    A prosthetic heart valve (11) having a pair of cooperating leaflets (15) mounted in a valve body (13) to alternate between an open position where the flow of blood in a downstream direction is permitted and a closed position where the flow of blood in the reverse direction is counteracted. Pivot member sets at diametrically opposite sides of each leaflet guide the leaflets moving between the open and closed positions, each set including an ear (41) or a recess (141) cooperating with an associated cavity (25) or knob (125) of an interior surface of the valve body. Each ear or recess bounds at least one leaflet passageway (76, 77, 176) through that ear or recess and located spaced from upstream and downstream ends of the ear for allowing flow through that ear and the associated cavity, or through that recess, when the leaflets are in the closed positions

    In het eerste deel van dit proefschrift worden Fontan patiënten beschreven die bij de geboorte slechts één werkzame hartkamer (pomp) hebben. Deze jonge kinderen moeten een aantal open hartoperaties ondergaan waarbij omleidingen rondom het defecte hart worden aangelegd. Deze operaties vinden al op zeer jonge leeftijd plaats waardoor slechts kleine buizen gebruikt kunnen worden voor de omleidingen. Een deel van deze kinderen wordt te groot voor de omleidingen en moeten opnieuw geopereerd worden. Om dit te voorkomen stellen wij een bepaalde volgorde van operaties voor waardoor de kinderen meer tijd krijgen om te groeien en er mogelijk grotere buizen geimplanteerd kunnen worden. In het tweede deel van dit proefschrift beschrijven wij de gevolgen van de implantatie van een kunstklep in patienten met aangeboren hartafwijkingen (AHA) die nu volwassen zijn. Patiënten met AHA hebben vaak afwijkende hartkleppen die reden zijn voor operatie. In veel gevallen moet dan een kunstklep worden geïmplanteerd. Dit kan een klep zijn die gemaakt is van kunstmateriaal of een biologische klep (meestal een varkensklep of menselijke donorklep). Patiënten met een AHA en een kunstklep verschillen van patiënten met verworven afwijkingen die een kunstklep hebben. Ze zijn jonger, ze zijn actiever, en ze hebben andere hartziekten. Patiënten met AHA worden met de huidige operatieve technieken en intensieve zorg steeds ouder en wij zien dat bijna 50% te maken krijgt met complicaties van de kunstklep. Deze lange termijn complicaties waren nog niet eerder op deze schaal in kaart gebracht en staan in verschillende hoofdstukken in dit proefschrift beschreven
