BMC Neurology. 2021;21(1): 414.BACKGROUND: While Covid-19 predominantly affects the respiratory system, neurological manifestations including encephalitis occur in some patients, possibly affecting the course and outcome of the disease. Here, we describe a unique case of a young man with Covid-19 and transient MOG-positive encephalitis, with a benign course.; CASE PRESENTATION: A 22-year-old male, with PCR confirmed Covid-19 infection was admitted because of persistent headache. The clinical examination was normal. Neuropsychological testing revealed distinct executive deficits. Brain MRI and cerebrospinal fluid (CSF) analysis were suggestive for encephalitis. Further laboratory examination revealed a serum MOG antibody titre. The headache improved with analgetic treatment and i.v. methylprednisolone. Consequently, the MOG antibody titer decreased and MRI lesions were resolving. The patient made a full recovery, with no signs of deterioration over the following months.; CONCLUSIONS: Covid-19 manifestations in the CNS include encephalitis with variable course and prognosis. This case highlights a possible association between inflammation due to COVID-19 and transient secondary autoimmunity with transient MOG antibodies and atypical clinical presentation. © 2021.     Prevalence and determinants of exclusive breastfeeding among lactating mothers in an urban slum, West Tripura: a cross sectional study

    Background: Exclusive breastfeeding is the optimal way to feed children during their first months of life, having important benefits for them and their mothers. Despite of all benefits, the practice of exclusive breastfeeding is quite low and information on influencing factors is limited especially from slum settlements. Objectives was to estimate the prevalence of exclusive breastfeeding and to find out the factors affecting exclusive breastfeeding among lactating mothers in an urban slum, West Tripura.Methods: A cross sectional study was conducted among 200 lactating mothers for a period of six months in an urban slum using structured interview schedule. Eligible mothers were selected by sample random sampling technique. Data has been analyzed by SPSS statistical software version 17.Results: Majority (43%) of the mothers was aged between 21-25 years and 89% were non working mothers. Among the participants, 55.5% were Hindu by religion, 60% belonged to nuclear family and 43.5% had studied up to primary level. The prevalence of exclusive breastfeeding rate among the study mothers were 60.5%. Only parity, place of delivery and number of antenatal check up were significantly associated with exclusive breastfeeding.Conclusions: Exclusive breast feeding (EBF) rate still needs to improve. Community level interventions like awareness programmes on exclusive breastfeeding, health education and behavior change communication among the target group is essential to improve the exclusive breastfeeding rate especially in slum settlements

    Ocular flutter as presenting manifestation of pediatric MOG antibody–associated demyelination: A case report

    A 13-year-old girl presented with a 5-day history of oscillopsia. On examination, ocular flutter and mild cerebellar signs were found. Brain magnetic resonance imaging (MRI) revealed four periventricular and subcortical non-enhancing lesions. Cerebrospinal fluid (CSF) oligoclonal bands were negative. Neuroblastoma or other malignancies were not found. She responded well to a corticosteroid–intravenous immunoglobulin (IVIG) combination and remained symptom-free for 3 years until presenting again with isolated ocular flutter. Brain MRI at this time remained atypical for classic multiple sclerosis (MS) with a predominance of juxtacortical demyelinating lesions. CSF was positive for oligoclonal bands. Serum myelin oligodendrocyte glycoprotein (MOG) antibodies were present. Ocular flutter can be the presenting feature of MOG antibody–associated pediatric demyelination. © The Author(s), 2018

    A North Sea offshore grid governance model: The allocation of ownership and operating responsibilities for a Meshed Offshore Grid

