Spectrum of Ectopic Pelvic Kidney in Children: A Ten-year Experience.
No full textIntroduction: Many patients with ectopic kidneys remain often undiagnosed
or asymptomatic throughout life. Ectopic pelvic kidneys present a large spectrum
of presentation and symptoms from renal dysplasia to a severe obstruction. One
of the most common problem is the UPJ obstruction with stones formation. We
present here our experience of pelvic kidney in children, the clinical presentation
and the surgical procedures performed
Material and Methods: A total of 17 children, aged from 6 months to 17
years, (14m;3f) were referred to our Institution between January 2004 to June
2014 for pelvic kidney. There were 5 (29.41%) left and 12 (70.5%) right Kidneys.
1. 9 patients presented with moderate to severe UPJ obstruction (three
cases with 10 to 20mm pelvic stones) with normal/moderately-reduced
≥25% relative function at MAG3 radionuclide scan of these patients,
5 with clear symptoms of UPJ obstruction underwent a laparoscopic
dismembered Anderson –Hynes pyeloplasty. The remaining 4,
presented symptomatic intermittent hydronephrosis with colic abdominal
pain. These patients underwent pelvis derotation through a minimally
invasive transposition of the UPJ.
2. 5 patients presented non-functioning kidneys, in 2 cases associated to
hypertension, and underwent laparoscopic nephrectomy.
3. 2 patients presented asymptomatic pelvic kidney.
4. 1 patient presented 3rd G. VUR on the ipsilateral pelvic kidney.
The associate pathology was 1 midshaft hypospadias, 2 criptorchidism , 1
mild mitral insufficiency.
The evaluation of each patient involved their personal and family medical
history, ultrasound examination, VCUG, MAG3 renal scan and MRI in selected
cases.
Results: After a mean follow-up of 6.9 yrs the majority of the patients are
well and none present hypertension. Symptoms resolved in 13 out of 15 surgical
patients. 2 patients needed the positioning of a double J stent from 3 to 8 months
after pyeloplasty that was removed from 6 to 12 weeks after the procedure
Conclusion: Pelvic kidneys present a large spectrum of symptoms.
UPJ obstruction with/without stones, intermittent hydronephrosis, recurrent
abdominal pain, UTI is the most frequent symptoms. The majority of our patients
needed surgical procedures. In four cases there were associated pathologies
as hypospadias, cryptorchidism, and mild mitral insufficiency. Laparoscopic
approach seems a useful tool for the treatment of these kidneys. Pelvis
derotation can be an easy and effective procedure in moderate, intermittent
obstruction

presents an acceptable low percentage of recurrences that is comparable to those of the open
technique, with the advantage of reduced post operative pain, rapid return to everyday
activities, and optimal cosmetic results. Although the results of our experience seems promising
we think that the follow-up is too short and the number of the patients too small for
drawing definitive conclusion

No full textLa mancata conoscenza ed applicazione della ergonomia in laparoscopia e segnatamente in laparoscopia pediatrica , dove il campo operatorio e lo spazio di lavoro sono più ridotti che nell’adulto, può porre seri problemi di salute per il chirurgo e può condizionare l’ esecuzione ed i tempi dell’intervento.Nella chirurgia laparoscopica la situazione è molto diversa. L'occhio e l' azione si trasferiscono all' interno dell' addome (Endoblocco operatorio) ed il chirurgo deve rinunciare a parte della sua potenzialità tattile e palpatoria che sarà mediata dagli strumenti operativi.
In questa condizione il chirurgo vede indirettamente il campo operatorio e può accedere alle strutture da manipolare in cavità (toracica, addominale , retroperitoneale etc.) attraverso lunghi strumenti che passano attraverso trocar che sono in posizione fissa. I nostri sensi, in questo caso, lavorano molto di più per ottenere lo stesso risultato. Il disegno appropriato degli strumenti e la disposizione della sala operatoria divengono allora critici per evitare stress ed errori. Possiamo esaminare l’ ergonomia in laparoscopia come l’insieme di problematiche che sono connesse

• alla strumentazione (progettazione e modalità di utilizzazione)

Experiences using the UML profile for MARTE to stochastically model postproduction interactive applications
No full textAbstract: We describe a practical approach applying the UML 2.0 standard MARTE profile to model stochastic interactive application workflows, using the PapyrusUML editor. We use the PaStep, PaCommStep, PaLogicalResource and GaCommHost MARTE stereotypes and find them sufficient for stochastic modelling with the exception of being unable to define non-standard probability distributions. We have investigated both Markovian stochastic models and discrete event simulation models, serializing UML deployment and state machine diagrams to automate model creation. The choice between using a stochastic model (e.g. PRISM Markov models) or discrete event simulation model (e.g. Monte Carlo simulations) depends on the complexity of the model, accuracy required and compute time needed. We find that PRISM models are fast to execute if the complexity is small and produce numerically accurate results. Discrete event simulation models are slower to execute but scale much better and are probably the default solution to a model of unknown complexity
Appendiceal MALT Lymphoma in Childhood – Presentation and Evolution
No full textLymphoma of mucosa-associated lymphoid tissue (MALT lymphoma) was first described
by Isaacson et al. in 1983 (Isaacson & Wright, 1984). According to the WHO lymphoma
classification, the indolent B cell lymphoma of MALT type is classified as a marginal zone
lymphoma, thus called because it originates from the B lymphocytes normally present in a
distinct anatomical location (marginal zone) of the secondary lymphoid follicles (Harris et
al., 2001). MALT lymphomas comprise up to 40% of adult non-Hodgkin lymphomas (NHL);
the median age at occurrence is 60 years, with a female predominance (Anonymous, 1997).
In paediatric age MALT lymphomas are very rare. We report on a case of MALT lymphoma
involving the appendix in a 6-year-old immunocompetent girl and its evolution toward an
inflammatory bowel disease (IBD) at a middle-term follow-up

analisi matematiche delle forme evidenziate dalle fotografie ottenute dai Rover NASA. Il tutto sembra indicare la possibile presenza di microorganismi in epoche antiche. Tuttavia, l’interpretazione di tali evidenze richiede cautela e ulteriore approfondimento
