    Familial combined hyperlipidemia: current status of the problem (literature review)

    Among the various dyslipidemia types, familial combined hyperlipidemia (FCH) is the most common genetic disorder, which is characterized by at least two dif­ferent forms of lipid disorders: hypercholesterolemia and hypertriglyceridemia. In FCH, the risk of atherosclerotic cardiovascular diseases (CVDs), including at a young age, increases significantly. The aim was to analyze literature data on modern criteria for diagnosis, pathogenesis and data from molecular genetic studies of FCH. Future studies aimed at understanding the underlying genetic and metabolic mechanisms of FCH and developing effective treatment strategies should include larger cohort studies with greater genetic diversity, as well as inves­tigation of epigenetic and lifestyle factors

Современные организации функционируют в динамичном мире и адаптируются к различным проявлениям этой динамики, проводя организационные изменения. Исследование специфических адаптационных ресурсов отдельных групп субъектов выявило возможности дифференцированного подхода в работе с персоналом в условиях организационных изменений. Обоснована дифференциация выборки по типам выбора субъектами жизненного пути. Цель. Исследовать специфику адаптационных ресурсов субъектов в условиях организационных изменений, имеющих разный тип выбора жизненного пути и особенности переживания стресса, жизнестойкости, толерантности к неопределенности. Материалы и методы исследования. В исследовании приняли участие 208 работников крупного промышленного предприятия г. Челябинска (из них 135 женщин и 73 мужчины). Применялась батарея следующих методик: методика «Типология личностного выбора жизненного пути» В.Г. Грязевой-Добшинской, А.С. Мальцевой; «Шкала психологического стресса» Л. Лемура, Р. Тесье, Л. Филлиона (в адаптации Н.Е. Водопьяновой); «Тест жизнестойкости» С. Мадди (в адаптации Д.А. Леонтьева, Е.В. Рассказовой); «Шкала толерантности к неопределенности» Д. МакЛейна в адаптации Е.Г. Луковицкой. Для статистической обработки использовался дискриминантный и факторный анализ данных. Результаты. Выявлены общие (неспицифические) адаптационные ресурсы: у субъектов всех типов выбора жизненного пути выявлена обратная зависимость параметров подверженности стрессу и жизнестойкости как ресурсов стрессоустойчивости. Выявлены специфические адаптационные ресурсы: у субъектов с гедонистическим типом – положительное отношение к новым, разнообразным ситуациям в жизни, предпочтение неопределенности; у субъектов с ценностным типом выбора – произвольный уровень отношения к неопределенным и сложным ситуациям; у субъектов с творческим типом – комплекс свойств, включающих «принятие риска» и всех компонентов «толерантности к неопределенности». Background. The modern world is characterized by instability, uncertainty, complexity and ambiguity. Therefore, a successful development of an organization requires organizational changes, which provoke the necessity of adaptation for people working in this organization. The study of specific adaptive resources revealed the benefits of a differentiated approach to personnel undergoing organizational changes. The differentiation of the sample depending on the strategies for choosing a way of life is justified. Aim. The paper aims to study the specifics of adaptive resources in the context of organizational changes and the features of experiencing stress, tolerance to uncertainty and resilience. Materials and methods. The study involved 208 employees of a large industrial enterprise in Chelyabinsk (of which 135 females and 73 males). The following methods were used during the study: typology for a personal choice of a way of life by V. Gryazeva-Dobshinskaya, A.S. Maltseva; the Psychological Stress Measure (PSM) by L. Lemyrr,R. Tessier, L. Fillion (adapted by N.E. Vodopyanova); the Hardiness test by S. Maddi (adapted by D. A. Leontiev, E. V. Rasskazova); the tolerance to uncertainty scale adapted by E.G. Lukovitskaya. For statistical processing, discriminant and factor analysis were used. Results. General (nonspecific) adaptive resources were revealed: in all subjects regardless of their strategies for choosing a way of life, an inverse correlation was found between stress exposure and resilience as stress resistance resources. Specific adaptive resources were identified: subjects of a hedonistic type had a positive attitude towards new, diverse situations in life and were characterized by a preference for uncertainty; subjects of a value-oriented type were characterized by an arbitrary level of attitude to uncertain and complex situations; creative subjects demonstrated a set of properties, including “challenge” and all the components of “uncertainty tolerance”

    Taking into account the ideas of the generalized two-level scheme, within the framework of the semiclassical approach to the consideration of the resonant interaction of the light field with matter, an analytical solution to the problem of the evolution of superradiance in a quasi-two-dimensional supercrystal formed by quantum dots is obtained. The calculation was carried out for the physical parameters of a semiconductor structure with quantum-well effects in the presence of resonant nonlinearity and intraband relaxation.С учетом представлений обобщенной двухуровневой схемы в рамках полуклассического подхода к рассмотрению резонансного взаимодействия светового поля с веществом получено аналитическое решение задачи об эволюции сверхизлучения в квазидвумерном суперкристалле, образованном квантовыми точками. Расчет проведен для физических параметров полупроводниковой структуры с квантоворазмерными эффектами при наличии резонансной нелинейности и внутризонной релаксации

    Searching for new genes associated with the familial hypercholesterolemia phenotype using whole-genome sequencing and machine learning

