    An Insight to Nanostructured Lipid Carrier System

    In pharmaceutical field, many drugs are being invented to combat the existing new diseases. The winds of change in the drug scenario are blowing forcefully worldwide. The emergence of new technologies provides an unique opportunities to exploit novel approaches in drug delivery. A shift from conventional drug delivery to novel drug delivery is noticed as shift from conventional drug delivery suffers from various drawbacks, But these new mighty compounds in drug therapy solely are not sufficient to meet the today’s need. There is an urgent need for the smart technology, as the drugs available suffer from serious problems like poor solubility and poor bioavailability. Most of the drugs that are available therapeutically comes under BCS class II ie, poorly soluble and high lipophilc. To assure progress in drug therapy, the development of new drugs merely is not sufficient. Issues arising in delivery of new drugs should also be addressed. Thus there is an emergent need to improve the bioavailability of these drugs, the only remedy or boon for such drugs is the discovery of some smart technologies which can improve the bioavailability of these drugs. Nano drug delivery systems are the one such universal approach which fulfills the lacuna, which exists in conventional drug delivery systems. These Nano drug delivery systems, improves the pharmacokinetic profiles of many drugs. In 1980 K. Eric Drexler developed and popularized the concept of nanotechnology. In this review a deep insight on Nano structured lipid carriers is discussed elaborating its birth, significant qualities compared to other colloidal systems, its structure, characteristics, preparation and application are spotted.     PRESCRIBING PATTERN OF ANTIHYPERTENSIVE INVOLVED IN HYPERTENSIVE PATIENTS IN ATERTIARY CARE TEACHING HOSPITAL

    Hypertension is one of the major global risk factor among the cardiovascular diseases leads to increase in morbidity, mortality rate as well as health burdens1 . In 2020 it has been estimated that 1.32 billion world populations will suffer from hypertension. As per current prevalence rate on hypertension in India is 18.52 and ranks at 67 among worldwide. WHO statistics reported that death rate among hypertensive patients in India was 1.79%. To determine the prescribing trends of antihypertensives involved in patients with hypertension. Out of 73 encounters, 43 (59%) males and 32(41%) females were observed and documented. Most patients with hypertension between the age group of 41 to 60 were 49(67.1%). The drug prescribing pattern involved with hypertension in this study was mostly single therapy (56.1%) when compared with combination therapy (43.9%). The most commonly used drug were amlodipine (4.1%) followed by clopidrogrel (8.2%), telmisartan (6.9%). The combination therapy was used frequently with Atorvastatin and fenofibrate (15%), Atenolol and amlodipine (11%), Telmisartan and hydrochlorothiazide (6.9%). The present study shows that ACE inhibitors and ARB blockers were frequently used in most of the prescriptions. Atorvastatin consumed more in all prescriptions followed by amlodipine and diuretics (furosemide). Key words: Hypertension, ACE inhibitors, ARB blocker

    The magnetoelectric device concept which enables the non-volatile electric field control of magnetism needs to be investigated for the development of practical information storage devices. In this aspect, the emerging field of magneto-ionics based on the modulation of magnetism by field-driven ion migration is promising because it only requires a simple sample structure in the solid state and has good cyclability. However, the degree of ion migration within the magnetic structure is strongly dependent on the crystal orientations. Since the epitaxial films growing on the commercial single crystal substrates have limited orientations, the ability of magnetism modulated by field-driven ion migration cannot be optimized and understood by using these data. In this work, we utilized the high-throughput synthesis approach, namely, combinatorial substrate epitaxy, which utilizes a polycrystalline substrate. This provides a platform to develop and understand the degree of ionic migration in different orientations of the model system CoFe2O4 (CFO) films. The library of electric driven nanoscale magnetization reversal data of CFO with different orientations was obtained by applying the electric field in the same region of known CFO grain orientations. It was determined from the analysis that the [110] crystal direction exhibits the maximum nanoscale magnetization reversal ratio. This is mainly attributed to the ease Co2+ migration in the [110] direction under the electric field assisted by a Fe3+ and oxygen vacancies. (C) 2017 Author(s).     Antimicrobial, Wound Healing And Antioxidant Activities Of Anthocephalus Cadamba

    Anthocephalus cadamba (Roxb.) Miq. Syn A. chinensis (Lamk) A. Rich (Rubiaceae) is ethnomedicinally widely used in the form of paste by tribe in western Ghats for treating skin diseases. In this context, antimicrobial potential of A. cadamba against a wide range of microorganisms was studied. To validate the ethnotherapeutic claims of the plant in skin diseases, wound healing activity was studied, besides antioxidant activity to understand the mechanism of wound healing. The alchoholic and aqueous extract of this plant showed significant antibacterial and antifungal activity against almost all the organisms: Micrococcus luteus , Bacillus subtilis , Staphylococcus aureus , Escherichia coli , Klebsiella pneumoniae , Proteus mirabilis , Pseudomonas aeruginosa , and four fungi Candida albicans , Trichophyton rubrum - dermatophyte fungi, Aspergillus niger , Aspergillus flavus and Aspergillus nidulans - systemic fungi, with especially good activity against the dermatophyte (Trichophyton rubrum) and some infectious bacteria (Escherichia coli, Proteus mirabilis and Staphylococcus aureus) with an MIC of 2.5 μg/disc. The results show that A. cadamba extract has potent wound healing capacity as shown from the wound contraction and increased tensile strength. The results also indicated that A. cadamba extract possesses potent antioxidant activity by inhibiting lipid peroxidation and increase in the superoxide dismutase (SOD) and catalase activity

    Comparative evaluation of biphasic insulin with metformin and triple oral hypoglycemic agents (OHA) in type 2 diabetes patients

    Introduction: The prevalence of secondary failure to oral hypoglycemic agents among type 2 diabetes mellitus (T2DM) patients ranges from 30 to 60%. The alternative approaches to overcome this issue are either switching to triple oral hypoglycemic agents (OHA) or intensifying the regimen by adding insulin. Objective: To compare the glycemic control achieved with biphasic insulin plus metformin and triple OHA in T2DM patients who were not adequately controlled with two OHA regimen. Methods: A qualitative prospective study was conducted at Asir diabetes center, Abha, KSA. Poorly controlled T2DM patients with two OHA for at least 1 year with glycated hemoglobin (HbA1c) >7.0% were included. Subjects were divided into group I (a third OHA was added to the existing two OHA regimen) and group II (switched over to Biphasic insulin and metformin). At baseline and 3-month intervals, level of HbA1C, Fasting Plasma Glucose (FPG), Postprandial Plasma Glucose (PPG), Blood Pressure (BP), lipid profile and hypoglycemic episodes were obtained and evaluated for one year. Results: 41.1% of patients were in group I and 58.9% were in group II. At the end of the study, there was a significant reduction in HbA1c in group II subjects comparing to group I (8.18 ± 1.32 vs 8.79 ± 1.81, p = 0.0238). FPG and PPG were improved also in group II. The mean body weight increased from baseline in group II is +4.48 kg and decreased from baseline in group I (−0.46 kg). 11.3% from group I and 23.7% from group II reported hypoglycaemic incidences. Conclusion: Biphasic insulin and metformin regimen could be an appropriate therapeutic option for achieving good glycemic control compared with triple OHA in patients with two OHA failure.
