Editorial: The complexity of primary antibody deficiencies
Primary antibody deficiencies represent by far the largest group of primary immunodeficiencies (PID) at 56% (ESID Registry), however the proportion of patients in whom antibody deficiency represents a component of their condition is greater still at around 75% (1). It has become clear over recent years that while the vast majority of such patients experience recurrent infections a significant proportion are also affected by non-infectious dysregulatory complications which include malignancy, autoimmunity, inflammation and allergy (Figure 1). The increasingly complex manifestations of Primary Antibody Deficiencies have a major impact on the clinical management of patients and raise diagnostic and therapeutic challenges (2). This Research Topic draws together a series of reports focusing on a range of these non-infectious complications and their clinical implications
Clinical and Genetic Characterization of Brazilian Patients with Chronic Granulomatous Disease and Mendelian Susceptibility to Mycobacterial Disease
Dentre pacientes com imunodeficiências primárias, existem aqueles com defeitos de fagócitos e outros componentes da imunidade inata. A doença granulomatosa crônica (DGC) é uma imunodeficiência primária (IDP) causada por mutações em um dos componentes protéicos, gp91-phox, p22-phox, p47-phox, p67-phox e p40-phox, da nicotinamida adenina dinucleotídeo fosfato (NADPH) dos fagócitos. Pacientes com DGC apresentam maior susceptibilidade a infecções, assim como hiperinflamação e reação adversa à vacinas como à do Bacilo Calmette-Guérin (BCG), como consequência da atividade microbicida defeituosa dos fagócitos. Por outro lado, a susceptibilidade mendeliana a micobactérias (MSMD) é uma condição que predispõe os pacientes a infecções pelo gênero Mycobacterium sp, levando a infecções graves e por vezes à morte. O objetivo deste trabalho foi realizar o diagnóstico clínico e a análise genético-molecular de pacientes brasileiros com DGC e MSMD. A explosão respiratória de granulócitos foi avaliada pelo ensaio de dihidrorodamina (DHR). A dosagem de citocinas do eixo IL-12/IFN-γ foi realizada mediante o ensaio de ELISA após estimulo com lisado de micobactérias (LM), proteína purifica (PPD) e BCG. O DNA genômico dos pacientes foi extraído, amplificado e sequenciado pelo método de Sanger e seqüenciamento completo de exoma. Durante o período de 2014-2018, 181 pacientes com histórico clínico sugestivo de DGC e 75 pacientes com diagnóstico sugestivo de MSMD foram encaminhados ao nosso laboratório. Após avaliação clínica e bioquímica dos pacientes, 23 deles foram diagnosticadas com DGC e 16 com MSMD. A análise genético-molecular permitiu identificar mutações em 14 pacientes com DGC, nove deles com DGC ligada ao cromossomo X (DGC-X) e 5 com DGC autossômica recessiva (DGC-AR). Identificamos mutações em 5 pacientes com MSMD, sendo três delas no receptor de IL-12 e duas no receptor da IL-17.Among patients with primary immunodeficiencies, there are those with defects in phagocytes and other components of the innate immunity. Chronic granulomatous disease (CGD) is a primary immunodeficiency (PID) caused by mutations in one of the protein components, gp91-phox, p22-phox, p47-phox, p67-phox and p40-phox of the Nicotinamide Adenine Dinucleotide Phosphate (NADPH) oxidase of phagocytes. The main goal of this study was to perform the clinical diagnosis and genetic-molecular analysis of Brazilian patients with CDG and MSMD. The respiratory burst of granulocytes was evaluated by the dihydrorhodamine (DHR) assay. Cytokine dosing of IL-12 / IFN-γ axis was performed by the ELISA assay after stimulation with mycobacterium lysate (LM), purified protein (PPD) and BCG. Patients genomic DNA was extracted, amplified and sequenced by the Sanger method and whole exome sequencing. During the period of 2014 to 2018, 181 patients with a clinical history suggestive of CDG and 75 patients with a diagnosis suggestive of MSMD were referred to our laboratory. After clinical and biochemical evaluation, 23 of them were diagnosed with CDG and 16 with MSMD. Genetic-molecular analysis allowed the identification of mutations in 14 patients with CDG, of those 9 had X-linked DGC (X-CGD) and 5 had autosomal recessive CGD (AR-CGD). Effect of interferon-gamma on dendritic cells of patients with CD40L defects.