    One of the most common congenital metabolic disorders is familial hypercholesterolemia. Familial hyper-cholesterolemia is a condition caused by a type of genetic defect leading to a decreased rate of removal of low-density lipoproteins from the bloodstream and a pronounced increase in the blood level of total cholesterol. This disease leads to the early development of cardiovascular diseases of atherosclerotic etiology. Familial hypercholesterolemia is a monogenic disease that is predominantly autosomal dominant. Rare pathogenic variants in the LDLR gene are present in 75–85 % of cases with an identified molecular genetic cause of the disease, and variants in other genes (APOB, PCSK9, LDLRAP1, ABCG5, ABCG8, and others) occur at a frequency of < 5 % in this group of patients. A negative result of genetic screening for pathogenic variants in genes of the low-density lipoprotein receptor and its ligands does not rule out a diagnosis of familial hypercholesterolemia. In 20–40 % of cases, molecular genetic testing fails to detect changes in the above genes. The aim of this work was to search for new genes associated with the familial hypercholesterolemia phenotype by modern high-tech methods of sequencing and machine learning. On the basis of a group of patients with familial hypercholesterolemia (enrolled according to the Dutch Lipid Clinic Network Criteria and including cases confirmed by molecular genetic analysis), decision trees were constructed, which made it possible to identify cases in the study population that require additional molecular genetic analysis. Five probands were identified as having the severest familial hypercholesterolemia without pathogenic variants in the studied genes and were analyzed by whole-genome sequencing on the HiSeq 1500 platform (Illumina). The whole-genome sequencing revealed rare variants in three out of five analyzed patients: a heterozygous variant (rs760657350) located in a splicing acceptor site in the PLD1 gene (c.2430-1G>A), a previously undescribed single-nucleotide deletion in the SIDT1 gene [c.2426del (p.Leu809CysfsTer2)], new missense variant c.10313C>G (p.Pro3438Arg) in the LRP1B gene, and single-nucleotide deletion variant rs753876598 [c.165del (p.Ser56AlafsTer11)] in the CETP gene. All these variants were found for the first time in patients with a clinical diagnosis of familial hypercholesterolemia. Variants were identified that may influence the formation of the familial hypercholesterolemia phenotype

    Long-term uroseptic and metabolic effects of a herbal medicinal preparation in women with type 2 diabetes taken gliflosins

    Background. The problem of urinary tract infections, including recurrent cystitis, and the choice of effective and safe means of treatment and prevention, taking into account the increasing burden of antibiotic resistance, are extremely relevant. Aim. To evaluate the effectiveness and safety of the herbal medicine Canephron® N for the prevention of exacerbations of recurrent cystitis and its effect on metabolic parameters in patients with type 2 diabetes mellitus taking sodium glucose cotransporter type 2 inhibitors. Material and methods. A prospective, randomized, open-label, parallel group study of 60 women without exacerbation of recurrent cystitis. The main group took the drug Canephron® N for 3 months, 2 tablets 3 times a day, and for the next 3 months they were monitored without taking the drug. The main evaluation parameters were: frequency of cystitis relapses, albumin/creatinine ratio in the morning urine, glomerular filtration rate, plasma lipid profile, glycemia and malondialdehyde in low-density lipoproteins. Results. 3 months after the end of taking the drug Canephron® N, patients in the main group maintained a positive effect in reducing exacerbations of cystitis – 3.3 times less relapses of the disease (3 and 10%, respectively; p=0.038). The urine albumin/creatinine ratio, lipid profile, glycemia and malondialdehyde in low-density lipoproteins after a 3-month treatment period had positive dynamics, but in the long-term period they corresponded to the values at the inclusion stage, both in the main and control groups. Conclusion. Prophylactic use of the herbal drug Canephron® N for 3 months is safe and reduces the frequency of relapses of cystitis in women with chronic cystitis against the background of diabetes mellitus taking sodium glucose cotransporter type 2 inhibitors, this effect persists for 3 months after discontinuation of the drug. Positive metabolic effects are associated with taking Canephron® N and are gradually leveled out when it is discontinued

    Prognosis of patients with coronary artery disease and diabetes — what to look for

    Patients with coronary artery disease (CAD) and diabetes are identified as a high cardiovascular risk group both in Russia and abroad. Having similar paths of development and progression of atherosclerosis and atherothrombosis, these diseases potentiate each other's thrombogenesis mechanisms. As a result, cardiovascular ischemic events occur, making a significant contribution to the mortality rates of this group of patients. Influencing the risk of thrombus formation is one of the key links on the way to improving the prognosis of patients with CAD and diabetes. Modern antithrombotic therapy, including not only the antiplatelet agent aspirin, but also the anticoagulant rivaroxaban at a dose of 2,5 mg, allows for the maximum impact on the development of atherothrombosis, thereby reducing the risks of cardiovascular events and all-cause death. The aim of the review was to analyze the potential of reducing the all-cause death risk by prescribing the main drug groups that are part of the therapy of patients with CAD and diabetes

    Photoacoustic effect in micro- and nanostructures: numerical simulations of Lagrange equations

    The  work  provides  the  description  of  theoretical  and  numerical  modeling  techniques of thermomechanical effects that take place in absorbing micro- and nanostructures of different materials under the action of pulsed laser radiation. A proposed technique of the numerical simulation is based on the solution of equations of motion of continuous media in the form of Lagrange for spatially inhomogeneous media.
