issues. A review of the electrical contact incidents which were reported to Eskom in 2011/12 revealed that in 61,5 percent of the cases that occurred in the Eastern Cape, Eskom had conducted electricity safety education before the incidents occurred. The recommendations in terms of the training delivery should be implemented to improve the delivery of the training. The author however recommends further research on how Eskom can reduce the number of electrical contact incidents that occur, as it has been proven through previous studies that safety education “has an impact on knowledge, behaviour, risk and skills and no papers have provided evidence of the impact of safety education on injury rates” (Mulvaney, Watson and Errington, 2012: 27)

    ...Introduction: Elevated symptom burden has been reported in concomitant asthma and overweight, although objective evidence of greater disease severity is lacking. This may reflect exaggerated symptom perception and has implications for optimal asthma management. Aims: To investigate respiratory load perception in overweight asthmatic (OA) children compared to normal weight asthmatic children (NWA) and healthy controls (HC). Methods: Incremental inspiratory resistive loading was performed in 27 children (9 OA, 9 NWA, 9 HC) with load perception assessed by visual analogue scale (VAS). Respiratory load was quantified via parasternal intercostal electromyography (EMGpara) expressed as neuroventilatory efficiency (NVE: ratio of tidal volume to EMGpara (ml/μV)). Linear mixed model analysis was used to examine changes in perception scores and NVE with increasing resistance, and to determine the influence of asthma and overweight. Results: VAS increased significantly with increasing resistance (slope (95% CI) 5.06 (1.83 – 8.29)mm/kPa/L/s, p=0.002), which was additionally influenced by degree of reduction in NVE (p=0.004). The slopes of these relationships did not differ with group, indicating no effect of asthma or overweight on either perception or objective response to increasing load. Weight had a significant effect on the intercept of the VAS/resistance relationship, with OA having a significantly higher intercept (9.96mm (95% CI 1.15 – 18.77), p=0.027), suggesting breathlessness at rest and higher VAS scores throughout. Conclusions: Respiratory load perception is not influenced by weight status. The increased symptom burden in overweight asthmatics may be related to elevated baseline respiratory load.Meeting abstract presented at European Respiratory Society International Congress, London, UK, 3-7 September 2016

    Contracts Discharged through Breach: Restitution for Services Rendered by the Innocent Party

    The author examines the autonomous claim in unjust enrichment available to a party who has elected to treat a contract as discharged because of the defendant\u27s fundamental breach. In this context, he concentrates upon the claim in respect of services rendered and addresses the primary questions that arise in the law of restitution: What factor renders the defendant\u27s enrichment unjust? Has the defendant in fact been enriched? How should the defendant\u27s enrichment be valued? The author suggests answers to these basic questions, but he also tries to show that there is little need to give a plaintiff, faced with a repudiatory breach, an alternative claim in restitution. The plaintiff should be restricted to a claim for damages for breach of contract. The plaintiff has chosen to make the contract on the terms agreed and there is nothing unfair in limiting the remedies to that bargain