Mutações no gene CD40LG estão associadas à Síndrome de Hiper-IgM, relacionada ao cromossomo X, que é uma Imunodeficiência Primária. A ausência da interação CD40L/CD40 (linfócitos-DCs) acarreta um aumento da suscetibilidade às infecções fúngicas. Os efeitos do tratamento in vitro com interferon-gama em células dendríticas de pacientes com mutações no CD40L demonstraram potencial sobre a imunologia das DCs e nos remete a fundamentar possíveis ensaios clínicos futuros, que tenham por tema a prevenção e o tratamento de infecções oportunistas.Mutations in the CD40LG gene are associated with the X-linked Hyper-IgM Syndrome, which is a Primary Immunodeficiency. The absence of the CD40L / CD40 interaction (lymphocytes-DCs) leads to increased susceptibility to fungal infections. The effects of in vitro gamma-interferon treatment on dendritic cells from patients with CD40L mutations have demonstrated potential for DC immunology and suggest that future clinical trials should address the prevention and treatment of opportunistic infections
Effect of BAY 41-2272 in human T lymphocytes.
No presente trabalho avaliamos o potencial do BAY 41-2272 e sua via, como uma ferramenta para modulação da função dos linfócitos. Para isso, realizamos tratamentos farmacológicos com BAY 41-2272, avaliando a produção de citocinas e observamos que este fármaco, como ativador direto, não induz produção de IFN, IL-4 e IL-10 nos linfócitos. No entanto, o prétratamento por 24 horas com BAY 41-2272, com posterior ativação com PMA, mostrou que esta droga tem efeito inibitório na produção das citocinas. Em vista disto, avaliamos se este fármaco seria capaz de ativar estas células através da expressão de CD69. Vimos que por si só esta droga não foi capaz de aumentar a expressão de CD69, no entanto o pré-tratamento com BAY 41-2272 inibiu a ativação dos linfócitos T CD4. Assim, avaliamos se o fármaco seria capaz de inibir a expressão de fatores de transcrição FOXP3, RORT, Tbet e GATA3. Vimos que o BAY 41-2272 não induziu expressão desses fatores de transcrição e o pré-tratamento com este fármaco não alterou a expressão de FOXP3, RORT e GATA3, mas inibiu a expressão de Tbet quando comparado ao estimulado com PMA e Ionomicina sem o prétratamento. Observamos também que o pré-tratamento com BAY 41-2272 inibiu a linfoproliferação. Estes resultados sugerem que o BAY 41-2272 e sua via, têm um perfil inibitório sobre os linfócitos T CD4, e potencialmente podem ser utilizados como imunomodulador em pacientes com comprometimento do sistema imunológico e síndromes linfoproliferativas.In this work, we evaluated the potential of BAY 41-2272 and its pathway as a tool for modulating lymphocyte function. For this, we performed pharmacological treatments with BAY 41-2272, evaluating the production of cytokines and observed that this drug, as a direct activator, does not induce production of IFN, IL-4 e IL-10. However, pre-treatment for 24 hours with BAY 41-2272 and subsequente activation with PMA showed that this drug has an inhibitory effect on cytokine production. Thus, we evaluated if this drug would be able to activate these cells through the CD69 expression. We saw that alone, BAY 41-2272 was not able to increase CD69 expression, however, pre-treatment inhibited activation of CD4 T lymphocytes. Then, we evaluated if this chemical compound would be able to inhibite the expression of transcription factors FOXP3, RORT, Tbet and GATA3. We have seen that BAY 41-2272 did not induce expression of these transcription factors and pretreatment with this drug did not alter expression of FOXP3, RORT and GATA3, but inhibited Tbet expression. We also observed that pre-treatment with BAY 41-2272 inhibited lymphoproliferation. These results suggest that BAY 41-2272 and its pathway have an inhibitory profile on CD4 T lymphocytes and can potentially be used as an immunomodulator in patients with impaired imune system and lymphoproliferative syndromes
Interaction among specific antibodies and dendritic cells from allergic patients.
