Open fractures from the clinical-therapeutic point of view, correlated with the global conditions of the polytraumatic patient are studied. The concept of polytraumatic patient is specified and the correlations of the patient's resuscitation and nurosurgical problems are pinpoited. Associated lesions in the polytraumatic patients studied ina one year period, chosen as a sample in the Author's Dept., are abalyzed and a classification based an the presence at the same time of one or more fractures and of a cranio-thoraco-abdominal lesions, are proposed. Contraindications and advantages of immediate and global treatment of all lesions presented by a polytraumaitc patient are discussed. The Authors' conclusion on the grounds of their experience and of the valuable results abtained are in favour of the treatment in emergency of all the existing lesions

In patients in whom extremity fractures form part of a multiple injury pcture it is often necessary for fractures to be stabilised as early as possible for better management of the overall situation and to avoid further complications connected with forced decubitus. AS both conservative treatment and osteosynthesis with a plate for a variety of reasons do not meet the needs of humeral fracture treatment in these patients, the use of external fixation is proposed in fractures of proximal humeral epiphisis and in plurifragmentary fractures of the disphysis, and of Rush's nail in certian fractures of the surgical neck and humeral disphysis. This treatment permits speedy patient mobilisation and good healing of humeral fractures. WE present 38 cases treated with this method, and good result are showed

Conservative treatment in a plaster cast and internal osteosynthesis, fall for different reasons to solve the problems of complex, comminuted and intra-articular fractures of the distal end of the radius. Hoffmann's external fixation is therefore proposed for this offers a considerable number of advantages: speed of operation, blind reduction, early mobilisation, rapid callus, variability in wrist position, atraumaticity of fixator removal, low cost and tolerance of the apparatus. We present 17 cases treated with this method, and good results
