    Disease-free survival as a surrogate for overall survival in patients with HER2-positive, early breast cancer in trials of adjuvant trastuzumab for up to 1 year: a systematic review and meta-analysis

    Background: Although frequently used as a primary endpoint, disease-free survival has not been validated as a surrogate for overall survival in early breast cancer. We investigated this surrogacy in the adjuvant setting of treatment with anti-HER2 antibodies. Methods: In a systematic review and meta-analysis, we identified published and non-published randomised controlled trials with completed accrual and available disease-free survival and overall survival results for the intention-to-treat population as of September 2016. Bibliographic databases (MEDLINE, Embase, and Cochrane Central Register of Controlled Trials), clinical trial registries (Clinicaltrials.gov, EU Clinical Trials Register, WHO International Clinical Trials Registry Platform, and PharmNet.Bund), and trial registries from relevant pharmaceutical companies were searched. Eligibility for treatment of HER2-positive early breast cancer required at least one group to have an anti-HER antibody treatment (ie, trastuzumab, pertuzumab, or trastuzumab emtansine) planned for 12 months, and at least one control arm with chemotherapy without the antibody, a lower total dose or duration of the antibody, or observation alone. Units of analysis were contrasts: two-group trials gave rise to one contrast, whereas trials with more than two groups gave rise to more than one contrast. We excluded trials enrolling patients with recurrent, metastatic, or non-invasive disease, and those testing neoadjuvant therapy exclusively. Our primary objective was to estimate patient-level and trial-level correlations between disease-free survival and overall survival. We measured the association between disease-free survival and overall survival using Spearman's correlation coefficient (r s ), and the association between hazard ratios (HRs) for disease-free survival and overall survival using R 2 .We computed the surrogate threshold effect, the maximum HR for disease-free survival that statistically predicts an HR for overall survival less than 1·00 in a future trial. Findings: Eight trials (n=21 480 patients) gave rise to a full set (12 contrasts). Patient-level associations between disease-free and overall survival were strong (r s =0·90 [95% CI 0·89–0·90]). Trial-level associations gave rise to values of R 2 of 0·75 (95% CI 0·50–1·00) for the full set. Subgroups defined by nodal status and hormone receptor status yielded qualitatively similar results. Depending on the expected number of deaths in a future trial, the surrogate threshold effects ranged from 0·56 to 0·81, based on the full set. Interpretation: These findings suggest that it is appropriate to continue to use disease-free survival as a surrogate for overall survival in trials in HER-2-positive, early breast cancer. The key limitation of this study is the dependence of its results on the trials included and on the existence of an outlying trial.     The presence of disseminated tumor cells (DTCs) in the bone marrow has been shown to predict poor clinical outcome in early stage breast cancer. However, peripheral blood is easier to obtain and allows for real time monitoring of minimal residual disease (MRD). Towards this end, circulating tumor cells (CTCs) in the blood are detected using either direct methods, mainly antibody-based assays (immunocytochemistry, immunofluorescence, flow cytometry), or indirect methods, mainly nucleic acid-based assays (detection of mRNA transcripts by reverse transcriptase polymerase chain reaction, RT-PCR). The detection of CTCs using RT-PCR for CK19 was shown to be an independent prognostic factor in women with early breast cancer. Furthermore, there has been considerable progress in genotyping, phenotyping and profiling micrometastatic cells. The challenge now is to integrate minimal residual disease as a prognostic and predictive tool in the management of breast cancer.     Bullying is a form of violent behavior that has a markedly negative effect on many aspects of the daily lives of thousands of children. Through a literature review, the present study investigates the term and the nature of bullying, as well as its effect on those involved, and the conditions that prevail in Greek schools. Moreover it contains information about the role of drama in education as a means of counteracting bullying in schools, and the way various experiential drama activities can equip primary school children with the social and emotional skills in order to successfully combat the phenomenon. © 2016 The Author(s)

    Ο Νότης Μαυρουδής ανήκει στη δεύτερη μεταπολεμική γενιά των Ελλήνων κιθαριστών, αποτελώντας ένα από τα σπουδαιότερα μέλη της. Αν και σήμερα είναι ευρύτερα γνωστός για την ποιοτική προσφορά του στο ελληνικό τραγούδι, ο ίδιος ανέπτυξε ποικίλη δράση στον τομέα της κλασικής κιθάρας ως σολίστ, δημιουργώντας αξιόλογο έργο με συμμετοχές σε ρεσιτάλ, βραβεία σε διαγωνισμούς, δισκογραφία, ραδιοφωνικές και τηλεοπτικές εμφανίσεις. Η εργασία εξετάζει την πορεία του ως σολίστ, ήδη από τα πρώτα μαθητικά του χρόνια, μέχρι την οριστική του απόφαση να εγκαταλείψει τη σολιστική σταδιοδρομία. Στο πρώτο μέρος παρουσιάζονται με λεπτομέρεια όλες οι συναυλίες και τα ρεσιτάλ που έδωσε, ο τόπος, αλλά και το πρόγραμμα που ερμήνευσε. Στο δεύτερο μέρος αναλύονται οι κοινωνικοπολιτικές συνθήκες που επικρατούσαν την εποχή που έδρασε ως σολίστ και που τον επηρέασαν στο μουσικό του ταξίδι. Για τις ανάγκες της εργασίας, καθώς και για περαιτέρω μουσικολογική έρευνα, καταλογογραφήθηκαν και μετατράπηκαν σε ψηφιακή μορφή από το αρχείο του Νότη Μαυρουδή, πλήθος μουσικών προγραμμάτων, αφισών, αποκομμάτων εφημερίδων και περιοδικών. Επίσης, σημαντική πηγή πληροφοριών αποδείχθηκε η καταγραφή της προφορικής μαρτυρίας του, που πραγματοποιήθηκε σε μορφή συνέντευξης κατά το πρώτο στάδιο της έρευνας, καθώς και η αλληλογραφία του με τον Δημήτρη Φάμπα, κατά τη διάρκεια της παραμονής του πρώτου στην Ιταλία.Notis Mavroudis belongs to the second post-war generation of Greek guitarists, being one of its most important members. Although today he is widely known for his high quality contribution to Greek song, he developed a variety of activities in the field of classical guitar as a soloist, creating remarkable work with participation in recitals, awards, competitions, discography, radio and television appearances. The work examines his career as a soloist, from his early student years, until his final decision to abandon his solo career. The first part presents in detail all the concerts and recitals he gave, the place, but also the program he performed. The second part analyzes the socio-political conditions that prevailed during the time he acted as a soloist and that influenced him in his musical journey. For the needs of the work, as well as for further musicological research, a number of music programs, posters, newspaper clippings and magazines were cataloged and converted into digital form from the archive of Notis Mavroudis. Also, an important source of information proved to be the recording of his oral testimony, which took place in the form of an interview during the first stage of the investigation, as well as his correspondence with Dimitris Fampas, during his stay in Italy
