    Informed consent process: challenges in clinical practice

    The concept of informed consent dates back to nearly a century ago. It was instituted with the aim to safeguard a patient from unreasonable intrusion and abuse to their body integrity and to enrich patients with the ability of own choice for selecting the type of treatment as per their will. In other words, any action by a clinician, either for trial or practice purposes, involving an intrusion inside the patient’s body has to be conducted only after the approval of the patient (if conscious) or acceptance from family members, relatives, or friends of the patient (if unconscious). Application of informed consent prior to any procedure follows its implementation called ‘informed consent process’. In the present review article, the author focused on the challenge encountered by clinicians while implementing an informed consent process. Various databases were searched from inception to August 2020. Only English language articles were searched, with the following keywords: "Informed consent", "Informed consent process", and "Consent form", linked with Boolean words "AND", "OR", and "NOT". Informed consent process implementation should overcome various setbacks which have been experienced by clinicians working in government and private hospitals, nursing homes, clinics, old age homes, etc. These challenges were discussed in this review under various headings for proper identification, justifying the cause and measures to minimize their impacts

    The role of sports physiotherapist in confronting exercise addiction.

    Everyday physical activity plays an important part in health maintenance and disease prevention. Excess exercise, however, can cause detrimental effects on both physical and mental health. It can also hamper the quality of life to an extent that individual is unable to regulate this behavior. 'Addicted' exercisers are more likely to work out for intrinsic rewards and experience troubling feelings of deprivation. In comparison, 'committed' exercisers participate in physical activity for extrinsic benefits and, when they cannot exercise, do not experience extreme withdrawal symptoms. Sportspersons indulge themselves in long hours of training as a response of their quest to improve endurance and performance in the face of common setbacks. Recognizing the addiction to exercise is also a contentious idea and the aim of this article is to draw an attention towards the same. Exercise Addiction diagnosis needs employment of questionnaires such as Exercise Dependence Scale, Obligatory Exercise Questionnaire, and Exercise Addiction Inventory. They need to be employed in the pre-participation evaluation of an individual participating in any sports event. Physiotherapists are often the first to meet an individual with exercise addiction. Physiotherapists play an important role in diagnosis of exercise addiction as well as in prevention as they are aware of challenges of treating exercise addicted patients and develop specific approaches to deal with their issues.     Neuromuscular Taping in Diabetes Mellitus: A Narrative Review of Current Understanding and Potential Therapeutic Applications

    Abstract Background: Diabetes Mellitus (DM) poses significant challenges, including peripheral neuropathy and impaired muscle function. Neuromuscular taping has emerged as a noninvasive therapeutic approach, potentially offering benefits in managing DM-related complications. Materials and Methods: A comprehensive literature search was conducted to identify relevant studies investigating the use of neuromuscular taping in individuals with DM. Key databases were explored using Boolean operators (AND, OR, etc.), and articles were selected based on predefined inclusion criteria. Results: The review identifies 22 relevant articles, with 8 specifically addressing NMT in DM. Out of these, 5 were randomized controlled trials, highlighting the research interest in assessing the effectiveness of NMT. The findings suggest that NMT may positively impact the Ankle Brachial Index, indicative of improved peripheral blood circulation and many other health-related parameters. Neuromuscular taping principles and application methods are outlined, providing a foundation for understanding its potential therapeutic effects. Discussion: The potential therapeutic mechanisms of neuromuscular taping are explored, emphasizing its influence on sensory feedback, muscle activation, and circulation. Safety considerations and the feasibility of integrating neuromuscular taping into comprehensive DM management are discussed. Conclusion: Neuromuscular taping holds promise as an adjunctive therapy for managing DM-related complications. This structured narrative review provides a comprehensive overview for health care limitations such as a lack of standardized protocols and heterogeneity in study designs are noted

    Eficácia da estabilização central e fortalecimento supervisionado do quadril na osteoartrite de joelho: um protocolo de ensaio clínico randomizado

