Knowledge, Attitudes, and Practices in Neonatal Diabetes Mellitus Management: the JEnious-NeOnatal-DIabetes (JENODI) Survey

    No full text
Introduction: We aimed to explore the knowledge, attitude, and management of neonatal diabetes mellitus (NDM) among members of the International Society for Pediatric and Adolescent Diabetes (ISPAD). Methods: Members of the society were invited to complete an online questionnaire posted on the ISPAD website. Results: We received 108 responses from 45 different countries. Of these, 103 were involved in NDM management. 87.9% of participants would start insulin at diagnosis, and 11% would prefer sulfonylurea (SU); 54.6% would start with an insulin pump, and 80.6% would use continuous glucose monitoring. Genetic testing was suggested by 97.2% (50.9% when diagnosis occurs up to 6 months, 15.7% up to 9 months, and 30.6% up to 12 months of age), while 79.6% routinely request it in clinical practice. Of the participants, 96.3% consider genetic testing necessary to identify children who can be treated with SU, and 26.9% would try SU before testing/obtaining results. Only 37% received specific training on NDM, while 44.5% felt less confident in managing patients with NDM. Incidence in the country of practice, participant's age, years of experience in the field, number of patients registered in the clinic, and number of patients with NDM followed up were associated with differences in answers. Conclusions: This survey offers the possibility of informing health providers about the awareness of different aspects of NDM management. Our results provide the opportunity to compare various aspects of diagnosis and treatment of NDM in different geographic areas. Continuous education is needed to boost physicians' confidence in managing patients with this rare form of diabetes

    Tennis elbow in aviators

    No full text
Tennis elbow, or lateral epicondylitis, is described in three aviators. In two of the three aviators, the tennis elbow was felt to be caused by duties involving flight operations and resulted in temporary suspension from flight operations. Relief of the symptoms was obtained with conservative therapy. The symptomatology, predisposing factors, physical findings, treatment, and prevention of tennis elbow are discussed.Journal Articl

The clinical approach to child and adolescent patients with lipodystrophy: a series of international case discussions

    No full text
Introduction: Lipodystrophy syndromes comprise a group of rare endocrine disorders characterized by the generalized or partial loss of adipose tissue. Affected individuals frequently display absolute or relative reductions in leptin, a key adipokine regulator of hunger-satiety signaling, and are predisposed to a range of metabolic and end-organ complications, often from a young age. The presentation and severity of lipodystrophy syndromes is largely dependent on the extent of adipose tissue loss while comorbidities often deteriorate with age. In this regard, optimizing care for children and adolescents with lipodystrophy syndromes is a pivotal step in supporting them into adulthood. To assist clinicians with limited experience of managing young patients with lipodystrophy syndromes, we describe our clinical approach to a series of pediatric patients with these rare diseases. Methods: The clinical history, diagnosis, disease management and follow-up care of 10 international pediatric patients with lipodystrophy syndromes are presented. Teaching points from each case study are also provided. Most of these cases are based on patients from our clinics with certain details changed to protect privacy. Others represent hypothetical scenarios based on our clinical experience supported by review of the medical literature and are included here for educational purposes. Results: Our patients illustrate the broad phenotypic spectrum of lipodystrophy syndromes that can manifest early in life. We highlight the importance of timely and accurate diagnosis in guiding early disease management strategies to help reduce the risk of comorbidities. The challenges faced by clinicians managing pediatric patients with lipodystrophy syndromes and how these challenges may differ from adult patients are also explored. Discussion: The cases presented in this manuscript may assist clinical teams to promptly diagnose and holistically manage young patients with lipodystrophy syndromes and help optimize clinical outcomes as they transition to adult care

    On necrocapitalism: A plague journal

M.I. Asma is the collective designation for six authors from Canada and the United States, representing a variety of revolutionary anticapitalist theoretical persuasions: J. Moufawad-Paul, Devin Zane Shaw, Mateo Andante, Johannah May Black, Alyson Escalante, and D. W. Fairlane. As the pandemic transitioned from science fiction to reality in early 2020, a number of writers and thinkers in the imperialist metropoles declared the impossibility of writing in the face of a future that is foreclosed. And yet, due to the nightmare that capitalism has been since its beginning, numerous writers and thinkers from the margins have always written in the face of such foreclosure. Meanwhile, other contemporary thinkers sought to conceptualize the unfolding pandemic according to conceptions of bio/necropolitics, forgetting the foundation upon which these conceptions have always existed. The M.I. Asma writing group came together to stake out a different terrain, thinking through the pandemic as events unfolded while also always working to think beyond the capitalist imaginary. Writing between April 2020 and May 2021, the authors set out to produce a serial theoretical philosophical project focused on class struggle in the midst of the COVID-19 pandemic. The authors approached the pandemic as an occasion to think capitalism according to what it always has been, what the pandemic reveals about its current ideological deployment, and how we can think about a communist alternative in the face of exterminism. This book collects, with some revisions and with a new epilogue, the entries from the On Necrocapitalism blog, where M.I. Asma’s interventions first appeared.DC Author's celebration 202