A atopia caracteriza-se pela tendência de um indivíduo a produzir IgE em quantidade elevada, em resposta a um alérgeno específico, levando ao desenvolvimento de asma, rinite ou eczema. Todavia, a manifestação do fenótipo da alergia depende da interação de fatores genéticos e exposição a alérgenos ambientais. Desta forma, o alérgeno é processado e apresentado aos linfócitos T, que desenvolvem uma resposta imune Th2, exacerbada característica da atopia. A principal célula que está envolvida na comunicação entre a imunidade inata e adaptativa é a célula dendrítica (DC) cuja função é capturar, processar e apresentar o antígeno aos linfócitos. As DCs imaturas capturam o antígeno e migram do tecido para o órgão linfóide periférico, onde elas se diferenciam em DCs maduras e apresentam o antígeno aos linfócitos T naive. Assim, os linfócitos T naive podem se diferenciar em subtipos de linfócitos efetores como: linfócitos Th1 e Th2. Esses linfócitos auxiliam na produção de anticorpos pelos linfócitos B na resposta imune humoral contra patógenos específicos. O sistema imune humoral compreende cinco classes de imunoglobulinas: IgG, IgM, IgD, IgE e IgA e sua produção sofre influência da imunidade celular. Desta forma, alérgenos provenientes de ácaros como, Dermatophagoides pteronyssinus, podem levar à inflamação alérgica, alterando a produção de anticorpos. Tendo em vista evidências que demonstram a interação das DCs com anticorpos, propomos investigar sua influência na apresentação dos principais alérgenos da poeira domiciliar e sua modulação sobre a resposta imune em indivíduos alérgicos e não alérgicos.Atopy is characterized by the trend of an individual to produce high amounts of IgE in response to a specific allergen, leading to the development of asthma, rhinitis or eczema. However, the manifestation of the allergy phenotype depends on the interaction of genetic factors and exposure to environmental allergens. In this way, the allergen is processed and presented to the T lymphocytes, which develop a Th2 immune response, exacerbated characteristic of atopy. The main cell that is involved in the communication between innate and adaptive immunity is the dendritic cell (DC) whose function is to capture, process and present the antigen to lymphocytes. Immature DCs capture the antigen and migrate from the tissue to the peripheral lymphoid organ, where they differentiate into mature DCs and present the antigen to naive T lymphocytes. Thus, naive T lymphocytes can differentiate into subtypes of effector lymphocytes such as Th1 and Th2 lymphocytes. These lymphocytes assist in the production of antibodies by B lymphocytes in the humoral immune response against specific pathogens. The humoral immune system comprises five classes of immunoglobulins: IgG, IgM, IgD, IgE and IgA and their production is influenced by cellular immunity. In this way, allergens from mites such as Dermatophagoides pteronyssinus, can lead to allergic inflammation, altering the production of antibodies. In view of evidence demonstrating the interaction of DCs with antibodies, we propose to investigate their influence on the presentation of the main house dust allergens and their modulation on the immune response in allergic and non-allergic individuals
Epidemiological study on Chronic Granulomatous Disease (CGD): Clinical and demographic characteristics
