    The growth of private hospitals and their health workforce in China: a comparison with public hospitals

    Background: China significantly opened its healthcare market through a series of market-opening policies in 2000–1. This study aims to explore the direct consequences of these policies—the growth of private hospitals, their workforce characteristics compared with public hospitals in China and the source of their healthcare workforce.Methods: first, we performed a segmented regression analysis of a longitudinal data series of the number of hospitals in China between 1990 and 2009 to examine the before and after effects of the market-opening policy on private hospitals. Then, to highlight the workforce differences between private and public hospitals, provincial survey data collected in 2009 were compared with data from a second database collected in 2002 to detect the mobility of medical staff from the public to the private hospitals.Results: the number of private hospitals rapidly increased after 2001, and the yearly growth rate increased from 19 to 205, represented primarily by an increase in specialty hospitals. Approximately 22.03% of the physicians in private hospitals are over the age of 60, whereas this proportion in public hospitals is only 2.97%. In 2008, at least 4.1% of the staff working in private hospitals had previously worked in local public hospitals in 2001.Conclusion: the broad expansion of private hospitals since 2001 is most likely the result of an unbiased market policy environment for private hospitals. Moreover, specific features of the hospital–physician relationship in China may account for the unbalanced age distribution feature among doctors and the mobility of the healthcare workforce in private hospitals

Routine grading of students’ work, however, is time consuming. Automating the grading process allows instructors to spend more of their time helping their students learn and engaging their students with more open-ended, creative activities. One way to automate grading is through computer-based assessment, where questions can be randomly parameterized and deterministic algorithms can be implemented to grade students’ responses to these questions. Computer-based assessment is particularly well suited for courses in STEM, where many of the building block skills can be formulated as algorithmically-scored tasks. This thesis focuses on computer-based assessment in three directions. The first direction concerns the adoption of computer-based assessments in a summative context, where I demonstrate that randomization of questions and a centralized proctored facility result in secure asynchronous computer-based summative assessments. In the second direction, I attempt to extract patterns from computer-based assessment data that allow instructors to adjust the assessments for the better. Lastly, I present an attempt to expand the set of autograded questions to include “Explain in Plain English” questions where students are asked to give natural language descriptions of what a piece of code does. I discuss the lessons learned from the attempt and propose how automated short answer grading should be evaluated in general

    Quantifying the functional and evolutionary relationships among sequences, transcription factor binding and gene expression

    A central challenge in regulatory genomics today is to understand the precise relationship between regulatory sequences, transcription factor (TF) binding and gene expression. Many studies have discussed how TFs recognize their DNA binding sites. However, it is not well understood how the various factors that influence TF-DNA binding alter the cascade of gene expression. Moreover, mutations in regulatory sequences are a key driving force of evolution and diseases. A number of studies have examined the sequence motif turnover and divergence in TF binding across species. However, there is currently a lack of clarity on what these changes mean to enhancer function. In this thesis, we used computational and statistical methods to quantitatively and systematically examine the relationships among regulatory sequences, TF binding, and gene expression, from both functional and evolutionary perspectives. At the functional level, we extended thermodynamics-based statistical models of the genetic sequence-to-function relationship to accurately predict gene expression. We incorporated chromatin accessibility and structural biological data into the models, described in Chapter 2 and 3. In doing so, we aimed to better identify transcription factor binding sites likely to influence gene expression, and thus, enhance the models’ capacity to predict gene expression. We demonstrated these improvements to gene expression modeling in Drosophila melanogaster by integrating DNaseI hypersensitivity assays and DNA shape. At the evolutionary level, we focused on regulatory variations between two distant Drosophila species to access inherent properties of enhancers, as described in Chapter 4. We used statistical and computational approaches to quantitatively examine the extent to which sequence and accessibility variations can predict TF occupancy divergence and enhancer activity change. We also found combinatorial TF binding can buffer variations at individual TF level to avoid drastic gene expression changes.Submission original under an indefinite embargo labeled 'Open Access'.     Concept and entity grounding using indirect supervision

    Extracting and disambiguating entities and concepts is a crucial step toward understanding natural language text. In this thesis, we consider the problem of grounding concepts and entities mentioned in text to one or more knowledge bases (KBs). A well-studied scenario of this problem is the one in which documents are given in English and the goal is to identify concept and entity mentions, and find the corresponding entries the mentions refer to in Wikipedia. We extend this problem in two directions: First, we study identifying and grounding entities written in any language to the English Wikipedia. Second, we investigate using multiple KBs which do not contain rich textual and structural information Wikipedia does. These more involved settings pose a few additional challenges beyond those addressed in the standard English Wikification problem. Key among them is that no supervision is available to facilitate training machine learning models. The first extension, cross-lingual Wikification, introduces problems such as recognizing multilingual named entities mentioned in text, translating non-English names into English, and computing word similarity across languages. Since it is impossible to acquire manually annotated examples for all languages, building models for all languages in Wikipedia requires exploring indirect or incidental supervision signals which already exist in Wikipedia. For the second setting, we need to deal with the fact that most KBs do not contain the rich information Wikipedia has; consequently, the main supervision signal used to train Wikification rankers does not exist anymore. In this thesis, we show that supervision signals can be obtained by carefully examining the redundancy and relations between multiple KBs.     In the version of this article, there were errors in Fig. 2a and d. In Fig. 2a, we have changed Cu2+ to Cu(0) in the revised version. While the two references cited in our paper used Cu2+ in their schematics,1,2 we believe that Cu(0) is the correct representation for the electrochemically mediated amine regeneration (EMAR)3. To be clear, the Cu metal anode is oxidized into cupric ions. The cupric ions then bind to the carbamate and displace the CO2 and form a copper–amine complex. The copper–amine complex is then reduced at the cathode where Cu metal is plated out. In Fig. 2d, we have changed the polarity of the cathode and anode in the revised version. A proton is released at the anode, while a hydroxide is released at the cathode. We have also simplified the quinone/hydroquinone chemistry in the revised version to be consistent with proton and hydroxide stoichiometry. The original and revised Fig. 2 images are shown below.