    INTRODUCTION: Osteoarthritis (OA) is a significant problem associated with pain; it reduces function, and hampered the quality of life. Only a few previous studies have established the comparative effect of core stability and hip strengthening exercise. OBJECTIVE: To investigate core stability and its relationship with supervised hip strengthening exercise in the management of knee OA. METHODS: This is a single centered project, two-group, pre-test, a post-test, randomized clinical trial where Forty-six patients with knee OA will be randomly allocated into two groups, Group A (experimental group 1) and B (experimental group 2) will receive core stability and hip strengthening exercise on three days a week for four weeks respectively across twelve treatment sessions. The primary outcome measure will be Knee Injury and Osteoarthritis Outcome measure (KOOS) in both English and Hindi versions, and secondary outcome measures will be Timed Up and Go test (TUG), Chair stand test to quantity pre and post effect of the intervention. EXPECTED RESULTS: This study planned to assess the efficacy and importance of core stability and hip strengthening exercise in reducing pain, improving function and quality of life in patients with knee OA. OBJETIVO: Investigar a estabilização e sua relação com o exercício supervisionado de fortalecimento do quadril no tratamento da OA do joelho. METODOLOGIA: Este é um projeto de ensaio clínico centralizado, dois grupos, pré-teste pós-teste, randomizado, onde quarenta e seis pacientes com OA de joelho serão alocados aleatoriamente em dois grupos. Grupo A (grupo experimental 1) e B (grupo experimental 2) receberão exercícios de estabilização e fortalecimento do quadril três dias por semana durante quatro semanas, respectivamente, em doze sessões de tratamento. A medida de desfecho primário será a medida de desfecho de lesão no joelho e osteoartrite (KOOS), ambas as versões em inglês e hindi, e as medidas de desfecho secundário serão o teste Timed Up and Go (TUG), o teste de suporte da cadeira para quantidade pré e pós efeito da intervenção. RESULTADOS ESPERADOS: Este estudo planejou avaliar a eficácia e a importância da estabilização e exercícios de fortalecimento do quadril na redução da dor, melhorando a função e a qualidade de vida em pacientes com OA de joelho.     BACKGROUND AND PURPOSE: The literature has shown the promising effect on shoulder internal rotators strengthening on fast bowlers. However, no previous study has demonstrated the effect of muscle energy technique for shoulder external rotators on fast bowlers. This study aims to analyze the efficacy of shoulder internal rotators muscles strengthening and muscle energy technique for external rotators enhancing the bowling in speed among fast bowlers in cricket. METHODS/DESIGN: Participants will be screened based on the inclusion and exclusion criteria. Demographic data shall be collected. Participants taken shall be in the age group between 18-30 years, be randomly assigned into two groups: exercise group and MET group. Outcomes considered in the study are Universal Goniometer and Bowling speed km\h. This is a two-group, pre-test, post-test randomized clinical trial. Participants will receive 5 sessions per week within 2 months. The range of motion and speed of the ball is the primary outcome measures to the pre and post-intervention effect of the treatments. PERSPECTIVES: Results from this study can provide the basis for studying the strengthening of the shoulder internal rotator muscles and MET of shoulder external rotators in rehabilitation and clinical setups, which will provide additional help to the bowlers increase bowling speed. The procedure of the study is noninvasive, safe, and feasible. The efficacy of shoulder internal rotator strengthening and muscle energy technique for shoulder external rotators on fast bowlers in each group will be established with this study. TRIAL REGISTRATION: Clinical Trials Registry- India. No entanto, nenhum estudo anterior demonstrou o efeito da técnica de energia muscular para rotadores externos de ombro em arremessadores rápidos. O objetivo deste estudo foi analisar a eficácia do fortalecimento dos músculos dos rotadores internos do ombro e da técnica de energia muscular para os rotadores externos aumentando a velocidade do boliche entre jogadores de críquete. MÉTODOS / DESENHO: Os participantes serão selecionados com base nos critérios de inclusão e exclusão. Dados demográficos devem ser coletados. Os participantes escolhidos devem estar na faixa etária entre 18-30 anos, sendo distribuídos aleatoriamente em dois grupos. Grupo exercício e grupo MET. Os resultados considerados no estudo são Goniômetro universal e velocidade de boliche km \ h. Este é um ensaio clínico randomizado de dois grupos, pré-teste e pós-teste. Os participantes receberão 5 sessões por semana em 2 meses. A amplitude de movimento e a velocidade da bola são as medidas de resultado primárias para o efeito pré e pós-intervenção dos tratamentos. PERSPECTIVAS: Os resultados deste estudo podem fornecer a base para estudar o fortalecimento dos músculos do rotador interno do ombro e MET dos rotadores externos do ombro para serem usados na reabilitação e instalações clínicas que fornecerão ajuda adicional aos jogadores de arremesso para aumentar a velocidade do boliche.     Impact of Aerobic Exercise on Physical Health, Cardiorespiratory Parameters, and Health-Related Quality of Life Among Children With Diabetes Mellitus: A Narrative Review

