Spinal cord compression secondary to intraspinal extramedullary hematopoiesis
Extramedullary hematopoiesis (EMH) is a rare cause of spinal cord compression (SCC) occurring in various hematological illnesses such as myeloproliferative disorders, thalassemias, and various types of anemia. EMH represents the growth of blood cells outside the bone marrow. Common EMH locations include the spleen, liver, lymph nodes, and paravertebral regions. When this occurs in the spinal cord, the mass effect can compress the spinal cord and cause different neurological symptoms depending on the area of the spinal cord affected. This report describes a 27-year-old female with a known case of beta-thalassemia major, who presented with mid-thoracic back pain, weakness, and paresthesia at the T10 level. Cation-? interactions of selected alkali metal ions with two benzene rings connected through linear chains, 2018
Quantum chemical calculations at M06-2X/6-311+G(2d,2p) level were employed to examine the structures, dissociation energies and vibrational spectra of complexes formed by Li+, Na+ and K+ with diphenylmethane, 1,2-diphenylethane, 1,3-diphenylpropane, [2.2]paracyclophane and [3.3]paracyclophane molecules. The effect of heteroatom substitution in the connecting chains of 1,3-diphenylpropane and [3.3]paracyclophane molecules on the binding affinity of cations with ligands was also included in this study. The complexes of [3.3] paracyclophane based molecules seem to have weak cation-pi interactions with metal ions Na+ and K+ when they were placed between two rings. The binding affinity of the metal ion varies from the ligands possessing single fusion to double fusion of alkyl chain (or hetero substituted alkyl chain) with two benzene rings. Vibrational frequencies related to metal-ligand stretching, C-H, N-H and P-H stretching are useful to characterize the complexes with different metal ions and their positional preference. Environmental impacts cost assessment model of residential building using an artificial neural network
Purpose: Buildings are major contributors to greenhouse gases (GHG) along the various stages of the building life cycle. A range of tools have been utilised for estimating building energy use and environmental impacts; these are time-consuming and require massive data that are not necessarily available during early design stages. Therefore, this study aimed to develop an Environmental Impacts Cost Assessment Model (EICAM) that quantifies both energy and environmental costs for residential buildings. Design/methodology/approach: An Artificial Neural Network (ANN) was employed to develop the EICAM. The model consists of six input parameters, including wall type, roof type, glazing type, window to wall ratio (WWR), shading device and building orientation. In addition, the model calculates four measures: annual energy cost, operational carbon over 20 years, envelope embodied carbon and total carbon per square metre. The ANN architecture is 6:13:4:4, where the conjugate gradient algorithm was applied to train the model and minimise the mean squared error (MSE). Furthermore, regression analysis for the ANN prediction for each output was performed. Findings: The MSE was minimised to 0.016 while training the model. Also, the correlation between each ANN output and the actual output was very strong, with an R2 value for each output of almost 0.998. Moreover, validation was conducted for each output, with the error percentages calculated at 0.26%, 0.25%, 0.03% and 0.27% for the annual energy cost, operational carbon, envelope materials embodied carbon and total carbon per square metre, respectively. Accordingly, the EICAM contributes to enhancing design decision-making concerning energy consumption and carbon emissions in the early design stages. Research limitations/implications: This study provides theoretical implications to the domain of building environmental impact assessment through illustrating a systematic approach for developing an energy-based prediction model that generates four environmental-oriented outputs, namely energy cost, operational energy carbon, envelope embodied carbon, and total carbon. The model developed has practical implications for the architectural/engineering (A/E) industries by providing a useful tool to easily predict environmental impact costs during the early design phase. This would enable designers in Saudi Arabia to make effective design decisions that would increase sustainability in the building life cycle. Originality/value: By providing a holistic predictive model entitled EICAM, this study endeavours to bridge the gap between energy costs and environmental impacts in a predictive model for Saudi residential units. The novelty of this model is that it is an alternative tool that quantifies both energy cost, as well as building’s environmental impact, in one model by using a machine learning approach. Besides, EICAM predicts its outcomes more quickly than conventional tools such as DesignBuilder and is reliable for predicting accurate environmental impact costs during early design stages.</p
Aberrant antigen expression in acute leukemia: A single center study in Saudi Arabia
Abstract: Appropriate clinical care of hematologic malignancies depends on timely and accurate diagnosis. The category of cancers known as acute leukemias is diverse, with each disease having a unique clinical presentation, prognosis, and preferred course of therapy. The majority of morphologic and cytochemical characteristics were used in historical categorization systems, but currently, immunophenotypic, cytogenetic, and molecular data are included to construct clinically useful diagnostic classifications. When combined with morphologic evaluation, multiparameter flow cytometry quickly and precisely determines antigen expression profiles in acute leukemias, which frequently supports a specific diagnosis or a limited differential. Because numerous recurrent molecular or cytogenetic abnormalities are linked to distinctive immunophenotypic characteristics, flow cytometry is a crucial tool for guiding additional testing.
