    Incremental large and small muscle mass exercise in patients with heart failure: Evidence of preserved peripheral hemodynamics and metabolism

    Aim: Doubt still remains as to whether peripheral vascular and skeletal muscle dysfunction accompany the compromised cardiac function associated with heart failure (HF). The aim was to examine the effect of HF on the hemodynamic and metabolic responses to exercise with both a large (cycle) and small (knee-extensor, KE) muscle mass in comparison to well-matched healthy controls (Ctrls). Methods: Utilizing blood sampling and thermodilution blood flow measurements we studied incremental cycle and KE exercise in 12 patients with HF (ejection fraction: 25 ± 3%) and 8 Ctrls. Results: Incremental cycle exercise in both groups (HF: 23 ± 1.3 to 116 ± 10 W; Ctrls: 22 ± 1 to 137 ± 5 W) resulted in a similar rise in leg blood flow (HF: 1525 ± 132 to 4216 ± 408 ml/min; Ctrls: 1774 ± 161 to 4713 ± 448 ml/min), oxygen consumption (HF: 206 ± 24 to 586 ± 34 ml/min; Ctrls: 252 ± 21 to 747 ± 89 ml/min), and lactate efflux across the leg (HF: 479 ± 122 to 4929 ± 1255 mM/min; Ctrls: 537 ± 155 to 5776 ± 1010 mM/min). Vascular resistance fell similarly in both groups with increasing exercise intensity (HF: 66 ± 10 to 24 ± 3 mmHg/l/min; Ctrls: 69 ± 12 to 24 ± 4 mmHg/l/min). Conclusion: Incremental KE exercise also revealed similar hemodynamic and metabolic responses in both Ctrls and patients. These data do not support the hypothesis that HF is accompanied by an intrinsic muscle myopathy that affects both vascular and metabolic responses to submaximal exercise. References Esposito F et al.     Unsupervised author identification and characterization

    Author identification is a hot topic, especially in the Internet age. Following our previous work in which we proposed a novel approach to this problem, based on relational representations that take into account the structure of sentences, here we present a tool that computes and visualizes a numerical and graphical characterization of the authors/texts based on several linguistic features. This tool, that extends a previous language analysis tool, is the ideal complement to the author identification technique, that is based on a clustering procedure whose outcomes (i.e., the authors’ models) are not human-readable. Both approaches are unsupervised, which allows them to tackle problems to which other state-of-the-art systems are not applicable

[email protected] AIMS: A number of studies have suggested the importance of motor development in autism. Motor development has been considered a possible bio-marker of autism since it does not depend on either social or linguistic development. In this study, using retrospective video analysis we investigated the first unsupported gait in toddlers with autism. METHODS: Fifty-five toddlers, belonging to three groups were recruited: toddlers with autistic disorder (AD, n=20, age 14.2mo, sd 1.4mo) and as comparison groups: typically developing toddlers (TD, n=20, age 12.9mo, sd 1.1mo) and toddlers with non-autistic developmental delays of mixed aetiology (DD, n=15, age 13.1mo, sd 0.8mo). The Walking Observation Scale (WOS) and the Positional Pattern for Symmetry during Walking (PPSW) were used to gather data on the first unsupported gait. The WOS includes 11 items that analyze gait through three axes: foot movements; arm movements; general movements while the PPSW analyses static and dynamical symmetry during gait. RESULTS: Our results have identified significant differences in gait patterns among the group of toddlers with AD as opposed to the control groups. Significant differences between AD and the two control groups were found for both WOS (p<.001) and PPSW (p<.001). CONCLUSION: The specificity of motor disturbances we have identified in autism (postural asymmetry) is consistent with previous findings that implicated cerebellar involvement in the motor symptoms of autism
