Clinical implication of thiopurine methyltransferase polymorphism in children with acute lymphoblastic leukemia: A preliminary Egyptian study
Background: 6-mercaptopurine (6-MP) is an essential component of pediatric acute lymphoblastic leukemia (ALL) maintenance therapy. Individual variability in this drug-related toxicity could be attributed in part to genetic polymorphism thiopurine methyltransferase (TPMT). Aim: To investigate the frequency of common TPMT polymorphisms in a cohort of Egyptian children with ALL and the possible relation between these polymorphisms and 6-MP with short-term complications. Materials and Methods: This study included 25 children. Data related to 6-MP toxicity during the maintenance phase were collected from the patients′ files. DNA was isolated and genotyping for TPMT G460A, and A719G mutations were performed by polymerase chain reaction-restriction fragment length polymorphism. Results: Twenty (80%) of the included 25 patients had a polymorphic TPMT allele. TPMTFNx013A was the most frequent (14/25, 56%), 8 patients were homozygous and 6 were heterozygous. TPMTFNx013C mutant allele was found in 4 patients (16%) in the heterozygous state while 2 patients (8%) were found to be heterozygous for TPMTFNx013B mutant allele. TPMT mutant patients, especially homozygous, were at greater risk of 6-MP hematological toxicity without significant difference regarding hepatic toxicity. Conclusions: TPMT polymorphism was common among the studied group and was associated with increased risk of drug toxicity. A population-based multi-center study is required to confirm our results

Structural adjustment in Egypt : the case of agriculture
This thesis assesses the appropriateness and effectiveness of the economic reform and structural adjustment programme adopted by Egypt in 1991. It also examines the main political and economic constraints of such policies. It argues that it is unlikely to provide sustainable or equitable growth. It also argues that privatisation
programmes in Egypt have a limited effect on improving levels of investment and growth. It stresses the need to encourage new investment to increase the productive
capacity of the Egyptian economy in order to generate sustainable growth. The thesis examines in particular, economic reforms in the agricultural sector. It focuses
on assessing price reforms so as to enquire how evenly distributed the benefits have been among farmers with different sizes of land holding. Those with big farms are
likely to gain more while very small farmers and the landless are likely to lose. Within agriculture the thesis assesses the impact of structural adjustment on Egypt's
agribusiness community. This sector is economically fragmented, has limited effectiveness and is politically weak regarding its participation and its influence on
economic policy. The thesis argues that the success Df this sector is based on the availability of a strong and effective state to provide the legal and regulatory.

Mercury removal from aqueous solutions using silica, polyacrylamide and hybrid silica-polyacrylamide aerogels
Mercury(II) ions adsorption from aqueous solutions onto silica, polyacrylamide, and hybrid silica-polyacrylamide aerogels is studied. The aerogels structure was verified by FTIR spectroscopy and their texture by nitrogen adsorption. The adsorbents were tested under different experimental conditions where the effect of temperature, pH, contact time, initial mercury(II) concentration, and aerogels quantity were investigated. The mercury adsorption onto the three aerogels was shown to be very fast, with the fastest being performed at 45 °C onto the hybrid aerogels. pH 11 was revealed optimum indicating a superlative surface interaction between the adsorbent and the adsorbate. The adsorption kinetics follows a pseudo second-order pointing out the co-existence of chemisorption and physisorption with the intra-particle diffusion being the rate controlling step. The mercury(II) adsorption fits well with Langmuir adsorption isotherms where the polyacrylamide aerogels showed the highest adsorption capacity, followed by the hybrid aerogels. The regeneration of the aerogels at pH 2 and their reuse at pH 11 was conducted for three consecutive reuses where the adsorption capacity was successfully maintained. The hybrid aerogels were found to be the most economically interesting adsorbents due to their noticeable adsorptive capacity after regeneration coupled with their no-swelling behavior. © 2010 Elsevier B.V. Glucose metabolism abnormalities among pediatric acute lymphoblastic leukemia survivors: Assessment and relation to body mass index and waist to hip ratio
AbstractBackgroundAs survival rates of pediatric acute lymphoblastic leukemia (ALL) improve, attention is turning to side and late effects of therapy including glucose metabolism abnormalities.ObjectiveTo asses the presence of abnormal glucose metabolism in pediatric ALL survivors and its possible relation to body mass index (BMI), waist to hip ratio and treatment related factors.Subjects and methodsRetrospective study with a prospective follow-up of 12 ALL survivors who had been off chemotherapy for >9months was done. Fifteen healthy sex and age matched children were involved as controls. Body mass index (BMI) waist to hip ratio (WHR), and Oral glucose tolerance test (OGTT) were performed with assessment of glycated hemoglobin (Hb A1C) and insulin sensitivity indices.ResultsAt study time the mean BMI, WHR, all components of the OGTT (except the 2h post load glucose), all indices of insulin sensitivity and the mean Hb A1C% were significantly higher compared to those of the controls. Two survivors (16.6%) developed transient hyperglycemia during therapy, one (8.3%) had pre-diabetes, seven (58.3%) had a risk level of Hb A1C but no one had diabetes mellitus (DM) or insulin resistance (IR). At study time the two survivors with transient hyperglycemia during therapy had a significantly high WHR compared to the remainders. WHR of the survivors at study time correlated significantly with fasting plasma glucose and area of insulin under the curve (AUC). The 2h post-prandial plasma glucose correlated with the duration after therapy completion.ConclusionsWHR may play a better role than BMI in the prediction of insulin resistance in those patients. Hb A1C may increase earlier than other indices of glucose tolerance
