    Alveolar deficiency is among the most common skeletal deformities after mandibular trauma and teeth loss. A combined recovery of missing bone and soft tissue is mandatory. Distraction osteogenesis increases simultaneously the volume of bone and soft tissues. After distraction the area is then suitable for the prosthetic rehabilitatio. The Author reports a case of alveolar reconstruction by axial vertical distractio osteogenesis. The surgical technique employed will be presented and the achieved preliminary results are discussed

    How to avoid surgical complications and failures in implantology

    Implants have evolved to a rapidly growing techniques for replace the missing teeth. The total number of implants being placed has increased significantly over the years, as the prevalence of complications. In oral implantology, the most serious complications and those most frequently described in the literature occur during surgery. They may result from inadequate planning, overworking of the implant bed, contamination of the implant by incorrect manipulation or mishandling; by poor implant orientation, or by the surgical procedure itself, which is not without risk. There are several reasons for the increased number of implant complications in the last years, mainly related to the increased number of implants being placed, the increased number of dentists, and the lack of adequate training. Intraoperative complications may be related with surgery such as hemorrage, neurosensory alterations, damage to adjacent teeth and mandibular fractures. Otherwise complications may be associated with implant placement such as absence of primary stability, fenestration or displacement into maxillary sinus. A spectrum of surgical implant complication is reported, and the author provide a detailed analysis of the etiology, prevention and treatment. The possibility of potential complications has to be considered before placing or restoring implants, and knowledge, learning, and experience are paramount to reducing the number and the severity

    Distraction osteogenesis has been one of the most innovative concepts in craniomaxillofacial surgery through the last 25 years. In 1987 Cesar A. Guerrero first performed a mandibular widening by distraction osteogenesis. Joseph G. McCarthy in 1992 published an extensive paper on the treatment of hemifacial microsomia. In 1994 Wangerin and Gropp and in 1996 Diner et al. published on the use of intraoral devices for mandibular distraction. After a quarter of century of extensive use Distraction Osteogenesis has today specific indications for congenital craniofacial and cleft deformities. Technology has evolved from the first application of external devices to intraoral and hybrid or semiburied techniques. In congenital craniomaxillofacial anomalies distraction is indicated during growth. Mandibular distraction osteogenesis can be safely and effectively used to avoid or remove tracheostomy in neonates with severe airway obstruction caused by micrognatia in Pierre Robin sequence. There is a great range of indications of DO in pediatric craniofacial deformities and this new concept seems to well combine the proven Tessier principle of “ first build, then move ”. On the other hand thorough Team evaluation needs to be esthablished prior to the surgical decision, and the indications of early distraction in neonates should be well-considered

    The effects of drag-reducing polymers on hemodynamics and whole blood-endothelial interactions in 3D-printed vascular topologies

Abstract: Most in vitro models use culture medium to apply fluid shear stress to endothelial cells, which does not capture the interaction between blood and endothelial cells. Here, we describe a new system to characterize whole blood flow through a 3D-printed, endothelialized vascular topology that induces flow separation at a bifurcation. Drag-reducing polymers, which have been previously studied as a potential therapy to reduce the pressure drop across the vascular bed, are evaluated for their effect on mitigating disturbed flow. Concentrations of 1000 ppm prevent recirculation and disturbed flow at the wall. Proteomic analysis of plasma collected from whole blood recirculated through the vascularized channel with and without drag-reducing polymers provides insight into the effects of flow regimes on levels of proteins indicative of endothelial-blood interaction. The results indicate that blood flow alters proteins associated with coagulation, inflammation, and other processes. Overall, these proof-of-concept experiments demonstrate the importance of using whole blood flow to study the endothelial response to perfusion

    Die Karlsdichtung von Morant und Galie gilt gemeinhin als «Prozessepos». Biographische Details der Vita Caroli Magni, einer Lebensgeschichte Karls des Großen in lateinischer Sprache aus der Feder seines Zeitgenossen Einhard, führen in der Folge dazu, dass Karl sowohl in der historiographischen als auch in der epischen Literatur bis ins späte Mittelalter immer wieder im Zusammenhang mit Rechtshandlungen dargestellt wird. In diesen Darstellungen erscheint Karl überwiegend als Herrscher, der die Rechtsordnung seiner Zeit kennt und anerkennt. Von dieser Tradition hebt sich die Figur Karls in Morant und Galie in einigen signifikanten Zügen ab: Karl, der im Prozess gegen Morant und Galie oberster Gerichtsherr, aber auch Mitankläger ist, befindet sich im Zwiespalt zwischen seiner institutionellen Rolle als Herrscher und Garant des Rechts einerseits und seiner privaten Rolle als vermeintlich betrogener Ehemann andererseits. In dieser Konfliktsituation riskiert er wiederholt durch unüberlegte Reaktionen im Affekt, dem Unrecht Vorschub zu leisten und die Wahrheitsfindung zu behindern. Vorliegende Untersuchung geht der Frage nach, welche Funktion dieser von der Tradition abweichenden Gestaltung der Figur Karls im Erzählkontext zukommt. Darüber hinaus werden Überlegungen zum Stellenwert des Karlsepos von Morant und Galie als mögliche Rechtserkenntnisquelle für mittelalterliche Rechtsgepflogenheiten angestellt

    The work culture of civil servants in this decentralization era tends to come fromthe working culture of each ethnic. The difference of culture applied causes thedifference of civil servants’ way, behavior, and action when performing their work. Thisresearch article aims to discuss one of the working cultures of the civil servants fromMinangkabau ethnic group named galie. This research was conducted by usingdescriptive-qualitative approach, and the data was collected trough observation,interview, and documentation study. Data is validated by triangulation and sourcetriangulation methods.Tthe data analysis is based on Miles and Huberman (2000). Theresult of the research shows that the galie work culture has usually been inherited in thefamily and society for a period time; hence the galie work culture is also implementedwhile working in the local government organization of Pasaman Barat regency. Theresearch concluded that the galie work culture does not cause work culture that canharm others, but whenever the work culture exists, the civil servants would be annoyed.Civil servants who have this work culture tend to avoid risks and prefer simpler workthan their colleagues.Key words: work ethnict culture, galie, civil servant, Minangkabau ethict</jats:p
