Use of cell fat mixed with platelet gel in progressive hemifacial atrophy
Progressive hemifacial atrophy, also known as Parry-Romberg syndrome, is an uncommon degenerative and poorly understood condition. It is characterized by a slow and progressive atrophy affecting one side of the face. The incidence and cause of this alteration are unknown, and the pathogenesis of the syndrome is not clear. Some authors attribute the atrophy of the subcutaneous system to an alteration of the sympathetic system. Others attribute it to an alteration of the nervous system at the encephalic level or to an interstitial neuritis of the trigeminal nerve. The most common complications that appear in association with this disorder are trigeminal neuritis, facial disorders, and epilepsy. The latter is the most frequent complication of the central nervous system. Characteristically, the atrophy progresses slowly for several years and soon becomes stable. After stabilization of the disease, plastic surgery of autogenous fat grafts can be performed. This study aimed through the presentation of clinical cases to suggest a therapeutic plan comprised of two sequential treatments: aquisition of platelet gel from a small volume of blood (9 ml) followed by the Coleman technique for reconstructing the three-dimensional projection of the face contour, restoring the superficial density of the facial tissues. The results obtained prove the efficacy of these two treatments combined, and the satisfaction of the patient confirms the quality of the results
The effect of autologous activated platelet rich plasma (AA-PRP) injection on pattern hair loss: clinical and histomorphometric evaluation
To investigate the safety and clinical efficacy of AA-PRP injections for pattern hair loss. AA-PRP, prepared from a small volume of blood, was injected on half of the selected patients' scalps with pattern hair loss. The other half was treated with placebo. Three treatments were given for each patient, with intervals of 1 month. The endpoints were hair re-growth, hair dystrophy as measured by dermoscopy, burning or itching sensation, and cell proliferation as measured by Ki-67 evaluation. At the end of the 3 cycles of treatment, the patients presented clinical improvement in the mean number of hairs, with a mean increase of 18.0 hairs in the target area, and a mean increase in total hair density of 27.7 ( number of hairs/cm(2)) compared with baseline values. Microscopic evaluation showed the increase of epidermis thickness and of the number of hair follicles two weeks after the last AA-PRP treatment compared to baseline value (P < 0.05). We also observed an increase of Ki67(+) keratinocytes of epidermis and of hair follicular bulge cells and a slight increase of small blood vessels around hair follicles in the treated skin compared to baseline (P < 0.05)
Zone, bordi, frontiere.
Systematic review: Oncological safety of reconstruction with fat grafting in breast cancer outcomes
background: autologous fat grafting (AFG) has become a commonly used procedure for breast reconstruction (BR) after cancer removal. nevertheless, oncological considerations remain for AFG after breast cancer surgery.objectives: this article aims to evaluate the oncological safety of AFG in BR and its effect on disease-free survival (DFS) and local-regional recurrences (LRR).methods: a systematic review regarding the use of AFG in BR to identify a difference in in-cidence rates of LRR and DFS between patients who had AFG and controls was performed us-ing pubmed, MEDLINE, embase, preMEDLINE, ebase, CINAHL, PsycINFO, clinicaltrials.gov, sco-pus, and cochrane databases. the protocol was developed following the preferred reporting for Items for systematic reviews-protocols (PRISMA-P) guidelines. the included studies had to match predetermined criteria according to the PICOS approach.results: a total of 11 studies were included. seven studies reported LRR, and 5 studies re-ported DFS in 5,886 patients. our systematic review showed that AFG was not associated with increased LRR and DFS. pooled hazard ratios (HRs) (95% confidence intervals [CIs]) for LRR and DFS were 1.26 (0.90-1.76) and 1.27 (0.96-1.69), respectively. conclusions: AFG can, therefore, be performed safely in BR after breast cancer. further, ran-domized controlled trials and related systematic reviews, as well as evidence-based medicine(EBM) studies of level 1, are required to consolidate the results of the studies identified in this systematic review.(c) 2022 british association of plastic, reconstructive and aesthetic surgeons. published by El-sevier Ltd. all rights reserved
Fibroadenoma in the bilateral accessory axillary breast
The authors treated a case of bilateral accessory axillary breast tissue. Excision with histologic examination confirmed the diagnosis of fibroadenoma. Treatment left the woman with incision scars (3.5 cm) in the axillary pyramid, a location often not seen during a patient's normal movements. Thus, despite a minor aesthetic incision, gives the advantage of complete histologic analysis was gained. Liposuction treatment was used in this case. The scar results were good