Even within a single acute leukemia subtype, the identification of certain antigens may have prognostic or therapeutic significance. The immunophenotypic fingerprint of a leukemia after first diagnosis serves as a helpful guide to track treatment response, minimal residual disease, and recurrence.
Objective: To evaluate flowcytometry role in distinguishing acute myeloid leukemia (AML) from ALL using CD markers.
Material and methods: This is retrospective study conducted in King Fahad Medical City in Riyadh city, in the period between November to December 2019 on patients with acute leukemia diagnosis.
Conclusion: Accurate acute leukemia diagnosis and categorization depend on immunophenotypic analysis. Multiparameter flow cytometry offers a quick and efficient way to gather these data, as well as prognostic data and a technique for assessing minimal residual illness.

Life Cycle Assessment of Low-Rise Office Building with Different Structure-Envelope Configurations
This paper presents life cycle assessment (LCA) approach to study the impact of structure and envelope types on the energy consumption and environmental impact of an office building in the New York City of USA. In addition, the future costs of environmental impact for various structure and envelope options are assessed according to the current practice and market price of CO2. Eight structure and envelope types for the low-rise office building are investigated, which include concrete and steel structures with various combinations of envelopes. The parameters such as life cycle energy use, global warming potential, and water, air and land emissions are analyzed. The energy simulation is performed by using eQUEST software while the environmental impact is assessed by using ATHENA® impact estimator. The building with concrete frame is proved to have lower environmental impact compared to that with steel frame. The precast concrete building is found to be the economical alternative due to the minimal environmental impact cost. Synthesis of Novel Triazinoindole-Based Thiourea Hybrid: A Study on α-Glucosidase Inhibitors and Their Molecular Docking
A new class of triazinoindole-bearing thiosemicarbazides (1–25) was synthesized and evaluated for α-glucosidase inhibitory potential. All synthesized analogs exhibited excellent inhibitory potential, with IC50 values ranging from 1.30 ± 0.01 to 35.80 ± 0.80 μM when compared to standard acarbose (an IC50 value of 38.60 ± 0.20 μM). Among the series, analogs 1 and 23 were found to be the most potent, with IC50 values of 1.30 ± 0.05 and 1.30 ± 0.01 μM, respectively. Prospective Matched Case-Control Study of Over-Early P100 Wave Latency in Migraine with Aura
A sizable portion of the world’s population suffers from migraines with aura. The purpose of this research is to describe the findings of a case-control study that was carried out to gain a better understanding of how migraine with aura manifests. The research looked at the P100 delay of the visual-evoked potential in both eyes of 92 healthy people and 44 patients who suffered from migraines with visual aura. All of the participants in the study were recruited from King Fahad University Hospital in Saudi Arabia. Both sets of people had the same ancestry and originated from the same location. Patients who suffered from migraines with aura exhibited a significantly shorter P100 delay in both eyes compared to healthy controls (p = 0.001), which is evidence that their early visual processing was distinct. In order to arrive at these findings, we compared people who suffer from migraines with aura to people who do not suffer from migraines and used them as subjects. These findings contribute to the ongoing attempts to bring the disease under control and provide vitally significant new information regarding the functioning of headaches with auras. The primary focus of study in the future should be on determining the nature of the connection between issues with early visual processing and headaches with aura
