Pharmacoeconomic analysis of adjuvant oral capecitabine vs intravenous 5-FU/LV in Dukes' C colon cancer: the X-ACT trial
No full textOral capecitabine (Xeloda<sup>®</sup>) is an effective drug with favourable safety in adjuvant and metastatic colorectal cancer. Oxaliplatin-based therapy is becoming standard for Dukes' C colon cancer in patients suitable for combination therapy, but is not yet approved by the UK National Institute for Health and Clinical Excellence (NICE) in the adjuvant setting. Adjuvant capecitabine is at least as effective as 5-fluorouracil/leucovorin (5-FU/LV), with significant superiority in relapse-free survival and a trend towards improved disease-free and overall survival. We assessed the cost-effectiveness of adjuvant capecitabine from payer (UK National Health Service (NHS)) and societal perspectives. We used clinical trial data and published sources to estimate incremental direct and societal costs and gains in quality-adjusted life months (QALMs). Acquisition costs were higher for capecitabine than 5-FU/LV, but higher 5-FU/LV administration costs resulted in 57% lower chemotherapy costs for capecitabine. Capecitabine vs 5-FU/LV-associated adverse events required fewer medications and hospitalisations (cost savings £3653). Societal costs, including patient travel/time costs, were reduced by >75% with capecitabine vs 5-FU/LV (cost savings £1318), with lifetime gain in QALMs of 9 months. Medical resource utilisation is significantly decreased with capecitabine vs 5-FU/LV, with cost savings to the NHS and society. Capecitabine is also projected to increase life expectancy vs 5-FU/LV. Cost savings and better outcomes make capecitabine a preferred adjuvant therapy for Dukes' C colon cancer. This pharmacoeconomic analysis strongly supports replacing 5-FU/LV with capecitabine in the adjuvant treatment of colon cancer in the UK
EpCAM Aptamer Activated 5-FU-Loaded PLGA Nanoparticles in CRC Treatment; In vitro and In vivo study
No full textIn this study, EpCAM aptamer-activated nanoparticles (Ap-NPs) were synthesized to enhance treatment efficiency in colorectal cancer (CRC). PLGA [poly (D, L-lactide-co-glycolide)] copolymer was fabricated by conjugation of COOH-PEG-NH2 to PLGA-COOH through an EDC/NHS-mediated chemistry. Afterward, 5‐fluorouracil-loaded (FU) nanoparticles were prepared using the water/oil/water double emulsion solvent evaporation method. The in vitro cytotoxicity of formulations was evaluated using the MTT assay in HCT-116, CT-26, and HEK-293 cell lines. For in vivo study, tumor-bearing BALB/c mice were established by subcutaneous injection of CT-26 cell line. The results indicated that fabricated AP-FU-NPs had 101 nm size with a spherical surface, relatively homogeneously and, satisfactory encapsulation efficiency (83.93%). In vitro experiments revealed that Ap-FU-NPs had a superior in vitro cytotoxicity than both FU-NPs and free 5-FU in CT-26 and HCT-116 cells but, were significantly low toxic against HEK-293 cells relative to free 5-FU. Furthermore, in vivo results showed no significant hemolytic effect, hepatic and renal injury, or weight loss. After treatment of various animal groups with formulations, notable tumor growth delay was observed followed the order: Ap-FU-NPs The overall strategy of this study is summarized in Graphical Abstract. The present study was carried out in the three steps summarized: 1) synthesis and characterization. At first, PEG was conjugated to PLGA polymers by EDC/NHS chemistry method, then the resulting polymers were used to prepare PLGA-PEG NPs via the W/O/W technique. Finally, EpCAM Aptamers were attached to fabricated NPs. 2) In the second step cytotoxicity of formulated NPs was evaluated in vitro experiments in HEK-293, CT-26, and HCT-116 cell lines. 3) In the third step in vitro results further was investigated in CT-26 tumor-bearing BALB/c mice. “Created with BioRender.com.”</p
Electric field determination of polymeric dc power cable in the presence of space charge
No full textSpace charge in polymeric power cables has been previously measured by the pulsed electroacoustic (PEA) technique and its influence on electric field distribution has been estimated assuming that the conductivity is independent of electric field. In this paper we proposed a method of determining electric field distribution considering the influence of the effect of electric field on the conductivity. The cable insulation is divided into thin layers with different conductivities and space charge present in the each layer is viewed as the consequence caused by difference in conductivity. Based on the hopping conduction mechanisms, it is possible to compute electric field distribution across the cable insulation. Compared with the electric field distribution without considering change in conductivity, the new proposed method shows reduction at the maximum electric field. However, the position of the maximum electric field shifts depending on space charge distribution