    EU climate goals force EU member states to increase the share of renewable electricity generation in their respective electricity markets. To reach these climate goals, offshore wind energy is regarded as one of the main technologies that can contribute to a full transition towards a fossil-free electricity system. Furthermore, 230 Gigawatts (GW) of offshore wind is projected to be necessary within the EU. Additionally, the EU has the objective to further integrate electricity markets within its member states. This further integration will therefore need a substantial increase in interconnection capacity between the current electricity markets. Both the increase in offshore wind energy and the increase in interconnection capacity will require offshore electricity transmission infrastructure. Currently, offshore wind generation is connected through individual (radial) offshore grid connections and interconnection capacity used for cross border trade is still developed through point-to-point interconnection. However, a meshed offshore grid (MOG) can combine the aforementioned functions of evacuating offshore wind and interconnect electricity markets. Additionally, a MOG is expected to provide additional economic benefits compared with the individual development of offshore wind farms (OWF) grid connections and the interconnection capacity between electricity markets. While a MOG can provide potential economic benefits, current development of a MOG is still lacking, caused by a variety of economic and regulatory knowledge gaps that need to be addressed. One of these knowledge gaps is which governance model, that allocates the responsibilities regarding the ownership and operation of such a MOG, is preferable for coordinated offshore grid developments. This research project is therefore focussing on this knowledge gap. In order to address this gap, an analytical framework is created through a literature review and desk research that provides both a foundation to conceptually develop the design space of a governance model, while also providing the tools to analyse the expected performance of a governance model. Additionally, a quantitative comparative analysis of currently applied governance models for individual OWF grid connections is performed to provide input for the analysis of governance models that can be applied for a MOG. Through the design space, four different governance models were constructed and analysed, based on the analytical framework. Three of the constructed governance models rely on long-term contracts and further unbundling of electricity grid activities (being electricity transmission and system operation), while one governance model (the TSO model) relies on regulation and bundling of the electricity grid activities. The analysis of these governance models shows that there is no optimal governance model. However, to facilitate the efficient deployment of a MOG, the TSO model is considered most preferable, as this governance model is able to facilitate the gradual development of a MOG, while also facilitating efficient operation by limiting coordination issues that can arise due to unbundling. This model will rely on current owners of the TSOs to provide the necessary investment capital and it remains to be seen whether these owners have the willingness to do so, provided the enormous scale of investments to reach EU climate goals.Systems Engineering, Policy Analysis and Management (SEPAM

    Treatment strategies of AQP4-IgG positive neuromyelitis optica spectrum disorder and MOG antibody-associated disorder in Switzerland: a nationwide survey

    Neuromyelitis optica spectrum disorders (NMOSD) and Myelin Oligodendrocyte Glycoprotein (MOG) antibody-associated disease (MOGAD) are neuroinflammatory conditions characterized by attacks, primarily affecting the spinal cord and the optic nerve. When left untreated, these disorders can result in severe neurological disability. Although recent advancements have improved treatment, many questions remain regarding the optimal management of these rare conditions. We conducted a national survey among neurologists in Switzerland experienced in treating NMOSD and MOGAD. The survey comprised 42 questions covering diagnostic methods, acute treatment, maintenance immunotherapy and approaches to long-term strategy. Twenty-one out of 28 invited neurologists took part in the survey (response rate 75 %). There was high consensus on treating acute attacks with high-dose steroids and with plasmapheresis in severe cases. In NMOSD, 71.4 % recommended oral steroid tapering, compared to 85.7 % in MOGAD. All participants advocated maintenance treatment after the first attack in NMOSD, compared to only 10 % in MOGAD. Indeed, many participants advised starting therapy in MOGAD after the first attack only in cases of severe attacks (38 %), persistent MOG-antibodies (10 %) or both (19 %). Rituximab was the most used first-line maintenance immunotherapy for both diseases. Approaches to treatment strategy for the long-term varied with a tendency to recommend de-escalation or discontinuation in stable patients with MOGAD, but not with NMOSD. This study highlights current treatment approaches to NMOSD and MOGAD across Switzerland. Rituximab remains the most prescribed drug for both conditions. The overall variability in recommendations underscores the need for greater awareness of disease-specific management and for further research to optimize treatment strategies for patients with NMOSD and MOGAD.     The principle of Reformed intertextual interpretation