A Doença Granulomatosa Crônica (DGC) é uma imunodeficiência primária caracterizada pela deficiência da produção de espécies reativas de oxigênio (EROs), devido a variantes genéticas deletérias em um dos 5 genes (CYBB, CYBA, NCF1, NCF2 e NCF4) que codificam as proteínas do complexo NADPH oxidase, resultando em atividade microbicida defeituosa e suscetibilidade a infecções. Apresenta as formas ligada ao X (CYBB) e autossômica recessiva, ambas com início das manifestações ainda nos 2 primeiros anos de vida. Estudos epidemiológicos mais abrangentes sobre a DGC no Brasil são raros e a caracterização desses pacientes dentro de um bloco continental com características físicas e geopolíticas semelhantes é de especial importância para seu entendimento e divulgação. Assim sendo, este estudo teve como objetivo determinar as características epidemiológicas, clínicas, geoespaciais e genético-moleculares de 238 pacientes com DGC, 196 do sexo masculino e 42 do sexo feminino de 8 países (México, Brasil, Chile, Costa Rica, Argentina, Paraguai, Peru e Uruguai), diagnosticados pelos ensaios de redução de nitroazul tetrazólio (NBT) ou oxidação de diidrorodamina 123 (DHR). Os pacientes foram diagnosticados genética e laboratorialmente e foram tratados e acompanhados em poucos centros especializados concentrados em regiões mais desenvolvidas e populosas dos respectivos países de origem. A variante genética foi obtida de 141 pacientes (59%), 109 com DGC-LX e 32 com DGC-AR, com mutações diversas e heterogenias nos genes CYBB, CYBA e NCF2, e única no gene NCF1, a mutação GT (c.75_76delGT). As principais manifestações clínicas foram Pneumonia (80%), linfadenopatia (63%), infecções cutâneas (55,5%), gastroenteropatia (52%), sepse (49,6%) e reação ao BCG (Bacillus Calmette-Guérin) (39,5%). Bactérias e fungos foram os patógenos mais comuns, como Staphylococcus aureus , Aspergillus spp., e micobactérias, especialmente pela reação adversa à vacina BCG, que afetou 45,2% dos pacientes vacinados com idade mediana de 5 meses, estando entre as manifestações mais precoces da doença. Profilaxia antimicrobiana com Sulfametoxazol em associação com Trimetoprima (SMX-TMP) e Itraconazol foi adotada por 97% dos pacientes, enquanto que o IFN-γ foi usado quase que exclusivamente por pacientes do México, principalmente nos casos de DGC clássico (DGC-LX). O transplante de células tronco hematopoiética (TCTH), em comparação com a profilaxia, não mostrou melhores resultados quanto a sobrevida e o Brasil, não usa o genótipo como critério para realização do transplante e o realiza em pacientes já com sequelas das infecções. Oitenta pacientes faleceram, predominantemente do sexo masculino (86%) e com DGC-LX (n = 43). A principal causa de óbito foi infecciosa, sendo PNM e sepse as mais frequentes (70% dos óbitos) e o Aspergillus spp , o patógeno mais letal (40% dos casos com patógeno isolado), com os pacientes com DGC-LX apresentando pior prognóstico e sobrevida. Estudos epidemiológicos são fundamentais para identificar as principais características da doença e a realidade desses pacientes, permitindo mostrar aqui que o diagnóstico genético precisa ser ampliado, que o uso do IFN-γ deve ser estimulado e que o momento da realização do TCTH precisa ser reavaliado para obter melhores resultados.Chronic Granulomatous Disease (CGD) is a primary immunodeficiency characterized by a deficiency in the production of reactive oxygen species (ROS) due to deleterious genetic variants in one of the 5 genes (CYBB, CYBA, NCF1, NCF2 and NCF4) that encode the proteins of the NADPH oxidase complex, resulting in defective microbicidal activity and susceptibility to infections. It presents the X-linked (CYBB) and autosomal recessive forms, both with onset of manifestations in the first 2 years of life. Larger-scale epidemiological studies on CGD in Brazil are rare and the characterization of these patients within a continental block with similar physical and geopolitical characteristics is of special importance for their understanding and dissemination. Therefore, this study aimed to determine the epidemiological, clinical, geospatial, molecular and genetics characteristics of 238 patients with CGD, 196 males and 42 females from 8 countries (Mexico, Brazil, Chile, Costa Rica, Argentina, Paraguay, Peru and Uruguay), diagnosed by the nitroblue tetrazolium (NBT) reduction or dihydrorhodamine 123 (DHR) oxidation assays. Patients were genetically and laboratory-diagnosed and were treated and