    Physician – Patient relationship in andrology: A new approach for an old scenario

    Physician- patient relationship (P-PR) is a central, critical and relevant element in medical care. In the spectrum of care of andrology, strong cooperation is needed from a large group of researchers and clinicians, who play different roles and, as a consequence, an intricate complex communication problem can arise. Techniques to build an appropriate P-PR relationships are reviewed here. The best andrological consultations begin when a good (i.e. perceived as safe by patients) setting and a sufficient amount of time is available. Active listening, kindness, empathy, competence, trust, efficacy and compliance/therapeutic alliance concur all together to build an appropriate P-PR relationship. Starting from a common agenda, the interaction process of P-PR should finally arrive at its goal, namely, shared decision making. The construction of a satisfactory P-PR is fundamentally important for a this. When there is a good P-PR relationship, a minimal effort is sufficient for finding a common ground for decision-making and P-PR conflicts occur seldom

    Bodily experiences in the elderly are caracterized by many aspects as biological changes age-related, comorbidity and cronicity of the diseases, functional impairment, need of many medications with side effects and iatrogenicity risk. Older people become still more bodily-centred for many remarkable reasons: the ill body as a disfunc- tional object and as a concrete evidence of time and of limited life perspective; the aged and suffering body with less and less sources of pleasure and sa- tisfaction; the body with its symbolic and commu- nicative meanings; the body as an organizer of the psyche and as a condensing area of several feelings (shame, rage, humiliation, incapacity, helplessness); the body as a projection screen of bad and re- jected part of the self. In the clinical context the manifold aspects of somatic expression of depres- sion in the aged people are well known. The authors stress from a psychodynamic perspec- tive the central role of bodily experience in suici- dality of aged depressed people: in these patients the unbearable mental pain (Shneidman, 1993) is strongly linked to the suicidal body (Orbach, 2008): feeling of helplessness, weakened or damaged ego, bodily dissociation and splits in the inner word that enable identification of bad parts of the self with the body, push the individual to cross the body barrier (Hale, 2008), i.e. to overcome the resistance to act out self-destructive fantasies

    AIM: To determine whether the surgical straightening of congenital penile curvature can improve intromission comfort, penile features, personal relationships and psychogenic erectile dysfunction (ED). METHODS: Fifty-four patients (mean age 24 years, range 20-31 years) whose congenital penile deviation due to physiological curvature was>or=25 degrees, as measured on a graph, and who were experiencing penetration discomfort were assessed specifically for the present study. Of these, 14 patients suffered from psychogenic ED. The assessment included a case history, an objective examination, a pharmacologically-induced erection with prostaglandin E1 10-20 g, a graph taken during erection, a basal and dynamic Duplex ultrasonograph, penile length measurement, nocturnal penile tumescence recording, hormonal profiles and a psychological interview to evaluate the quality of their personal relationships according to Hinde's parameters (contents, number, features, frequency, ability to perceive limits of mutuality, subjective perception of the other person[s], and reliability). All patients underwent the Nesbit procedure. The initial assessment was repeated at 3 and 12 months after surgery. Data analyses were carried out using the z test. RESULTS: Subjective judgement of cosmetic penile features and vaginal intromission comfort improved significantly after surgery whereas the quality of personal relationships and ED did not. CONCLUSION: The surgical straightening of congenital penile curvature improved intromission comfort and penile features, but it failed to improve interpersonal relationships or psychogenic ED
