    Split obturator: An innovative approach

    A palatal prosthesis can improve function by closing the palatal defect, preventing regurgitation, and improving swallowing and speech. Although techniques have been previously described for fabrication of palatal obturator, but there has not been any technique to devise an obturator for a patient with palatal defect with a quadhelix orthodontic appliance overlying it. One cannot wait in such patients for completion of lengthy orthodontic treatment and then think of devising prosthesis as the patient cannot carry out normal functions like swallowing and speech without the closure of defect. This article focuses on an innovative method of fabricating a palatal obturator which aims at restoring the above-mentioned functions along with improving aesthetics. It also enables us to devise the fabrication of prosthesis in two parts for easy insertion and removal and as well as to be self-cleansable.     Peripheral giant cell granuloma

    Peripheral giant cell granuloma or the so-called "giant cell epulis" is the most common oral giant cell lesion. It normally presents as a soft tissue purplish-red nodule consisting of multinucleated giant cells in a background of mononuclear stromal cells and extravasated red blood cells. This lesion probably does not represent a true neoplasm, but rather may be reactive in nature, believed to be stimulated by local irritation or trauma, but the cause is not certainly known. This article reports a case of peripheral giant cell granuloma arising at the maxillary anterior region in a 22-year-old female patient. The lesion was completely excised to the periosteum level and there is no residual or recurrent swelling or bony defect apparent in the area of biopsy after a follow-up period of 6 months

    Restoring Esthetics by Immediately Loaded Implant Placement

    The replacement of a single missing anterior tooth with an implant-supported crown is a demanding therapy. This report describes a treatment in which an anterior maxillary implant was immediately restored with a provisional restoration. During the provisional phase, an optimal emergence profile was created by adjusting the provisional restoration. An impression was made with an individually fabricated impression post for an accurate reproduction of the established emergence profile and, finally, a screw-retained all-ceramic crown was placed. By implementing this protocol, an optimal definitive result could be achieved, together with immediate patient satisfaction.</jats:p

    Integration of Electro-Mechanical Impedance technique with global vibration technique

Traditional damage detection techniques, also known as local damage detections, such as visual inspection and ultrasonic testing, have been implemented since the mid 20th century. However, these techniques often require prior knowledge of potential damage locations and require bulky testing equipment. Alternative techniques, the Global Vibration Techniques, were first introduced to analyze the modal information of the structure to assess its overall health state. The drawback of these methods is their insensitivity towards the incipient local damage. With the development of sensor technology, a local damage detection technique, the Electro-Mechanical Impedance (EMI) method, has emerged. EMI measures the electrical admittance by the impedance analyzer, and evaluates the health status of the structure by comparing the baseline signature with the damaged signature. It allows users to access the structure remotely, but it loses its sensitivity when the damage is significant. Therefore, Bhalla, Shanker and Gupta proposed integrating the Global Vibration Techniques with the EMI technique so as to tap on the strengths of the respective techniques. This new method, the Integration of Global Vibration Technique and EMI Technique, draws on EMI's high sensitivity towards early incipient damage and Global Vibration Techniques' sensitivity at late damage stages. The author further examines the integrated method in terms of practicality and scalability.     Fabrication of Custom Ocular Prosthesis Using a Graph Grid: A Case Report

    Patients&nbsp; requiring&nbsp; treatment&nbsp; with&nbsp; custom&nbsp; ocular&nbsp; prosthesis&nbsp; are&nbsp; those&nbsp; who&nbsp; have&nbsp; lost&nbsp; ocular&nbsp; structures through orbital evisceration or orbital enucleation which was necessary as a surgical intervention for a congenital&nbsp; defect,&nbsp; pathology&nbsp; or&nbsp; an&nbsp; accident.&nbsp; The&nbsp; disfigurement&nbsp; associated&nbsp; with&nbsp; eye&nbsp; loss&nbsp; can&nbsp; cause significant&nbsp; physical&nbsp; and&nbsp; emotional&nbsp; disturbance.&nbsp; As&nbsp; iris&nbsp; placement&nbsp; is&nbsp; a&nbsp; technique&nbsp; sensitive&nbsp; procedure, visual assessment alone may not be accurate.The&nbsp; present article&nbsp; is an illustration of a case report&nbsp; of&nbsp; a lady chosen for a custom ocular prosthesis. The method described here uses a transparent grid template from which the iris is traced.     The Detection and Role of Human Endogenous Retrovirus K (HML-2) In Rheumatoid Arthritis

    A thesis submitted in partial fulfilment of the requirements of the University of Wolverhampton for the degree of Doctor of PhilosophyHuman endogenous retroviruses are the remnants of ancient retroviral infections present within our genome. These molecular fossils show similarities with present day exogenous retroviruses but act as typical Mendelian elements that are passed vertically between generations. Despite being repeatedly linked to a number of autoimmune diseases and disorders, no conclusive proof has been identified. Rheumatoid arthritis (RA) is one such disease which has been associated with an increase in HERV expression, compared to controls. In order to elucidate a clear role for HERVs in RA pathogenesis, autoantigens implicated in disease pathogenesis were scanned for sequence homology to retroviral genes. Such epitopes would induce antibodies cross reactive with host proteins, resulting in disease. Short peptides mimicking these regions were synthesised and the prevalence of anti-HERV antibodies was determined in RA patients and disease controls. Additionally, a novel real-time Polymerase Chain Reaction (PCR) assay was developed to accurately quantify levels of HERV-K (HML-2) gag expression, relative to normalised levels of housekeeping gene expression. Both serological and molecular assays showed significant increases in HERV-K (HML-2) activity in RA patients compared to disease controls with CD4+ lymphocytes harbouring the highest activity. The real-time assay was also used to determine whether factors within the synovium could modulate HERVs, resulting in their upregulation. Exogenous viral protein expression and pro-inflammatory cytokines were shown to exert a significant modulatory effect over HERV-K (HML-2) transcription. From this data, it is clear that RA patients have increased levels of HERV-K (HML-2) gag activity compared to controls. Despite this it is likely that factors within the synovium such as exogenous viral expression and pro-inflammatory cytokines also influence HERV-K (HML-2) transcription possibly contributing to a role of bystander activation, i.e. being influenced by external factors, rather than actively contributing to disease processes.
