Self-operated endovaginal telemonitoring: using internet-based home monitoring of follicular growth in assisted reproduction technology
No full textThe need for serial vaginal sonographies to monitor ovarian stimulation for artificial reproductive technology (ART) treatments remains a major practical and organizational drawback for both patients and health care providers. This hampers access to treatment for many couple seeking treatment using IVF or renders it strenuous or expensive from an organizational point of view. It creates a high hidden cost when people have to travel frequently to have their sonograms made or have to plan a prolonged hotel stay near the center where oocyte retrieval and IVF embryology work takes place. We are currently introducing a method allowing patients and/or their partners or, if appropriate, midwives and nurses independent from highly specialized services to make vaginal sonographies at home or near the place where patients live. Specific software has been developed that can be installed on any portable PC or tablet PC. Current experience has shown that after initial teaching many patients are prepared and competent to make and send video recordings to the center or the physician following them up. They can do this where and when it suits them best. Receipt of images is acknowledged by direct email. After analysis of the growing follicles and uterine lining, a structured response is sent to the patient, comprising dosing advice and next-step instructions. This simplification of the uncontested need to perform these follow-up sonographies, even if applicable to just a selected proportion of IVF patients, could fit in the general tendency to make IVF more patient-centered and patient-friendly, to implement telemedicine and increase patient empowerment by supervised active participation to their treatment [1].