followed up in a few specialized centers concentrated in more developed and populous regions in their countries of origin. The genetic variant was obtained from 141 patients (59%), 109 with XL-CGD and 32 with AR-CGD, with diverse and heterogeneous mutations in the CYBB, CYBA and NCF2 genes, and the only one in the NCF1 gene, the GT mutation (c. 75_76delGT). The main clinical manifestations were pneumonia (80%), lymphadenopathy (63%), skin infections (55,5%), gastroenteropathy (52%), sepsis (49,6%) and adverse reaction to BCG (Bacillus Calmette-Guérin) (39,5%). Bacteria and fungi were the most common pathogens, such as Staphylococcus aureus , Aspergillus spp , and mycobacteria, especially due to the adverse reaction to the BCG vaccine, which affected 45,2% of vaccinated patients with a median age of 5 months, being among the first manifestations of the disease. Antimicrobial prophylaxis with Sulfamethoxazole in association with Trimethoprim (SMX-TMP) and Itraconazole was adopted by 97% of patients, while IFN-γ was used almost exclusively by patients from Mexico, mainly in cases of classic CGD (XL-CGD). Hematopoietic stem cell transplantation (HSCT), compared to prophylaxis, did not show better results in terms of survival and Brazil does not use the genotype as a criterion for performing the transplant and performs it in patients already with sequelae of infections. Eighty patients died, predominantly male (86%) and with XL-CGD (n = 43). The main cause of death was infectious, with PNM and sepsis being the most frequent (70% of deaths) and Aspergillus spp. the most lethal pathogen (40% of cases with isolated pathogen), with patients with CGD-LX having a worse prognosis and survival. Epidemiological studies are essential to identify the main characteristics of the disease and the reality of these patients, allowing us to show that the genetic diagnosis needs to be expanded, that the use of IFN-γ should be encouraged and that the timing of HSCT needs to be reassessed to get better results
Efeito do BAY 41-2272, estimulador de guanilato ciclase solúvel, em neutrófilos humanos
Os neutrófilos estão entre as principais células da imunidade inata e são as primeiras células a migrarem para o sítio de infecção. O BAY 41-2272, estimulador de guanilato ciclase solúvel, é capaz de ativar fagócitos mononucleares e em neutrófilos diminuir migração in vivo e in vitro. O presente trabalho teve como objetivo avaliar o potencial do BAY 41-2272, e sua via, como uma ferramenta farmacológica para modulação da função dos neutrófilos. Para isso foi realizado o tratamento in vitro dos PMNs com o BAY 41-2272. A viabilidade celular, quimiotaxia e funções efetoras como burst oxidativo e produção de citocinas foram avaliadas, observando-se que o BAY 41-2272, como ativador direto, não altera estas funções. No entanto, o pré-tratamento com BAY 41-2272 nas doses de 3 M e 30M por uma hora, com subsequente ativação com PMA, mostrou perfil inibitório na quimiotaxia, produção da citocina IL-8 e no burst oxidativo . Foram avaliados também a expressão de moléculas como CD15, CD31, CD35, CD49d, CD63, CD66b, CD162 e a produção de GMPc. As moléculas de superfícies não mostraram alterações após tratamento direto com BAY 41-2272. A produção de GMPc foi induzida na dose de 30 M de BAY 41-2272 e pelo SNAP (doador de NO). Esses dados sugerem um potencial inibitório do BAY 41-2272 sobre a ação dos neutrófilos, e uma possível alternativa a ser explorada em seus aspectos translacionais na busca por novas terapias destinadas ao controle de doenças ligadas a inflamações crônicas e doenças auto-imunes.Neutrophils are the major cells of innate immunity and are the first cells to migrate to the site of infection. BAY 41-2272, a soluble guanylate cyclase stimulator, is capable of activating mononuclear phagocytes and in neutrophils decreasing migration in vivo and in vitro. The present work aimed to evaluate the