    Differences in steep quality and body mass index in asthmatic children and healthy control group

    Introduction: Childhood asthma has become a serious public health problem. Obesity has been determined to be one of the risk factors of asthma. Aim: We aimed to determine the difference in body mass index (BMI) and sleep quality in pediatric asthmatic individuals compared to their peers. Method: Thirty children aged 8-17 years were followed up in the Pediatric Outpatient Clinic for asthma along with 30 healthy children. The BMI percentile values of the children were recorded. The Pittsburgh Sleep Quality Index was used to assess sleep quality. Results: Each group in our study had 10 girls and 20 boys. The mean age was found to be 11.76 +/- 2.69 years in asthma group and 11.33 +/- 2.29 years in the healthy group. The asthma group were found to be more obese than the healthy group (P = 0.033). There was a significant difference between groups interested in a sport (P = 0.028) and steep quality (P = 0.007). Conclusion: It was observed that the asthma group had more obesity and poorer sleep quality than the healthy group. Further, it was determined that in the asthma group, the level of interest in any sport was less than that in the healthy group. We think that high obesity in the asthma group reduces the effect of corticosteroids, and the continuity of nighttime cough symptoms causes deterioration in sleep quality. We conclude that participation in sports activities should be encouraged to reduce the level of obesity in asthmatic children. (C) 2021 Codon Publications. Published by Codon Publications

    A different starting line for allergic march: food protein-induced allergic proctocolitis

    Objective: The aim of this study is to investigate the long-term prognosis of food protein-induced allergic proctocolitis (FPIAP) patients, the risk of developing both allergic and gastrointestinal diseases, and to evaluate whether it leads to allergic march. Methods: A total of 149 children who were diagnosed with FPIAP and developed tolerance at least 5 years prior to the study and 41 children (with no history of food allergy) as a control group were enrolled. Both groups were re-evaluated for allergic diseases as well as gastrointestinal disorders. Results: The mean age of diagnosis for the FPIAP group was 4.2 +/- 3.0 months, while the mean age of tolerance was 13.9 +/- 7.7 months. The mean age of both FPIAP and control groups at the last visit was 101.6 +/- 24.4 and 96.3 +/- 24.1 months, respectively (P = 0.213). At the final evaluation of both groups, the comorbid allergic disease was significantly higher in the FPIAP group (P 18 months, compared to subjects that developed tolerance at =18 months (P < 0.001 and < 0.001, respectively).     Time effect of dupilumab to treat severe uncontrolled asthma in adolescents

    Background: Dupilumab is a new biological drug approved for the treatment of type 2 inflammatory diseases, such as asthma. Dupilumab is a fully humanized monoclonal antibody that acts against both interleukin-4 and interleukin-13 receptors. This study evaluated the timedependent effect of dupilumab on asthma exacerbations and quality of life in adolescents with uncontrolled severe asthma.Materials and Methods: Five adolescents suffering from uncontrolled severe asthma and treated with dupilumab were recruited. All subjects were evaluated for 4, 12, and 24 weeks after the first dose of dupilumab. Outcome measures included lung function, fractional exhaled nitric oxide, asthma control and quality of life assessed by validated questionnaires (Asthma Control Test and Asthma Control Questionnaire).Results: The quality of life improved quickly after 4 weeks of treatment and 80% of adolescents halved the dose of inhaled corticosteroids necessary to control asthma symptoms. These results were still maintained for 24 weeks after start of the therapy. None of the patients had any asthma exacerbation during the study period.Conclusion: Results of this study demonstrated that dupilumab was quickly effective to reduce asthma exacerbation and ameliorate quality of life in severe asthmatic adolescents.(c) 2023 Codon Publications.     Practical protocol of the food allergy committee of the seicap on open oral food challenges to nuts

    Food allergy is rising rapidly among children, and allergy to nuts is one of the most prevalent allergies among them. The category "nuts and seeds" include several plant foods from different botanical families, very different from each other. It is not uncommon to detect co-sensitization to different nuts. However, true co-allergy is less frequent. Up to 80% of patients with positive skin prick tests or specific IgE without true history of reaction who avoid certain nuts, might tolerate them in an Oral Food Challenge (OFC). Although molecular diagnostic techniques help to improve nut allergy diagnosis, OFC still remains the gold standard. For this reason, after reviewing the current bibliography and the recommendations of different allergy societies on standardization of open OFC, the Food Allergy Committee of the Spanish Society of Pediatric Allergy, Asthma and Clinical Immunology (SEICAP) food allergy working group proposed a unified protocol to undertake these OFC, which include preliminary recommendations, unification of total dose, number of doses and interval between doses.     22q11.2 deletion syndrome: 20 years of experience from two pediatric immunology units and review of clues for diagnosis and disease management

    Introduction and objectives: The purpose of this study was to evaluate patients diagnosed with 22q11.2 deletion syndrome and determine the clues directing to diagnosis and evaluation of immunological findings for excellent management of the disease. Material and methods: Thirty-three pediatric patients with 22q11.2 deletion syndrome diagnosed between 1998 and 2019 at Pediatric Immunology Division of Ege University Faculty of Medicine and SBU Izmir Dr Behcet Uz Children's Education and Research Hospital were evaluated. Results: This study includes the largest case series reported from Turkey. Congenital cardiac anomalies were the most common pathology associated with the syndrome (90.9%). Hypocalcemic symptoms were observed in 13 patients (40%). Twenty-two of the 33 (66.6%) patients were diagnosed before two years of age. Autoimmune diseases, dysmorphic facial findings, recurrent infections, growth retardation, and speech impairment were other clues for diagnosis in older patients. Clinical spectrum and immunological abnormalities of this syndrome are quite variable. All T-cell subset counts were less than 5th percentile below median by age in one patient (3%) and 10 patients had normal all T-cell subset counts (30.3%). Overall, 69.6% of the patients had normal IgG, IgA, and IgM levels and two patients had panhypogammaglobulinemia. Recurrent infections were revealed in 75.7% of the patients during follow-up. Conclusions: Presence of cardiac anomaly is more helpful in the diagnosis, especially under two years of age. Patients with immunologically high or standard risk did not show any difference in terms of numbers and severity of infections and autoimmunity. (C) 2021 Codon Publications.     Efficacy of near infrared spectroscopy to segregate raw milk from individual cows between herds for product innovation and traceability

