    Professionalization of Anesthesiologists and Critical Care Specialists in Humanitarian Action: A Nationwide Poll Among Italian Residents.

    BACKGROUND: Over the last decades, humanitarian crises have seen a sharp upward trend. Regrettably, physicians involved in humanitarian action have often demonstrated incomplete preparation for these compelling events which have proved to be quite different from their daily work. Responders to these crises have included an unpredictable mix of beginner-level, mid-level, and expert-level providers. The quality of care has varied considerably. The international humanitarian community, in responding to international calls for improved accountability, transparency, coordination, and a registry of professionalized international responders, has recently launched a call for further professionalization within the humanitarian assistance sector, especially among academic-affiliated education and training programs. As anesthetists have been involved traditionally in medical relief operations, and recent disasters have seen a massive engagement of young physicians, the authors conducted, as a first step, a poll among residents in Anesthesia and Critical Care Medicine in Italy to evaluate their interest in participating in competency-based humanitarian assistance education and in training incorporated early in residencies. METHODS: The Directors of all the 39 accredited anesthesia/critical care training programs in Italy were contacted and asked to submit a questionnaire to their residents regarding the objectives of the poll study. After acceptance to participate, residents were enrolled and asked to complete a web-based poll. RESULTS: A total of 29 (74%) of the initial training programs participated in the poll. Out of the 1,362 questionnaires mailed to residents, 924 (68%) were fully completed and returned. Only 63(6.8%) of the respondents voiced prior participation in humanitarian missions, but up to 690 (74.7%) stated they were interested in participating in future humanitarian deployments during their residency that carried over into their professional careers. Countrywide, 896 (97%) favored prior preparation for residents before participating in humanitarian missions, while the need for a specific, formal, professionalization process of the entire humanitarian aid sector was supported by 889 (96.2%). CONCLUSIONS: In Italy, the majority of anesthesia/critical care residents, through a formal poll study, affirmed interest in participating in humanitarian assistance missions and believe that further professionalization within the humanitarian aid sector is required.     Introduction: As a result of the gaps in humanitarian response highlighted by several reports, the international community called for an increased professionalization of humanitarian aid workers. This paper describes a pilot project by an Italian university and a non-profit, non-governmental organization to implement a medical apprenticeship in low-income countries during Anesthesia and Intensive Care Medicine residencies. Methods: Before deployment, participants were required to complete a dedicated online training course about safety and security in the field, principles of anesthesia at the district hospital level, emergency and essential surgical care, essentials of medical treatment in resource-constrained environments and psychological support in emergencies. Results: At the end of the program, a qualitative self-evaluation questionnaire administered to participants highlighted how the project allowed the participants to advance their professional skills when working in a low-resource environment, while also mastering their adapting skills and the ability to interact and cooperate with local healthcare personnel. The project also proved to be a means for personal growth, making these experiences a recommendation for all residents as a necessary step for the professionalization of healthcare personnel involved in humanitarian aid

    Specific and General Information Sharing Among Academic Scientists

    We provide theoretical and empirical evidence on the factors that influence the willingness of academic scientists to share research results. We distinguish between two types of sharing, specific sharing in which a researcher shares her data or materials with another and general sharing in which scientists report results to the entire community (as in conference presentations). We present two simple games in which scientists research a problem of scientific merit (with an associated prize of academic and/or commercial value). In both cases, the scientists have intermediate research results but none has solved the entire problem.We test these models using a unique survey of bio-scientists in the UK and Germany regarding their willingness to "share." Our results generally support both models. In both, sharing is negatively related to competition and the importance of patents. In other respects they differ markedly. For example, large teams are more likely to share specifically but less likely to share generally. Rank does not matter for general sharing, but it does for specific sharing, where untenured faculty are less likely to share. One important implication is that policies designed to enhance sharing must be tailored to the type of sharing.

