Quadri borderline in adolescenza: aspetti diagnostici

    No full text
A brief literature's review about borderline syndrome is presented. The Authors stress how much the diagnosis is based upon the mental functioning rather than descriptive criteria. Therefore defence mechanisms, objectual relationship and integration of identity are fundamental to evaluate and understand borderline patients. This diagnosis is particularly difficult in adolescence because of similar aspects between adolescent process and borderline organisation. A sort of comparison between borderline and adolescent characteristics is presented both in terms of psychological functioning and clinical symptoms. A clinical vignette is described

The Authors think about the complex theme of aggressiveness, hopelessness and destructiveness in adolescence, trying to propose some reflections and inputs. Developing this subject the Authors use some clinical cases seen at the Department they work in, trying to read them in psychodynamic terms and using the point of view of different Authors (such as Winnicott, Meltzer, jeammet et al.). Two main concepts enliven this work. The first one is the importance of the environment for the adolescent's development: it is fundamental the way in which the others replay to adolescent's action in terms of the meaning acquired by this actions. The second one is the continuity exsisting within the internai world, so that it is possible to find pathological aspects in healthy people and healthy aspects in pathological people: comparing and weighing up the mixture of pathological and healthy aspects, each time combining in a different measure, give us the opportunity to understand and possibly help the adolescent we are dealing with

The Authors selected adolescents coming to the outpatients' department they work in, for age, sex, problems presented and some other characteristics. Data from the clinical study of two case records are presented, compared and discussed. Particular attention is paid to the analysis of the outcome of the two groups, following the application of different settings. The most interesting result is that both the right flexibility in approaching adolescents and the major space offered to the their parents in the second group of adolescent as compared to the first group seem to improve the outcome in terms of acknowledgement of an emotional disease and of the commitment to further psychological help