potential of BAY 41-2272, and its pathway, as a pharmacological tool for modulating neutrophil function. For this, the in vitro treatment of PMNs with BAY 41-2272 was performed. Cell viability, chemotaxis and effector functions such as oxidative burst and cytokine production were evaluated, observing that BAY 41-2272, as a direct activator, does not alter these functions. However, pretreatment with BAY 41-2272 at the doses of 3 M and 30 M for one hour, with subsequent activation with PMA, showed an inhibitory profile in chemotaxis, IL-8 cytokine production and in the \"oxidative burst\". We also evaluated the expression of molecules such as CD15, CD31, CD35, CD49d, CD63, CD66b, CD162 and cGMP production. Surface molecules did not show changes after direct treatment with BAY 41-2272. The cGMP production was induced at the dose of 30 M BAY 41-2272 and for SNAP (NO donor). These data suggest an inhibitory potential of BAY 41-2272 on the action of neutrophils, and a possible alternative to be explored in its translational aspects in the search for new therapies aimed at the control of diseases linked to chronic inflammation and autoimmune diseases
Study of new molecular genetic defects in patients with clinical diagnosis of primary immunodeficiency.
As imunodeficiências primárias são um grupo heterogêneo de doenças hereditárias do sistema. Aqui nós descrevemos 4 famílias (2 Turcas e 2 Brasileiras), que apresentaram infecções recorrentes desde os primeiros dias de vida. Após uma análise clínica bem detalhada, combinamos as técnicas de sequenciamento de alta geração para identificar novos defeitos genéticos que levam ao fenótipo de IDP. Finalizamos com a identificação e caracterização de três IDP, sendo que duas inéditas. A primeira identificada (P1) foi causada por uma mutação bialélica no sítio de splice do gene PRKCD (c.1352+1G>A). A segunda (P2 e P3) foi causada por uma mutação bialélica no gene que codifica NIK (c. C1694G; p. Pro565Arg). A terceira (P4) foi causada uma mutação no gene IL7Rα (c.G353A). Finalizamos a análise da P5, mas nenhum dos genes candidatos foi confirmado. A análise genética e a identificação do defeito genético, permite que nossos pacientes possam ter uma melhor sobrevida, podendo realizar um tratamento correto e permite o aconselhamento genético na família.Primary immunodeficiencies are a heterogeneous group of inherited diseases of the immune system. Here we describe 5 patients from 4 families (2 Turks and 2 Brazilian), all patients had recurrent infections since the firsts days of life. After a very detailed clinical analysis, we applied the Next Generation Sequencing to identify new genes that could be lead to PID phenotype. We finished with the identification and characterization of 3 PID, where 2 of them was new. The first identified (P1) was a biallelic mutation in the splice site of the gene PRKCD (c.1352 + 1G>A). The second (P2 and P3) was a biallelic mutation in the gene encoding NIK (MAP3K14; c.C1694G;. p.Pro565Arg). The third (P4) has a mutation in the gene IL7Rα (c.G353A). We finished the analysis of P5, but no candidate gene was confirmed to be the defect cause. Genetic analysis and identification of the genetic defect allows our patients may have a better survival and can perform a proper treatment and genetic counseling allows the family
Chronic granulomatous disease: clinical, genetic and immunological characterization of patients with susceptibility to infections and hyperinflammation
