Patrimoni a rischio sono quegli archivi audiovisivi anni Ottanta (anche di natura tea-trale) non ancora censiti nella loro completezza e che in alcuni casi, necessitano di restauro a fini di conservazione e valorizzazione dei contenuti. dedicato tutta la sua vita alla produzione, all’archiviazione e restauro di materiali d’arte audiovisivi e infine, all’insegnamento; il suo laboratorio (un vero e proprio “museo storico del video”) ha ancora macchine antiche infunzione (116 videoregistratori, 90 proiettori, 74 registratori audio, 138 macchine fotografiche di tutte le epoche) grazie alla costante manutenzione, perfettamente in grado di leggere e duplicare formati oggi obsoleti, di dif-ficile reperibilità anche per Mediateche regionali. Interviene su VHS e Super VHS, video 8, Hi 8, U-Matic -BVU, Betamax, Betacam, Video 2000 per digitalizzarli e acquisire i contenuti prima che questi vadano irrimediabilmente perduti2. Cataract surgery in patients with pseudoexfoliation syndrome: current updates
Luigi Fontana, Marco Coassin, Alfonso Iovieno, Antonio Moramarco, Luca Cimino Ophthalmology Unit, Arcispedale Santa Maria Nuova – IRCCS, Reggio Emilia, Italy Abstract: Pseudoexfoliation is a ubiquitous syndrome of multifactorial origin affecting elderly people by increasing the risk of cataract and secondary glaucoma development. Despite modern techniques and technologies for cataract surgery, pseudoexfoliation syndrome represents a challenge for surgeons because of the increased weakness of the zonular apparatus and limited pupil dilation. Due to the inherent difficulties during surgery, the risk of vitreous loss in these patients is several times higher than in cataract patients without pseudoexfoliation. Using currently available surgical devices (ophthalmic viscosurgical device, iris retractors and ring dilators, capsular tension ring, etc.), the risk of intraoperative complications may be much reduced, allowing the surgeon to handle difficult cases with greater confidence and safety. This review analyzes the methodologic approach to the patient with zonular laxity with the aim of providing useful advices to limit the risks of intraoperative and postoperative complications. From the preoperative planning, to the intraoperative management of the small pupil and phacodonesis, and to the postoperative correction of capsule phimosis and intraocular lens dislocation, a step approach to the surgical management of pseudoexfoliation patients is illustrated. We appreciate the interest that our work has generated, and we take this opportunity to address Dr. Levitz’s concerns. In our paper, we wanted to evaluate whether the addition of NSAID eyedrops to topical steroids could benefit patients with PXF after cataract surgery. The primary efficacy endpoint of the study was a 30% postoperative reduction in anterior chamber flare in Group 1 when compared with Group 2 at 3 days. Eyes in Group 1 received dexamethasone 0.1% and tobramycin 0.3%. Eyes in Group 2 received bromfenac drops in addition. Anterior chamber inflammation was measured with a FM-700 laser flare meter (Kowa Co. Ltd.). We showed that the addition of bromfenac to dexamethasone drops significantly reduced the postoperative flare in PXF patients.
Macular edema was clearly proposed as 1 of the secondary endpoints of our study. The clinical trial was designed and the sample size calculated to address the primary endpoint. We agree that a study of the effect of NSAIDs on postoperative macular edema would have required a larger number of participants. The relative value of our findings on CME and the relevance of this secondary endpoint were discussed in detail in the Discussion section of our paper.