    Diabetes mellitus (DM) in children poses significant challenges to their physical health and overall well-being. While aerobic exercise (AE) has been extensively studied in managing DM, its role remains underexplored in the pediatric population. This narrative review aims to systematically evaluate the impact of AE on physical health, cardiorespiratory parameters, and health-related quality of life (HRQoL) in children with DM. A comprehensive literature review was conducted, focusing on studies examining AE interventions in children with DM. The review assessed the effects on physical health, cardiorespiratory fitness, and HRQoL, utilizing more commonly used HRQoL tools, such as the Pediatric Quality of Life Inventory (PedsQL). A total of eight studies with 589 participants were included in the review. The mean age of participants was 12.4 ± 2.8 years. Evidence indicates positive influences on cardiorespiratory parameters, including improved endothelial function, increased aerobic capacity, and better blood pressure control. HRQoL assessments reveal improved self-esteem, mood, and overall well-being, attributed to the physical and psychological benefits of regular exercise. AE holds significant potential as an adjunctive therapy to improve physical health, cardiorespiratory parameters, and HRQoL in children with DM. The comprehensive evaluation of social, psychological, and physical effects using HRQoL tools, such as PedsQL, and other factors, such as independent functioning, underscores the importance of integrating AE into diabetes management plans

    Effect of Tactile and Kinaesthetic Stimulation in Preterm Neonates with Hyperbilirubinaemia: A Study Protocol

    Introduction: Elevated Total Serum/Plasma Bilirubin (TSB) levels in the blood manifest as hyperbilirubinaemia, which is commonly observed among newborns and can result in significant long-term consequences. Need for the study: Long-term phototherapy may have negative effects on newborns. These effects can potentially be mitigated if tactile and kinaesthetic stimulation are proven to be effective as adjuvants to phototherapy, thereby reducing its duration. Aim: The study aims to investigate the effect of tactile and kinaesthetic stimulation among preterm neonates with hyperbilirubinaemia. Materials and Methods: Ninety-four neonates with hyperbilirubinaemia admitted to a Neonatal Intensive Care Unit (NICU) will be recruited and allocated through block randomisation into two groups: the experimental group (massage therapy + tactile stimulation + phototherapy) and the control group (only phototherapy). TSB levels, Transcutaneous Bilirubin (TcB) levels, and weight gain in neonates will be measured as outcomes at baseline and each day for two days following treatment

    Introduction:&nbsp; Gynaecological cancer growth represents more than one out of eight diseases.12.5% of female malignancies are genital group: uterus (4.8%), ovary (3.7%), cervix (2.7%), and others including vulva (1.3%). Vulva Cancer is generally an uncommon infection of women but currently the occurrence is of 2-3 for every 100,000 females and a middle age of 65-70, vulvar malignancy has shown an expanding frequency (20% somewhere in the range of 1973 and 2000) with simultaneously diminishing middle age at beginning throughout recent many years. Method: Multi layered bandaging was applied for 22 hours every day over left lower limb followed by a session of active exercises of lower limb muscles. Materials used: Stockinette (inner lining), Padding made of foam, short stretch bandages using measurement scales: Circumference measurement at given level, Quality of life EORTC QLQ- C30, LEFS Lower extremity functional scale.     Disaster Health Management and Competencies Among Physiotherapy Professionals: An Exclusive Review with Bibliometric Analysis

    Background: Disaster health management is the management of concerns related to the health of populations and medical facilities before, during, and after a disaster. This paper aims to estimate the contribution of physiotherapy professionals in disaster health by literature search and bibliometric study. Methods: The review is devoted to identifying physiotherapist competencies in disaster settings and the possible possibilities of their usage in emergency medical teams. The study underlines the shortcomings in the existing literature review examining this issue and underscores the need to strengthen physiotherapists' disaster health management competencies further. Therefore, one needs to take cognizance of the diverse range of skills that physiotherapists possess to enhance their function in emergency response units and preparedness and response to disasters. Results: The articles reviewed totaled 654 of them on Disaster Health Management in Physiotherapy with increased research activity observed which is spearheaded by the U.S while Monash University was found to have contributed the most. Some of the key themes that came out were; education, competency, and physiotherapy with Physical Therapy journal publishing most articles. Conclusion: This study covers general background information, areas of interest, and recent developments in Disaster Health Management and Physiotherapy Competencies. According to the analyzed literature, there is evidence that the amount of work done in these fields is increasing, and there are good opportunities to share information among researchers, institutions, and countries
