    A numerical micro-mechanical study on damage induced by the curing process in carbon/epoxy unidirectional material

    A numerical framework for the determination of the damage, the stress and the inelastic strain field induced by the curing process for unidirectional carbon/epoxy composite material is proposed. The approach integrates a network model for the estimation of the matrix shrinkage and elastic properties evolution during curing with damageable elasto-plastic constitutive equations. Simulations of the curing process and of the cured material transverse mechanical response are carried out. A sensitivity study is realised for different volume fractions and matrix tensile strengths for single fiber models. Analyses of Representative Volume Elements (RVEs) with randomly distributed fibers are also performed and a comparison with the ideally cured version of the same model is used to highlight the importance of considering the curing process effects in micromechanical models. The proposed framework, with a proper calibration of the constituents, contributes to the enhancement of the fidelity of numerical micromechanics for this class of materials

    Intravitreal Bevacizumab versus Low Fluence Photodynamic Therapy for the treatmentof Chronic Central Serous Chorioretinopathy

    Abstract PURPOSE: To report the effectiveness and safety of intravitreal bevacizumab injection (IVB) compared with low-fluence photodynamic therapy (L-PDT) in eyes with chronic central serous chorioretinopathy (CSC). METHODS: This was a prospective comparative interventional study of 22 patients affected by chronic CSC. Patients were randomly assigned to group 1 (12 patients treated with 1.25 mg IVB) and group 2 (10 patients treated with L-PDT). In group 2, PDT with verteporfin was delivered at low fluence (300 mW/cm(2) for 83 s, 25 J/cm(2)). Follow-up visits were scheduled at 1, 3, 6, and 9 months. RESULTS: The improvement in visual acuity was greater in group 1 than in group 2, although the difference was not statistically significant (P = 0.59). The mean change in central point thickness over 9 months from baseline was 127 μm (SD 36) in group 1 and 114 μm (SD 42) in group 2. After the first injection, retreatments were given if recurrence was noted, with an average of 3.0 ± 1 injections in group 1 and 1.6 ± 0.6 treatments with L-PDT in group 2. The difference between retreatment in the 2 groups was not statistically significant (P = 0.45). CONCLUSIONS: Based on the results obtained after 9 months of follow-up, our study provides evidence that IVB may be a treatment option for chronic CSC

    Diagnosis, management and outcome of clinically-suspected spinal infection

    Objectives: Spontaneous spinal infection (SI) is a quite rare but serious entity. This study aimed to evaluate outcome and follow-up data of SI cases without a microbiological diagnosis (suspected SI). Methods: We undertook a retrospective, comparative study of 82 spontaneous SI cases in adults presenting over an 11-year period to two Italian hospitals. Results: The diagnostic yields of blood culture, percutaneous needle biopsy of spine, and surgical sample culture were 43.6%, 72.7%, and 91.6%, respectively. Overall, causative organisms were identified in 60 (73.2%) cases, the most frequently isolated pathogens being Staphylococcus aureus and Mycobacterium tuberculosis. The median diagnostic delay was similar (pZ0.39) in pyogenic (1 month) and suspected (0.5 month) SI cases, and longer in tuberculous cases (4 months) than in the other SI case groups (pZ0.069 and pZ0.062, respectively). All patients received antibiotic treatment, and 21 (25.5%) underwent surgery, that was required more frequently in tuberculous (40.7%) than in pyogenic (25.0%) and suspected SI cases (9.1%) (pZ0.028). Of 67 patients who completed a 1-year follow-up period, 24 had persisting painful disability that was more frequent in tuberculous (66.7%) cases than in pyogenic (21.7%) and suspected SI (15.0%) cases (pZ0.03). Conclusions: Although a microbiological diagnosis was not achieved in nearly a quarter of SI cases, both diagnostic delay and outcome were similar to those of pyogenic SI cases.
