    The submitted publications relate to research on the epidemiology and control of diseases of importance in developing countries, and fall into two main classes. The first consists of research on methodological issues in statistics and epidemiology arising in the course of the field research. The second consists of substantive epidemiological research carried out in collaboration with scientists in developing countries over twenty years.The methodological research includes work on methods for assessing whether the change in a measured value depends on its initial value; alternative measures of diarrhoeal morbidity; design issues in vaccine trials; use of the case-control study design to investigate risk factors for severe malaria; the development and validation of verbal autopsy instruments to diagnose cause of death in countries with inadequate vital registration; studies of the bias resulting from poor methodological quality in randomised controlled trials; and use of attributable fraction methods to estimate the proportion of febrile episodes attributable to malaria and to evaluate alternative malaria case definitions. Design issues in cluster-randomised trials, particularly in the context of HIV intervention trials in developing countries, are also discussed.Among many publications on HIV infection is a key paper discussing the interaction of HIV and other sexually-transmitted diseases (STDs). This argued the need for randomised trials of the impact of STD control on the HIV epidemic, and led to the first such trial in Mwanza Region, Tanzania. The Mwanza trial showed that improved STD treatment services resulted in a 40% reduction in HIV incidence in the general population, and that the intervention was highly cost-effective. This was the first randomised trial anywhere in the world to demonstrate the impact of prevention measures on HIV incidence in the general population. Other papers explore risk factors for HIV-1, HIV-2 and other STDs in a range of settings, the interaction of HIV and tuberculosis, the natural history of HIV infection, simulation models of HIV and other STDs, and the use of Herpes simplex virus type 2 infection as a marker of sexual risk behaviour in rural Africa.In research on sickle cell disease, conducted with the MRC Laboratories in Jamaica, key achievements were: clarification of haematological risk factors for proliferative sickle retinopathy, and the demonstration that some "established" risk factors were a statistical artefact due to age-confounding; the first study in which representative subgroups of sickle-cell patients with different alpha-thalassaemia genotypes were defined and analysed; and comprehensive analyses of the steady-state haematology of adult patients with homozygous sickle cell diseaseCollaborative research on malaria with the MRC Laboratories in The Gambia include a controlled trial of chemoprophylaxis in childhood; one of the first published trials of the health impact of insecticidetreated bednets, which led on to many other trials; immuno-epidemiological studies of malaria in Gambian children; and trials of folate and iron supplements. Other studies include the first field trial of the SPf66 malaria vaccine in Africa.Studies of other diseases include an analysis of admissions to a rural Ugandan hospital over a twentyseven year period; the first study to provide clear evidence of the development of acquired resistance to urinary schistosomiasis; research on leptospirosis in the West Indies; and studies of child morbidity and nutrition in The Gambia.A.