A Doença Granulomatosa Crônica é caracterizada pela falha da explosão respiratória em fagócitos devido a perda de função da NADPH oxidase, gerando quadros de susceptibilidade a infecções e hiperinflamação, cujos mecanismos permanecem pouco esclarecidos. Nosso trabalho analisou uma coorte de 11 pacientes DGC com quadro clínico compatível com DGC (como abcessos, pneumonias, granulomas, infecções bacterianas e fúngicas). Foram identificadas variantes patogênicas em 5 pacientes, sendo 4 no gene CYBB e 1 no NCF1. A partir dessa coorte, investigamos respostas realizadas por células dendríticas derivadas de monócitos (mo-DC) e a polarização de linfócitos T de pacientes DGC em comparação com indivíduos saudáveis. Mo-DCs cultivadas com LPS, Candida albicans e BCG apresentaram elevada expressão do marcador CD1a, redução da expressão de CD40 e perfis de citocinas favorecendo a polarização de linfócitos Th17 (elevada produção de IL-23 e IL-6) e desfavorecendo resposta Th1 (baixa produção de IL-12p70). Linfócitos T efetores cocultuvados com mo-DCs maduras autólogas de pacientes DGC apresentaram capacidade normal de diferenciação de células T naïve em células T efetoras Th1 e Th17, porém há uma hiperexpressão do eixo Th17, evidenciado pelo aumento da expressão de IL-23 pelas mo-DCs, aumento da população de linfócitos Th17 e a produção exacerbada de IL-17A e IL-6 por linfócitos. A hiperativação do eixo Th17 tem sido relatado em uma gama de doenças inflamatórias, como doença inflamatória intestinal, psoríase, artrite reumatoide e esclerose múltipla. O estudo apontou possíveis novos mecanismos relacionados a hiperinflamação e a susceptibilidade a infecções bacterianas e fúngicas em DGC.Chronic Granulomatous Disease is characterized by failure of the respiratory burst in phagocytes due to loss of NADPH oxidase function, generating susceptibility to infections and hyper-inflammation whose mechanisms remain unclear. Our study analyzeda cohort of 11 CGD patients with a clinical picture of CGD (such as abscesses, pneumonia, granulomas, bacterial and fungal infections). Pathogenic variants were identified in 5 patients, 4 in the CYBB gene and 1 in the NCF1. From this cohort, we investigated responses performed by monocyte-derived dendritic cells (mo-DC) and the polarization of T lymphocytes from CGD patients compared to healthy subjects. Mo-DCs cultured with LPS, Candida albicans and BCG showed high expression of the CD1a marker, reduced expression of CD40 and cytokine profiles favoring the polarization of Th17 lymphocytes (elevated production of IL-23 and IL-6) and disfavoring the Th1 response (low production of IL-12p70). Effector T lymphocytes co-cultured with mature autologous mo-DCs from CGD patients showed normal ability to differentiate from naive T cells into Th1 and Th17 effector T cells, but there is a hyperexpression of the Th17 axis, evidenced by the increased expression of IL-23 by the DCs, an increase in the Th17 lymphocyte population and the exacerbated production of IL-17A and IL-6 by lymphocytes. Hyperactivation of the Th17 axis has been reported in a range of inflammatory diseases such as inflammatory bowel disease, psoriasis, rheumatoid arthritis and multiple sclerosis. Genetic and molecular investigation of patients with Severe Combined Immunodeficiency,
A imunodeficiência combinada grave (SCID) é uma doença caracterizada por profunda deficiência de células T, que afeta as imunidades celular e humoral e gera anormalidades graves no desenvolvimento e funções do sistema imune. Recém-nascidos com SCID apresentam a doença nos primeiros meses de vida e tem grande susceptibilidade a infecções. Sem tratamento, essas condições são invariavelmente fatais, porém se reconhecidas precocemente, há a possibilidade da realização do transplante de células-tronco hematopoiéticas, o tratamento curativo, o que torna a SCID uma emergência pediátrica. A investigação do defeito genético é uma prerrogativa para o condicionamento adequado do transplante, a terapia gênica, o aconselhamento genético e o diagnóstico pré-natal. No Brasil, o conhecimento sobre SCID é incipiente e não existem dados moleculares sobre pacientes com a doença. Sendo assim, este estudo teve por objetivo investigar defeitos genético-moleculares de pacientes brasileiros com SCID. Foram incluídos 13 pacientes, todos com início precoce dos sintomas e manifestações