Dexamethasone drops were prescribed 4 times a day and then tapered, as is often done after cataract surgery. Both groups received therapy for 2 weeks because bromfenac is not approved for longer treatment in the European Union. We agree that data on longer or stronger treatment regimens would have been of interest. We see that patients in the experimental group received more drops than the ones in the standard group. This is in line with our purpose, which was to study the effect of adding topical NSAIDs to steroids
Laser flare and cell photometry to measure inflammation after cataract surgery: a tool to predict the risk of cystoid macular edema
Purpose To review the current adoption of laser flare and cell photometry (LFCP) in the setting of modern cataract surgery to analyze inflammation aiming to identify evidence of a correlation between LFCP values and the risk of cystoid macular edema (CME) development. Methods An extensive PubMed literature search was performed to review all the published studies investigating inflammation by LFCP after uncomplicated phacoemulsification. The following keywords were used: cataract surgery, cystoid macular edema, anterior chamber inflammation, laser flare, and cell photometry. Results Seventy-eight original articles investigating inflammation by LFCP were selected. Macula alterations were searched in 12 (15%) trials, by optical coherence tomography or fluorescein angiography in 11 (14%) and 1 (1%) studies, respectively. Among them, 9 (12%) papers investigated the correlation between LFCP values and cystic changes to the macula: 7 (9%) and 2 (3%) studies identified a positive and negative correlation, respectively. Three (4%) papers did not perform any correlation analysis. Conclusion CME, as a consequence of uncontrolled postoperative inflammation, is a common cause for unfavorable visual outcomes following uncomplicated phacoemulsification with IOL implantation. After surgery, intraocular inflammation is generally assessed by qualitative methods. Although well-established and practical in uveitis, they are inadequate to detect the modest inflammatory response that usually occurs after uneventful phacoemulsification. LFCP correlate with the chance of macula alteration after surgery and higher the values higher the risk of CME. The quantitative analysis of intraocular inflammation by LFCP after cataract surgery might be a tool to predict the risk of pseudophakic CME

Vernal keratoconjunctivitis
Vernal keratoconjunctivitis (VKC) is an allergic eye disease that especially affects young boys. The most common symptoms are itching, photophobia, burning, and tearing. The most common signs are giant papillae, superficial keratitis, and conjunctival hyperaemia. Patients with VKC frequently have a family or medical history of atopic diseases, such as asthma, rhinitis, and eczema. However, VKC is not associated with a positive skin test or RAST in 42-47% of patients, confirming that it is not solely an IgE-mediated disease. On the basis of challenge studies as well as immunohistochemical and mediator studies, a Th2-driven mechanism with the involvement of mast cells, eosinophils, and lymphocytes has been suggested. Th2 lymphocytes are responsible for both hyperproduction of IgE (interleukin 4, IL-4) and for differentiation and activation of mast cells (IL-3) and eosinophils (IL-5). Other studies have demonstrated the involvement of neural factors such as substance P and NGF in the pathogenesis of VKC, and the overexpression of oestrogen and progesterone receptors in the conjunctiva of VKC patients has introduced the possible involvement of sex hormones. Thus, the pathogenesis of VKC is probably multifactorial, with the interaction of the immune, nervous, and endocrine systems. The clinical management of VKC requires a swift diagnosis, correct therapy, and evaluation of the prognosis. The diagnosis is generally based on the signs and symptoms of the disease, but in difficult cases can be aided by conjunctival scraping, demonstrating the presence of infiltrating eosinophils. Therapeutic options are many, in most cases topical, and should be chosen on the basis of the severity of the disease. The most effective drugs, steroids, should however be carefully administered, and only for brief periods, to avoid secondary development of glaucoma.A 2% solution of cyclosporine in olive oil or in castor oil should be considered as an alternative. The long-term prognosis of patients is generally good; however 6% of patients develop corneal damage, cataract, or glaucoma

Enzymatic systems able to reduce either dehydroascorbate or ascorbyl radical back to ascorbate by "recycling" vitamin C may contribute to lowering the nutritional requirement of it and to increase tissue antioxidant capacity. The activities of two enzymatic activities, GSH-dehydroascorbate reductase (two-electron reduction pathway) and NADH-semidehydroascorbate reductase (one-electron reduction pathway) in pig tissues, have been investigated. The activity of glutathione-dependent reduction of dehydroascorbate, although measurable, appeared negligible taking into consideration the low physiological substrate concentration.
