La modalità di selezione degli stati del mondo nel compito di Wason in versione reasoning about e reasoning from
Il compito di selezione di Wason corrobora l’ipotesi della dominio-specificità del ragionamento umano?
Difference in performance in Wason selection tasks between descriptive and deontic sentences - or specific subclasses of deontic sentences - has been interpreted by some scholars as an experimental evidence of the domain-specificity of human reasoning and of the mind's massive modularity hypotheses. An alternative explanation of this difference is that the two tasks are incomparable because they are structurally different and therefore require different cognitive processes. In a between-subject study we presented four selection tasks with analogous structure that differed only for the rule content (descriptive, deontic, social contract and precaution). Results showed no difference between the experimental conditions and suggest that the difference between descriptive and deontic tasks, found in other studies, was due to the type of reasoning required by the experimental instructions rather than to the specific content of the conditional sentences
Hypomethylating agents for the treatment of acute myeloid leukemia in the elderly: for all, none, or which patients?
Il compito di selezione di Wason corrobora l’ipotesi della modularità del ragionamento umano?
Is outcome of older people with acute myeloid leukemia improving with new therapeutic approaches and stem cell transplantation?
Introduction: The clinical outcome of older patients with acute myeloid leukemia (AML) is still poor, especially for those who are unfit to treatments aimed at altering the natural course of the disease. Hypomethylating agents (HMA) offer an important therapeutic opportunity to a consistent number of patients, but long-term results are largely unsatisfactory. Area covered: Recently, a number of new agents have been registered for AML, some of which selectively available for older patient population, with promising results in terms of response rate and survival. Furthermore, the upper age limit for allogeneic stem cell transplantation is constantly increasing, so that this procedure is offered and actually given to an increasing number of older patients with AML. A literature review was conducted of the PubMed database for articles published in English as well as for abstracts from most important and recent hematology meetings on AML in older patients. Expert opinion: Appropriate selection among different options on the basis of clinical fitness and molecular findings at diagnosis as well as at relapse would result in improvement of therapeutic results, sparing unnecessary toxicity and optimizing health systems resources

Laparoendoscopic extraperitoneal surgical techniques for ventral hernias and diastasis recti repair: a systematic review
Purposethis systematic review aims to classify and summarize the characteristics and outcomes of the different laparoendoscopic extraperitoneal approaches for the repair of ventral hernias and diastasis recti described in the last 10 years.Methodsa literature search was performed by two reviewers in December 2023 including articles from January 2013, 01 to December 2023, 15. The techniques were selected according to the surgical access site (anterior or posterior to the rectus sheath), the access type (laparoendoscopic, single incision laparoscopic, mini or less open), the main space used to repair the defect (subcutaneous or retromuscular) and the mesh place (onlay, sublay-retromuscular or sublay-preperitoneal) and classified as anterior or posterior approaches.Resultsthe literature search retrieved 1755 results and 27 articles were included in the study. The studies included 1874 patients, the mean age ranged from 37.8 to 60.2 years. The access site was anterior in 16 cases and posterior in 11 cases. The mesh was positioned onlay in 13 cases and sublay in 13 cases, with only one study using no mesh. Complications were: seroma, ranging from 0.8 to 81%, followed by skin complications (leak, ischemia, necrosis) from 0.8 to 6.4%, surgical site infections and bleeding. Recurrences ranged from 0% to 12,5%, with a mean follow-up from 1 to 24 months.Conclusionthis systematic review confirms the presence of several new minimally invasive extraperitoneal techniques for the repair of abdominal wall defects, with different advantages and disadvantages. Further studies, with more extensive follow-up data and wider patient groups, are necessary to define specific indications for each technique