    Cows with specialised characteristics and requirements can be aggregated into different herds for targeted nutritional management and to facilitate on-farm segregation of raw milk for the production of high-value niche dairy products, offering improved economic returns. Rapid methods for independent verification of product quality and origin are desirable to support validation and traceability of such products. This study examined the use of near infrared spectroscopy (NIRS) to segregate raw milk from individual cows of multiple breeds from different herds fed on the same or differing feeding regimes, and to correlate and evaluate the efficacy of the predictions for crude protein and the milk fatty acid (FA) phenotypes for each of the herds. Reference values and near infrared spectra were obtained from representative freeze-dried raw milk samples (n = 220) collected from 847 lactating cows of 3 breeds from the Lincoln University dairy farm in New Zealand. The feed sources (i.e. pasture or pasture with lucerne silage) significantly influenced the protein and the FA values, and these differences were reflected in NIRS analyses. The partial least square regression models for crude protein determination showed excellent results, whereas for the most dominant FA, they were not appreciable. Maximum separation was obtained between the herds on the same feeding regime (mean specificity = 95.2%) using the partial least square discriminant analysis, and its overall performance in differentiating the objects was better than that of the soft independent modelling of class analogy. The multiclass analyses conducted in this study offer improvements to current approaches for evaluating and validating raw milk for the manufacture of specific dairy products, and for enhancing product traceability

    Aeroallergen sensitization in school-age children with allergic rhinitis

    Background: Pandemic period may affect aeroallergen sensitization. Objective: The study aimed to investigate changes in allergen sensitivities of skin prick test (SPT) in patients with allergic rhinitis (AR) during pandemic and to evaluate relationship with disease severity. Methods: In all, 164 AR patients with or without asthma, aged 6-17 years, who have undergone SPTs prior to the pandemic and after October 1, 2021 (18th month of the pandemic), were evaluated retrospectively. The wheal size of allergens in performed SPTs during and prior to the pandemic were compared. Detected changes in allergen sensitivities via SPT results were compared with changes in the disease severity parameters (AR severity, asthma severity, and the number of asthma exacerbations per year), frequency of upper respiratory tract infections and antibiotic use, laboratory parameters, demographic characteristics, and visual analogue scores (VAS). Results: House dust mites (HDMs), cat, pollen, Artemisia, and Cupressus sensitization increased in AR patients during the Coronavirus disease 2019 (COVID-19) pandemic. HDM, mold, and pollen wheal diameters increased in SPTs. Proportion of polysensitization increased during the pandemic, compared to pre-pandemic period (9.1% vs 3%; P < 0.001), and number of non-sensitized patients decreased during the pandemic period compared to the pre-pandemic period (7.9% vs 22.6%; P < 0.001). An increase in HDM sensitivity in SPTs was correlated with VAS for nasal blockage, and an increase in cat sensitivity was correlated with VAS for all nasal symptoms. Conclusion: We believe that inhalant allergen sensitization might have been affected by the lifestyle changes of patients during the pandemic. Hence, it is important to evaluate patients for allergen sensitization, especially patients with moderate/severe AR, to revise disease control measurements. (c) 2023 Codon Publications.     Prevalence of allergic disorders and risk factors associated with food allergy in Turkish preschoolers

    Background: The prevalence of allergic disorders is on the rise, affecting about 10% of the population. In this retrospective cohort, we investigated prevalence of allergic disorders, associated risk factors, and the outcome of food allergies. Material and methods: We analyzed data from birth cohorts of two university hospitals&apos; well-child outpatient clinics. Factors related to onset and type of allergic diseases were assessed from demographic, socioeconomic, and clinical data. Results: Analyses were performed on 949 (431F/518M) infants at a mean current age of 28 +/- 6 months. Any allergic disease was established among 177 cases (22%); atopic dermatitis in 123 (12.8%), respiratory allergies in 55 (5.7%), and food allergy in 41 (4.3%). The risk for allergic disorders was found to be significantly increased for male gender (OR: 2.31, 95% CI; 1.54-3.46), and positive parental atopy (OR: 1.94, 95% CI; 1.31-2.86). The risk of food allergies was significantly higher in the male gender (OR: 2.47, 95% CI; 1.21-5.02), who consumed egg-white between 6 and 12 months (OR: 2.34, 95% CI; 1.22-4.48), and who were formula-fed before 6 months (OR: 2.16, 95% CI; 1.14-4.10). We found no significant association between the rate of food allergy outgrowth or food induced-anaphylaxis with regards to the timing of introducing egg-white into the diet. Conclusions: Although the introduction of egg-white into infant diet at 6-12 months of life appeared as an independent risk for any food allergy, none of the patients developed anaphylaxis. Age at symptom onset and outgrowing food allergy were similar compared to those introduced egg-white after 12 months. We recommend promoting exclusive breast-feeding during the first 6 months of life, and avoidance of prolonged restrictive diets for children with food allergy. (C) 2021 Codon Publications.
