    Radiology in surgical practice

    This book presents radiological images covering the spectrum of radiology used in general surgery including plain X-rays, contrast studies, CT, MRI and nuclear medicine studies. Images in this text have been selected specifically to illustrate key features of importance in surgical diagnosis and management. Each section has been written in conjunction with a radiologist and practicing surgeon to ensure its clinical relevance. The text is divided into two main sections. The first focuses on the various forms of imaging ranging from plain film radiography up to PET scanning and nuclear medicine.     Motility across esophageal anastomoses after esophagectomy or gastrectomy

    The esophageal motility of 23 patients who underwent esophagectomy or gastrectomy for carcinoma of the esophagus or gastroesophageal junction was recorded daily during the immediate post-operative period for high-pressure transients or other motility disturbances. Patients were divided into three subgroups according to the level of the esophageal anastomosis: group 1, neck (n = 4); group 2, thoracic (n = 14); group 3, diaphragmatic hiatus (n = 5). Peristalsis was absent in all patients studied during ventilation and sedation in the intensive care unit (five patients). Early post-operative esophageal motility after esophageal anastomosis varied somewhat with the length of residual esophagus. With short lengths of residual esophagus, no consistent motility pattern emerged. With longer lengths, early peristaltic activity was evident, but diminished over the first few post-operative days. Esophageal resection and anastomosis is associated with loss of peristalsis in the initial post-operative period, consistent with the concept of an esophageal ileus.G. Mathew, J.C. Myers, D.I. The author argues that both welfare professionals and gerontologists need to pay far more consideration to belief as a constituent of well-being in later life. The book looks to the future and increasing diversity of choice in matters of belief among Britain and Europe's older citizens as a consequence of immigration and globalisation

Published by John Wiley & Sons, Ltd.Background: The development of endoscopic techniques, particularly endoscopic stapling, has led to a re-evaluation of the treatment of pharyngeal pouch. The pathophysiology and treatment of the condition is reviewed. Methods: An electronic literature search was undertaken on the pathophysiology, history and surgery of pharyngeal pouch (Zenker’s diverticulum). These last two terms were used to the search the Cochrane, Medline and Embase databases (from 1966 to date) and the bibliographies of extracted articles. Results and conclusion: With recognition of the central role of the cricopharyngeus muscle in the pathogenesis of pouch formation, the emphasis on treatment has shifted from diverticulectomy to cricopharyngeal myotomy. Minimally invasive techniques have become established since the advent of endoscopic stapling devices. Although randomized controlled data are lacking, the endoscopic approach appears to offer advantages in terms of a shorter duration of anaesthesia, more rapid resumption of oral intake, shorter hospital stay and quicker recovery.     In a certain time work agreement involving two parties, namely the P.G Mojo Sragen party and the worker, it is called a reciprocal agreement and therefore creates rights and obligations for the parties. If in the implementation of the rights and obligations between P.G Mojo Sragen and workers, one of the parties does not carry out their achievements, it will result in legal liability. The purpose of this study was to determine the process of agreement between P.G Mojo Sragen and workers, the applicable regulations and the rights and obligations of a certain time work agreement between P.G Mojo Sragen and workers, as well as legal responsibility if one of the parties made a mistake from the work agreement. The method used in this study is a normative approach because in this study, which are legal rules, legal principles in legal responsibility between P.G Mojo Sragen and workers. The type of research used by the author in this research is descriptive research to clearly describe the legal responsibility between P.G Mojo Sragen and workers. The results of this study indicate that in the process of implementing a work agreement for a certain time, prior to the occurrence of the agreement, the administrative requirements and legal requirements in accordance with Article 1320 of the Civil Code are: (1) Agreeing (2) Legal Capability (3) A certain matter (4) An lawful cause. At the time the agreement was signed with a certain time agreement P.G Mojo Sragen with workers that had been made by P.G Mojo Sragen based on a standard agreement. After the agreement, a binding legal relationship arises and gives birth to rights and obligations. If in the implementation one of the parties does not fulfill its rights and obligations, then it must be responsible on the basis of default in accordance with Article 1243 of the Civil Code and if one of the parties violates the applicable law, it can be accounted for based on unlawful acts in accordance with Article 1365 of the Civil Code

    Proximal anastomosis using the OrVil (TM) circular stapler in major upper gastrointestinal surgery

    Anastomoses in major upper gastrointestinal surgery can be technically demanding, especially handsewn anastomoses traversing the diaphragmatic hiatus. The OrVil™ stapler is a unique circular stapler that allows rapid creation of various upper gastrointestinal anastomoses in technically challenging circumstances, particularly if additional proximal clearance is desirable. Little is reported in the literature regarding its outcomes and complication rates.     Laparoscopic cardiomyotomy and anterior partial fundoplication for achalasia

    Background: Although surgical myotomy is considered the gold standard, many different treatments have been advocated for achalasia. There are now a number of reports of cardiomyotomy being performed laparoscopically. Methods: This is a prospective study of 82 patients (47 male and 35 female; median age, 47 years) who underwent laparoscopic cardiomyotomy and anterior partial fundoplication for achalasia. Results: Four of the 82 procedures required conversion to open surgery, all during the early stages of the series, and two required early reoperation for a postoperative problem. The median operating time was 80 min (range, 32-210). the median hospital stay was 3 days (range, 2-18), and normal physical activity was resumed after a median of 2 weeks (range, 3 days to 12 weeks). Follow-up ranged up to 8 years (median, 2). Postoperatively, symptoms of dysphagia (to both solids and liquids), heartburn, odynophagia, chest pain, regurgitation, and cough were significantly reduced in all patients. The median overall satisfaction score (graded from 0 to 10, with 10 representing total satisfaction) was 9 (range, 0-10), and 90% of patients were highly satisfied with the surgical outcome. Conclusion: Laparoscopic cardiomyotomy with anterior partial fundoplication achieves excellent symptomatic relief for patients with achalasia, and it can be performed with minimal morbidity.R.
