Heart failure and sleep disorders
No full textAwareness of the importance of sleep-related disorders in patients with cardiovascular diseases is growing. In particular, sleep-disordered breathing, short sleep time, and low sleep quality are frequently reported by patients with heart failure (HF). Sleep-disordered breathing, which includes obstructive sleep apnoea (OSA) and central sleep apnoea (CSA), is common in patients with HF and has been suggested to increase the morbidity and mortality in these patients. Both OSA and CSA are associated with increased sympathetic activation, vagal withdrawal, altered haemodynamic loading conditions, and hypoxaemia. Moreover, OSA is strongly associated with arterial hypertension, the most common risk factor for cardiac hypertrophy and failure. Intrathoracic pressure changes are also associated with OSA, contributing to haemodynamic alterations and potentially affecting overexpression of genes involved in ventricular remodelling. HF treatment can decrease the severity of both OSA and CSA. Indeed, furosemide and spironolactone administration, exercise training, cardiac resynchronization therapy, and eventually heart transplantation have shown a positive effect on OSA and CSA in patients with HF. At present, whether CSA should be treated and, if so, which is the optimal therapy is still debated. Exercise testing in the clinical management of patients affected by pulmonary arterial hypertension
No full textPatients affected by pulmonary arterial hypertension (PAH) show a reduced exercise tolerance with early occurrence of dyspnoea and fatigue. The origin of functional capacity limitation is multifactorial and several mechanisms have been proposed, including right heart failure, which leads to a limited increase in cardiac output during exercise, and hyperventilation with a reduced perfusion of properly ventilated alveoli. In addition, abnormalities in arterial blood gases are observed, with the occurrence of hypoxemia and hypocapnia, related to an abnormal ventilation/perfusion match, gas diffusion abnormalities, low mixed venous oxygen saturation and to the development of intra- and extra-pulmonary right-to-left shunts. At present, the 6-minute walking test is the most used method to assess exercise tolerance in PAH; it is also useful to monitor the response to therapy and provides prognostic information. However, the assessment of functional capacity by cardiopulmonary exercise test (CPET) seems to be more complete, because CPET allows for discrimination between the metabolic, cardiovascular and pulmonary components of exercise limitation. Moreover, CPET estimates the severity of disease and assesses patients' prognosis and response to therapy. In PAH, a typical CPET-response is observed, characterized by a severe reduction in peak VO(2), work rate, O(2) pulse and anaerobic threshold and by a marked increase in VE/VCO(2) slope and in the dead space to tidal volume ratio. However, the use of CPET should be limited to experienced centres. This review will focus on resting lung function and exercise tolerance tests, showing that CPET can provide the physiological explanation of functional limitation in PAH

No full textI just wrote a few hours ago, a propos of Selected Stories from Panchatantra, that it was curious that the same publisher put out two Panchatantra books by the same author but different illustrators within about four years of each other. Then again, there is yet another Panchatantra book within this series pictured on the back cover of this book! And this is that book pictured on that back cover! It matches its series mate in format: 7 x 9¼. It has the same author, illustrator, and length of 120 pages. It offers 38 numbered fables. There is again a T of C on 3-4. Among many old friends, I have enjoyed reading Jackal Remains Jackal (13). A lioness brings up a jackal with her cubs but notices that the jackal hangs back while the cubs attack an elephant. The lioness tells the jackal what he is and tells him that her cubs will never tolerate the company of a coward. The talkative tortoise here flies with swans (58). The foolish monkeys blow on their non-fire with bamboo pipes (66). La Fontaine's cunning old cat judges between the squirrel and the mouse on 115-18. The curious monkey on 118 loses his tail in the split log. The cover proclaims Illustrated in Colour, and there are one or two larger-than-half-page colored illustrations for each of the stories. Most curious of them may be the detailed presentation of the bedbug and the mosquito on the fat king's bed (29). The most dramatic may be the crab's throttling of the crane in mid-air on 91.Retold by Rashmi Jaiswa
Heart failure outpatient clinics resources in Italy: a viewpoint of Italian Society of Cardiology organization
No full textThe current paper reports the model organization, level of health care, and type of medical and research activities related to the existing heart failure centers of the Italian Society of Cardiology. Of note, we conduced an internal survey among the members of heart failure working group and related hospital and territorial sites about the quality of care and assistance levels according to the local hospital resources and type of diagnostic therapeutic and management resources. Thirty-two hospital ambulatorial structures have been identified, the centers were equally distributed within the national ground, with similar concentration between north and south regions of the Italian country. We distinguished three different levels of organization: (1) basal territorial clinics in which patients with suspected or already diagnosed heart failure (HF) are initially identified and screened; (2) intermediate clinics in which HF patients can be routinary followed by HF specialists supported by a dedicated staff including imaging and arrythmologist experts, and interventional cardiologist; (3) advanced clinics composed by all the technical and staff resources capable of guarantying repetitive invasive assessment, continuous invasive monitoring, dedicated telemedicine structures focused on more advanced HF management integrated by heart transplantation or mechanical assistance programs. Different type of assistance is supported by a relevant number of research activity primarily conducted by the Italian Society of Cardiology or spontaneous studies arranged by HF specialist members. The number of HF centers has increased over the past few decades in proportion to the progressive rise in HF diagnoses and associated hospitalization. The expansion of ambulatory structures has been facilitated by an increasing socioeconomic and research influence. The quality of HF services in Italy could be raised by improving the network and connections between HF specialists, general practitioners (GPs), caregivers, and other specialists frequently working in this field

No full textIt is curious that the same publisher put out two Panchatantra books by the same author but different illustrators within about four years of each other. Then again, there is yet another Panchatantra book within this series pictured on the back cover of this book! This slightly smaller-format (7 x 9¼) paperback is like Alka's Panchatantra Stories, for which I guessed a date of 2001. It offers 31 numbered fables on 120 pages. There is a T of C on 3-4. This collection features standard Panchatantra stories. New to me is a story prominently pictured on the book's cover: Greed and the Strange Wheel (16) has four friends searching for wealth. The last of them, too greedy to want to share the others' wealth, encounters a man who like him had been searching for diamonds. This man has a wheel revolving around his bloody head. Finding this new searcher at last relieves him of his burden, and now the new victim must wait for another greedy searcher to come. Again here, GGE appears as a Panchatantra story (23), as does MSA (39), again with a washerman as protagonist. On 73, a weaver with a wish to be granted is prevailed upon by his wife to ask for two more hands; the villagers think he is some evil spirit and beat him to death. The second-to-last story involves an iron bar that is allegedly eaten by mice (111). The cover proclaims Illustrated in Colour, and there are one or two larger-than-half-page colored illustrations for each of the stories.Retold by Rashmi Jaiswa