    There has been a growing interest in intertextuality as a hermeneutical category in contemporary current biblical studies. The texture of a particular text is thickened and its meaning extended by its interplay with other texts, especially when the reader recognizes that the repetition of similar phrases and subject matter form part of an integral whole. The concept of intertextuality in this article firstly challenges the traditional approach that assumes that there is one meaning in a text that can be deduced when the author's intention is determined. Secondly, it disagrees with the New Criticism in which only the autonomous text plays the dominant interpretive role. The reader is considered to be merely a passive consumer of the text. Thirdly, it differs from the post-structural/deconstructional way which declares “the death of the author”

    This article presents seven early intervention programmes in two towns in southern England. Three are close linked to schools, one works with school-aged children but outside the school, and three programs work exclusively with preschool children and parents in community settings. Although the evaluative investigation of these programs is still halfway, some tentative insight into what they do for parents can be given. It is the social support provided by the programs which parents value most. The author draws special attention to the phenomenon of dropping out of the program.     Remembering his friendship with the late Prof. Ante Glavičić – in this first decade – the author states: we, as we live now, work and try to do only good things, like him, but we will also inevitably disappear when it is our time, not knowing when that will be?! Ante’s time came when he was 72, studiously working 41 years of service. He lived 6 years into his pension, although still working and useful. He left unexpectedly, calmly, not burdening anyone with his patient’s pain. His friends said: and so they leave life those who the Almighty rewarded believing that – or maybe not - we might also experience it ourselves? According to the author it would be true happiness! With this unavoidable fact and behind him now only memories live of the remaining trace different from the others – obviously due to people’s various ways of life. The memory is bordered with personal conduct, character and a wide human activity, which someone performed or by some facts separated himself from others by their habits and preferences. Does the author also really believe that this was the case with Prof. Ante Glavičić!? With his declining years the author of these lines understood that there is only one happiness, and that is the duty which we have (or don’t have!), and too often the only consolation is work, because the secret of the human being is not only in that it lives, but in that – for that which it lives? Ante obviously knew why he wanted to live from his days of studies to the last conversation about his Senj and Velebit, which he also intrinsically embraced with his gained knowledge. For the author he has remained Velebit’s Ante, because it is also there that he best got to know how to follow him on his trial, when he had the opportunity and possibility, so again he returns alongside him again on this trail…Sjećajući se mog druženja s pokojnim prof. Antom Glavičićem - pri ovom prvom desetljeću - svjestan sam činjenice – da mi, koji sada živimo – radimo i nastojimo činiti samo dobro, kao i on, ali ćemo i mi neminovno nestati kada za to naš čas dođe, ne znajući kada će to biti?! Antin čas došao je u njegovoj 72. godini, marljivo odradivši 41 godinu radnog staža. U mirovini je živio 6 godina, ali i dalje radno i korisno. Otišao je neočekivano – po mom mišljenju smireno, ne opterećujući nikoga bolesničkim mukama. Moji stari su govorili: "tako iz života odlaze oni koje je Svevišnji nagradio vjerujući da – ili – ne, možda doživimo i sami? Po mome, bila bi to zaista sreća! S tom neumitnom činjenicom i iza njega sada dalje žive samo sjećanja na ostavljeni trag različit od drugih – očito zbog ljudski različitog načina života. Sjećanje je omeđeno osobnim ponašanjem, karakterom i širinom ljudske aktivnosti, koju je netko obavljao ili se po nekim činjenicama izdvajao od drugih vlastitim navikama i sklonostima. Mislim, da je i u njega zaista bilo tako!? Uza svoje staračke godine shvatio sam da je samo jedno sreća, a to je dužnost koju imamo (ili nemamo!), a prečesto jedina je utjeha rad, jer tajna čovjekova bića nije samo u tome što se živi, već u tome – za što se živi?. Ante je očito znao za što želi živjeti od dana studija do zadnjeg razgovora o svom Senju i Velebitu, koje je istinski i prigrlio svojim stečenim znanjem. Za mene je ostao velebitski Ante, jer sam ga tamo i najbolje upoznao prateći ga na njegovu tragu, kada sam za to imao priliku i mogućnost, pa se sada ponovo vraćam uz njega na taj trag
