Full text linkVitamin D features immunomodulatory effects on both the innate and adaptive immune systems, which may explain the growing evidence connecting vitamin D to allergic diseases. A wealth of studies describing a beneficial effect of vitamin D on atopic dermatitis (AD) prevalence and severity are known. However, observations linking high vitamin D levels to an increased risk of developing AD have also been published, effectively creating a controversy. In this paper, we review the existing literature on the association between AD and vitamin D levels, focusing on childhood. As of today, the role of vitamin D in AD is far from clear; additional studies are particularly needed in order to confirm the promising therapeutic role of vitamin D supplementation in childhood AD
Topical 1% propranolol in liposomal gel: A new adjuvant tool for chronic leprosy ulcers
Full text linkObjective: To evaluate the effects of 1% topical propranolol in liposomal gel in 3 patients with plantar ulcers. Methods: We enrolled 3 patients with 3 ulcers who had completed the WHO recommended treatment regimen. The ulcers were cleaned with sterile normal saline, and 1% topical propranolol in liposomal gel was applied 2 times/day for 3 months, or less if complete healing was reached before. Assessment of ulcer re-epithelization was recorded at baseline, 6 weeks, and 3 and 6 months after initiation of treatment. Results: Response in the form of granulation tissue formation started by the second week. Substantial reduction in size subsequently continued over the next 3 months. Two of the 3 patients showed complete healing of the ulcers at the 6 months follow up. In the 3rd patient, the ulcer showed only modest signs of healing. Surprisingly, in all patients, the sensory function was restored, particularly in terms of pain. Some motor functional recovery at the ulcer site and surrounding tissue was also documented. Conclusion: To the best of our knowledge, this is the first trial of topical propranolol for the treatment of trophic ulcers of leprosy. This may represent a promising adjuvant therapy for leprosy ulcers, including ulcers of older age. Further studies are warranted with a larger number of patients and a longer period of follow up to determine the ideal candidates and to identify clinical factors predictive of response
Airborne contact dermatitis
Full text linkAirborne dermatoses are complaints linked to external environmental, chemical, and biotic agents carried through the air. In general, airborne dermatoses that are most common in the work environment, tend to cause diagnostic problems that are challenging for both the patient and the doctor. It should also be borne in mind that since the external culprit agents are present in the environment, they do not only come in contact with the skin and mucosa, but can also be inhaled or ingested, thus also causing respiratory and systemic symptoms. Among the various clinical forms, airborne contact dermatitis interests the parts of the body exposed to the air: face, neck, upper aspect of the chest, hands, wrists. These cases must be differentiated from photocontact dermatitis; in the latter case, however, shadowed anatomic areas, such as the upper eyelids, behind the ears, the submandibular region, and under the hair, are not affected
Progetto “Scalo Internazionale Migranti”
No full textL'articolo descrive l'attività di monitoraggio e promozione della salute effettuata da alcuni professionisti sanitari volontari nel contesto dello Scalo Internazionale Migranti, nome scelto per un stabile in disuso delle ferrovie dello stato, occupato da migranti ed italiani per fronteggiare una rischiosa situazione abitativa che ha coinvolto una comunità di uomini donne e bambini, sopratutto di nazionalità rumena, rimasti senza dimora dopo l'abbattimento di una baraccopoli costruita sulle rive del fiume Reno nella città di Bologna.
Nell'articolo vengono descritti i vari determinanti di salute che gravano sulla comunità sgomberata, e viene riportata l'attività sanitari svolta all'interno del contesto sia in termini di prestazioni effettuate sia di problematiche riscontrate.
L'attività sanitaria in senso stretto ha avuto come esito:
il rilascio di 93 tesserini STP (straniero temporaneamente presente);
la presa in carico di 20 donne in gravidanza avviate all'interno dei percorsi per l' ottenimento del permesso di soggiorno ed all'appropriato monitoraggio della gravidanza;
l'avvio di 17 neonati sono stati alla pediatria di comunità ed al corretto percorso vaccinale, tra questi 12 sono stati sottoposti a screening per la TBC;
al completamento del ciclo vaccinale per la DPT per 20 bambini in età scolare;
l'avvio di 80 adulti al presidio Penumotisiologico per esami radiografici 8 dei quali hanno necessitato un ulteriore approfondimento con TAC torace.
Agli ospiti dello scalo è stato inoltre offerto un percorso per il disagio psichico ed uno spazio di confronto sanitario partecipativo.
L'esperienza sanitaria all'interno dello scalo ha dimostrato l'utilità di un approccio socialmente informato, responsabile e partecipato dei sanitari sottolineando il ruolo di ponte tra area della fragilità e istituzioni che questi professionisti possono ricoprire
Nummular contact eczema: Presentation of a pediatric case
Full text linkThe etiopathogenesis of the nummular eczema is not fully known. Various causative factors have been proposed, and among these only a few studies investigate the relevance of contact allergy. Here, we present a case of nummular contact eczema in an 8-year-old allergic to perfumes. Since atopic dermatitis may also present with different clinical phenotypes, among which nummular eczema is quite frequent, the possible significant differences between non-atopic and atopic nummular diseases are considered. Based on the high incidence of contact sensitization in all cases of nummular eczema, it is recommended to perform patch tests for diagnostic-preventive purposes