clínicas esperadas em SCID (principalmente infecções respiratórias, de pele, diarreia crônica e atraso de crescimento). Os patógenos isolados foram vírus, bactérias e fungos oportunistas comumente encontrados em pacientes SCID. A partir da quantificação de TRECS e KRECs e imunofenotipagem de linfócitos, foi montado o perfil imunológico de cada paciente, que guiou o sequenciamento direto de Sangerdos genes mais frequentemente mutados em cada imunofenótipo de SCID. Mutações em 3 pacientes foram identificadas por Sanger e, posteriormente, 8 pacientes cujas mutações não foram encontradas no Sanger foram encaminhados para o sequenciamento completo de exoma, que resultou na identificação do gene afetado em 62,5% dos casos. Ao todo, foram identificadas mutações patogênicas em 8 dos 13 pacientes. Os resultados revelaram 6 alterações em 5 genes de SCID clássica (IL7R, RAG2, DCLRE1C, JAK3, IL2RG), 1 mutação no gene CD3G e 2 alterações em CECR1. Das 9 mutações encontradas, 5 não possuíam registro na literatura. O estudo genético de SCID em nosso país é problemático, principalmente porque ainda hoje, a esmagadora maioria dos pacientes não é diagnosticada. A implementação da quantificação de TRECs e KRECs como triagem neonatal para linfopenias graves é uma ferramenta fundamental para que os pacientes SCID possam ser identificados, investigados e tratados adequadamente.Primary immunodeficiencies are a heterogeneous group of genetic diseases that lead to increased susceptibility to infections and affect mostly children. Severe Combined Immunodeficiency (SCID) is the most severe of all these diseases and is characterized by profound T cell deficiency, which affects cellular and humoral immunities and leads to severe abnormalities in the development and function of the immune system. Newborns with SCID present the disease in the first months of life and are highly susceptible to infections. Without treatment, these conditions are invariably fatal, but if recognized early, there is the possibility of hematopoietic stem cell transplantation, the curative treatment, which makes SCID a pediatric emergency. Identifying the genetic defect of SCID patients is a prerequisite for proper transplant conditioning, gene therapy, genetic counseling and prenatal diagnosis. Knowledge about SCID is still incipient in Brazil, and there are virtually no molecular data on patients with the disease. Therefore, this study aimed to investigate genetic-molecular defects of Brazilian patients with SCID. Thirteen patients were recruited, all with early onset of symptoms and clinical manifestations expected of classic SCIDs (mainly respiratory and skin infections, chronic diarrhea and failure to thrive). The pathogens isolated were opportunistic viruses, bacteria and fungi often reported in SCID patients. The immunological profile from each patient was defined by the quantification of TRECS and KRECs and lymphocyte immunophenotyping, which was meant to guide direct sequencing by Sanger of the most frequently mutated genes of each SCID immunophenotype. Mutations in 3 patients were identified by Sanger and, subsequently, 8 patients whose mutations were not identified by Sanger were referred for whole exome sequencing, which resulted in the identification of the affected gene in 62,5% of cases. Pathogenic mutations were identified in 8 of the 13 patients. The results revealed 6 mutations in 5 genes associated to classical SCID genes (IL7R, RAG2, DCLRE1C, JAK3, IL2RG), 1 mutation in the CD3G gene, and 2 mutations in CECR1. Five of the 9 mutations found had no record in the literature. SCID genetic investigation in our country is troublesome, mainly because even nowadays, the vast majority of patients are not diagnosed properly. Newborn screening for SCID and other severe lymphopenias by the quantification of TRECs and KRECs is key for the identification, investigation and proper treatment of SCID patients
