Assessment tools for counselling in long-term care at home

Ein geeignetes Instrument zur Unterstützung einer systematischen Situationsanalyse und zur Erfassung des Beratungsbedarfes kann die Qualität der Beratung verbessern und auf ein einheitliches Niveau heben. Auf Basis einer systematischen Literaturrecherche werden in diesem Beitrag acht Assessmentinstrumente vorgestellt, die für die Beratungsangebote nach SGB XI in Frage kommen. Dabei zeigt sich Entwicklungsbedarf für ein Instrument, welches die Besonderheiten des intrapersonalen Beratungsprozesses berücksichtigt und unabhängig vom gesetzlich definierten Beratungsanlass zum Einsatz kommen kann.Counselling services contribute to strengthening and stabilising home care arrangements. Using an appropriate assessment instrument to support a systematic analysis of the care situation and the counselling needs may lead to improved and consistent quality of counselling services. Based on a systematic literature search, this paper presents eight assessment tools that can be used for counselling services within the regulatory framework of the German long-term care insurance system. It shows a need for the development of an assessment tool that takes into account the special features of the intra-personal counselling process
and can be used independently of the legally defined reason for counselling
Counselling in home care : Assessment for prevention and intervention (PIP assessment)
No full textBeratung kann zur Prävention bei pflegebedürftigen Menschen und ihren Angehörigen sowie zur Stabilität häuslicher Pflegesituationen beitragen. Im Projekt „Präventive Beratung und Intervention in der häuslichen Pflege“ wurde ein Instrument entwickelt, das die Einschätzung von gesundheitlichen Risiken und Ressourcen in der häuslichen Versorgung sowie die Identifizierung geeigneter Maßnahmen ermöglicht. Die theoretische Grundlage zur Ausarbeitung relevanter inhaltlicher Dimensionen bildete die Analyse verfügbarer Assessments und Dokumente. Die Konzeption des PIP-Assessments wurde von Beraterinnen und Beratern in Fokusgruppensitzungen begleitet. Das Assessment wurde in der Beratungspraxis erprobt und ist zur Anwendung frei zugänglich.Counselling can contribute to prevention as well as to the stability of home care arrangements for people in need of care and their caregivers. The project “Preventive Counseling and Intervention in Home Care” aimed at the development of an instrumentthat enables the assessment of health risks and resources in home care as well as the identification of suitable measures. The analysis of available assessment instruments and documents formed the theoretical basis for the development of relevant content dimensions. The design of the assessment was accompanied by focus group discussions with experts from the counselling field. The PIP assessment was piloted in practice settings and revised on the basis of the evaluation and is freely available for use