No full textThe author shares his reflections on state of art in housing and urban planning, deficiencies, expectations and possibilities in the Sahel region of Africa. He notices, that the housing problem in Africa is one of the challenges, which should be solved in order to recover life dignity of African people and secure their rights to traditional family life in acceptable conditions. The paper describes the studies on the typical dispersed urban structures and the need to foster this type of settlement structure and proposals of introduction of new on light steel frame housing system in the area of Sahel, combining the traditional way of building houses with modern technology. The particularly analysed case, is the housing problem in the Republic of Chad. The author presents the basic discussion on this topic and his architectural proposals. Unfortunately, the gap between the needs and the financial feasibility of housing construction in this area, makes this project already at the starting point extremely difficult to be realized without external subventions.Problem mieszkaniowy w Afryce jest jednym z wyzwań, które należy rozwiązać, aby Afrykanie mogli odzyskać godność życiową i zabezpieczyć swoje prawa do tradycyjnego życia rodzinnego w akceptowalnych warunkach. W artykule opisano badania nad typowymi rozproszonymi strukturami miejskimi i potrzebą wspierania tego typu struktur osadniczych oraz propozycji wprowadzenia nowego systemu konstrukcji domów, opartym na lekkim szkielecie stalowym, łączącym tradycyjny sposób budowania domów z nowoczesną technologią. Przypadkiem szczególnie analizowanym jest problem mieszkaniowy w Republice Czadu. Autor przedstawia podstawową dyskusję na ten temat i swoje propozycje architektoniczne. Niestety luka między potrzebami mieszkaniowymi w Czadzie a finansową wykonalnością budownictwa mieszkaniowego w tym obszarze sprawia, że projekt ten, już w punkcie wyjścia, jest niezwykle trudny do realizacji bez uzyskania zewnętrznych dotacji

No full textW artykule przedstawiono najważniejsze zagadnienia związane z postrzeganiem i waloryzacją przestrzeni miejskiej przez różne grupy użytkowników. W pierwszej części ukazano teoretyczne ujęcie kwestii percepcji przestrzeni miasta w postaci koncepcji Kevina Lyncha oraz Yi Fu Tuana. Następnie wyróżniono główne grupy uczestników procesu percepcji oraz opisano ich rolę w przestrzeni. W ostatniej części zaproponowano i opisano teoretyczne implikacje łańcucha przyczynowo-skutkowego procesu waloryzacji przestrzeni w postaci oceny i przemian przestrzeni. Głównym celem artykułu jest wskazanie teoretycznych podstaw procesów postrzegania przestrzeni miast, w tym prezentacja modelu „nowej percepcji” w sytuacji zmian w przestrzeni miejskiej

Żadna z nich nie ma odniesienia do oryginalnego kontekstu, w którym pojęcie „w poezji jest malarstwo” ma znaczenie.W swoich wierszach Du Fu testował granice przedstawiania w poezji i poszerzał horyzont znaczeniowy poezji w pogoni za tym, co określał jako najwyższe znaczenia piękna wykraczającego poza słowa i obrazy, tworząc krytyczną syntezę swojej poetyki.The various elements of poetry-painting analogy applied to Du Fu’s poetry reflect critical differentiation rather than a community of thought. None of them have reference to the original context in which the notion of “in poetry is painting” has meaning.In his poetry, Du Fu tested boundaries of poetry and widened the horizon of poetry in pursuit of what he termed as the highest meanings of beauty beyond words and images, creating a critical synthesis of poetry
