Advances in pharmacotherapy for wet age-related macular degeneration
Introduction: In developed countries, neovascular age-related macular degeneration (AMD) is the leading cause of irreversible central blindness. Although AMD pathogenesis is complex and still not fully understood, many involved mechanisms are already partially known and could be promising targets for future therapies. Currently, anti-VEGF drugs are the standard care of this condition.Areas covered: This review summarizes both the current available and the emerging pharmacological therapies for the management of neovascular AMD. At first, we briefly focused on anti-VEGF compounds that are commonly used. Then, we reviewed the mechanisms of action and potential advantages of new candidate drugs that are being evaluated in clinical trials.Expert opinion: Although anti-VEGF drugs have shown mild-term good efficacy and safety profile in the treatment of neovascular AMD, they are far away from being a perfect therapy. Pharmacological research should focus on finding new molecular targets in the AMD pathogenetical pathway and on developing longer lasting agents or new drug delivery systems. Besides the development of new drugs, a better characterization of patients is also needed, taking into account variables such as choroidal neovascularization subtypes and genetic factors, in order to identify a tailored treatment for each patient
Ocular Findings in {COVID}-19 Patients: A Review of Direct Manifestations and Indirect Effects on the Eye
The novel pandemic coronavirus disease 2019 (COVID-19), caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has challenged the medical community. While diagnostic and therapeutic efforts have been focused on respiratory complications of the disease, several ocular implications have also emerged. SARS-CoV-2 RNA has been found in tears of the infected patients, and reports suggest that the ocular surface could serve as a portal of entry and a reservoir for viral transmission. Clinically, COVID-19 has been associated with mild conjunctivitis, which can be the first and only symptom of the disease. Subtle retinal changes like hyperreflective lesions in the inner layers on optical coherence tomography (OCT), cotton-wool spots, and microhemorrhages have also been reported. In addition, COVID-19 has been associated with an increased incidence of systemic diseases like diabetes mellitus and Kawasaki disease, which are particularly relevant for ophthalmologists due to their potentially severe ocular manifestations. Several treatment strategies are currently under investigation for COVID-19, but none of them have been proved to be safe and effective to date. Intensive care unit patients, due to risk factors like invasive mechanical ventilation, prone position, and multiresistant bacterial exposure, may develop ocular complications like ocular surface disorders, secondary infections, and less frequently acute ischemic optic neuropathy and intraocular pressure elevation. Among the array of drugs that have shown positive results, the use of hydroxychloroquine and chloroquine has raised a concern due to their well-known retinal toxic effects. However, the risk of retinal toxicity with short-term high-dose use of antimalarials is still unknown. Ocular side effects have also been reported with other investigational drugs like lopinavir-ritonavir, interferons, and interleukin-1 and interleukin-6 inhibitors. The aim of this review was to summarize ophthalmological implications of SARS-CoV-2 infection to serve as a reference for eye care and other physicians for prompt diagnosis and management
Piilostani näen tiheään kasvaneet - Tutkielma samassa aiheessa pysymisen synnyttämistä oppimiskokemuksista maalaamisessa
Tutkielma käsittelee samassa aiheessa pysymistä maalaamisessa ja pohjautuu tekijän taiteelliseen työskentelyyn, jossa maalausaiheena ovat puunrungot. Tutkielmassa pohditaan samassa aiheessa pysymisen merkityksiä oppimiskokemusten kannalta.
Tutkimuksellisessa asennoitumisessa on piirteitä fenomenologiasta. Taiteellinen ajattelu vaikuttaa tutkielman taustalla, ja aineiston purkamiseen käytetään teemoittelevaa otetta. Tekijää inspiroivat Jaana Houessoun, Leena Valkeapään, Mira Kallio-Tavinin ja Otso Kantokorven kirjoitukset.
Tekijä on maalannut puunrunkoaiheisia maalauksia kahdeksan vuoden ajan, mistä on muodostunut teossarja Piilostani näen tiheään kasvaneet.
This thesis explores a phenomenon of staying in the same subject area throughout the author’s own artistic work focusing on tree trunks. The thesis contemplates the significance of staying in the same subject as a learning experience. The aim is to verbalize and conceptualize painter’s experience of staying in the same subject and develop the communicability of artistic work as a part of art teacher’s work. The thesis aims to answer the following questions: What do I learn by painting the same subject? How can this approach be considered pedagogical? First, the thesis investigates the topic from the author’s own point of view and then reflects learning opportunities in the teaching of painting in general.
The artistic and arts based research and the literature of artistic work form the theoretical base of this research. The author wishes to participate in the academic discussion regarding artistic work, learning and experiencing through arts. This research approach is influenced by phenomenology. The study is affected by artistic thinking, and the research material is grouped into themes. This thesis is inspired by the writings of Jaana Houessou, Leena Valkeapää, Mira Kallio-Tavin and Otso Kantokorpi.
The author has painted tree trunks for eight years, which has formed a painting series called From my hideout I see the densely grown (in Finnish Piilostani näen tiheään kasvaneet). The material consists of paintings and a painting diary which have been completed during the research period between January and October 2015. At the end of this period, the author perceived a chronological process that revealed a large variety of phenomena repeating during the process, such as difficulties, success, the importance of repetition, the comfort zone, the sensibility of painting, the routines, the sensitization, the significance of a teacher, the overcoming challenges, the increase in courage and tolerance to uncertainty, and the respect for a process. The abovementioned phenomena form five larger themes strongly related to learning: process, own space, interaction, courage, and acceptance.
In the conclusion section, the author reflects on the studied themes and research questions. The five themes represent learning and help to clarify the studied phenomenon in a more general level. The understanding of researched phenomena were broadened by in-depth observations of the author´s own learning, painting and literature. The thesis concludes that staying in the same subject area teaches by its rewarding aspect and by opening oneself to emerging phenomena. The mental aspects of painting, such as acceptance, interaction and courage, are considered to form the key learning experiences. At the end of this thesis, the author ponders the changes in her own thinking and the possibilities of applying the thesis outcomes in the field of teaching of painting. The author found terms and words describing the phenomena emerging from the painting process. Those terms and words can also assist in the teaching of painting. The author assesses the importance of teacher involvement in painting and practical applications of these results. The thesis concludes that faults should be allowed and failures should be considered as natural parts of artistic work. The author recommends that the staying in the same subject area could be applied in multidisciplinary contexts through the utilization of group work
