    TO THE EDITOR: Intravenous immunoglobulin (IVIG) therapy has gained popularity for the treatment of neuromuscular diseases (i.e., myasthenia gravis, inflammatory myopathy, chronic inflammatory demyelinating polyneuropathy), although adverse events are associated with high-dose IVIG infusions.1,2 Common adverse reactions to IVIG therapy are anxiety, headache, fever, chills, chest pain, dyspnea, nausea, and abdominal pain.3 More serious adverse events include anaphylaxis, hemolytic anemia, hepatitis C, and thrombosis.3 Studies have shown documented effects of IVIG on blood rheology. It increases plasma viscosity in a dose-related response and may also activate platelets.2–4 High-dose IVIG therapy is approximately 24–54 g/d.

    Formal Development of a Total Order Broadcast for Distributed Transactions using Event-B

    Abstract. In a replicated database system, copies of the database are kept across several sites for fault-tolerance and availability. Data access in such systems is usually done within a transactional framework. A read-only transaction accesses data locally and an update transaction modifies the database at all sites. Total order broadcast primitives have been proposed to support transactions and allow fault-tolerant cooperation between the sites in a distributed system. In this paper, we identify and analyze the problem of formation of deadlocks among conflicting update transactions due to race conditions and outline how a system of total order broadcast prevents deadlocks and transaction failures. Later we outline how a refinement based approach with Event-B can be used for formal development of the models of total order broadcast. In this approach we begin with the abstract model of a total order broadcast and verify that the required ordering properties are preserved by the system. total order can correctly be implemented by using a notion of sequence number. This technique requires us to discharge proof obligations due to consistency and refinement checking. To discharge the proof obligations we are required to discover invariants that describes the relationship between the abstract total order and the underlying mechanism.

    Allergic rhinitis is one of the most common chronic conditions in the US, surpassing even heart disease. Over 1billionisspentannuallyforoverthecounter(OTC)andprescriptionmedications.Inaddition,allergicrhinitiscausesanestimatedannuallossof1 billion is spent annually for over-the-counter (OTC) and prescription medications. In addition, allergic rhinitis causes an estimated annual loss of 639 million related to lost work days and diminished work performance. If not effectively treated, allergic rhinitis may lead to potential complications such as otitis media, sinusitis, facial and dental abnormalities, hearing loss, poor speech development, and recurrent upper respiratory tract infections. Ineffective treatment often the result of intentional or unintentional noncompliance and inadequate patient education. Therefore, it is imperative that the clinician effectively educate the patient and tailor the treatment regimen so it enhances compliant behavior

    This article questions the validity of Deborah S. Cornelius’s claims which she presents in her recently published book on interwar and World War II Hungary. These exonerate the revisionist, anti-Semitic and war-time policies of the Horthy regime. The monograph also presents the Hungarian leaders in an undeservedly positive light. The author of the review demonstrates that Cornelius’s representation of the past was accomplished by the selective reading of primary and secondary sources. Cornelius also commits too many factual errors in order to justify some of her assertions

    Learning theories and interprofessional education: a user's guide

    There is increasing interest in the theoretical underpinning of interprofessional education (IPE) and writers in this field are drawing on a wide range of disciplines for theories that have utility in IPE. While this has undoubtedly enriched the research literature, for the educational practitioner, whose aim is to develop and deliver an IPE curriculum that has sound theoretical underpinnings, this plethora of theories has become a confusing, and un-navigable quagmire. This article aims to provide a compass for those educational practitioners by presenting a framework that summarizes key learning theories used in IPE and the relationship between them. The study reviews key contemporary learning theories from the wider field of education used in IPE and the explicit applications of these theories in the IPE literature to either curriculum design or programme evaluation. Through presenting a broad overview and summary framework, the study clarifies the way in which learning theories can aid IPE curriculum development and evaluation. It also highlights areas where future theoretical development in the IPE field is required