    Bilateral and unilateral arm training improve motor function through differing neuroplastic mechanisms: a single-blinded randomized controlled trial

    BACKGROUND AND PURPOSE: This randomized controlled trial tests the efficacy of bilateral arm training with rhythmic auditory cueing (BATRAC) versus dose-matched therapeutic exercises (DMTEs) on upper-extremity (UE) function in stroke survivors and uses functional magnetic resonance imaging (fMRI) to examine effects on cortical reorganization. METHODS: A total of 111 adults with chronic UE paresis were randomized to 6 weeks (3×/week) of BATRAC or DMTE. Primary end points of UE assessments of Fugl-Meyer UE Test (FM) and modified Wolf Motor Function Test Time (WT) were performed 6 weeks prior to and at baseline, after training, and 4 months later. Pretraining and posttraining, fMRI for UE movement was evaluated in 17 BATRAC and 21 DMTE participants. RESULTS: The improvements in UE function (BATRAC: FM Δ = 1.1 + 0.5, P = .03; WT Δ = -2.6 + 0.8, P < .00; DMTE: FM Δ = 1.9 + 0.4, P < .00; WT Δ = -1.6 + 0.7; P = .04) were comparable between groups and retained after 4 months. Satisfaction was higher after BATRAC than DMTE (P = .003). BATRAC led to significantly higher increase in activation in ipsilesional precentral, anterior cingulate and postcentral gyri, and supplementary motor area and contralesional superior frontal gyrus (P < .05). Activation change in the latter was correlated with improvement in the WMFT (P = .01). CONCLUSIONS: BATRAC is not superior to DMTE, but both rehabilitation programs durably improve motor function for individuals with chronic UE hemiparesis and with varied deficit severity. Adaptations in brain activation are greater after BATRAC than DMTE, suggesting that given similar benefits to motor function, these therapies operate through different mechanisms

    René Géronimo Favaloro : pioneer of Cardiac Surgery

    Dr. René G. Favaloro moved to the Cleveland Clinic in 1962 and proceeded to reshape the face of cardiac surgery as we knew it. Together with his colleagues at the Cleveland Clinic, Drs. Effler, Sones, Proudfit, Groves, Sheldon and countless others, he contributed to the double internal mammary arterymyocardial implantation by the Vineberg method, and by May 1967, he reconstructed the right coronary artery by the saphenous vein graft interposition. These landmark procedures paved the way for the aorto-coronary saphenous vein bypass graft in October 1967. Many similar breakthroughs ensued, with the application of the bypass technique to the left coronary artery, the combination of coronary artery bypass graft with left ventricular reconstruction and valve repair/replacement and finally, by December, a double bypass to the right coronary artery and anterior descending branch of the left coronary artery. In June, 1971, Dr. Favaloro decided to leave the Cleveland Clinic and return to Argentina where he created a medical centre, a teaching unit, a research department and finally an Institute of Cardiology and Cardiovascular Surgery. This was his greatest personal ambition. Over and above his brilliant mind and craft, Dr. Favaloro was a man of integrity, courage, honesty and humility, whose name will never cease to reverberate throughout the history of medicine.peer-reviewe

    Archaeological Testing and Preliminary Archival Investigations of Four Sites (41TV632, 41TV633, 41TV634 and 41TV635) at Waters Park, Travis County, Texas

    The Waters Park community and Sites 41TV632 through 635 are located on the left bank of Walnut Creek near its headwaters traversed by the Southern Pacific Railroad, FM 1325, and the Waters Park Road. The sites pertaining to the old Waters Park community lying in the proposed right-of-way of the north extension of Loop 1 (MoPac Boulevard) are those cited above. The sites were identified in 1974 during an independent survey. A large number of other sites are found in or adjacent to the right-of-way. Among these are Site 41TV39, the Rogers Spring Site previously excavated by The University of Texas; Site 41TV291, the James P. Wallace Site to be excavated by the State Department of Highways and Public Transportation (SDHPT); and several sites near the right-of-way. It was determined at that time that there were important sites at the locality. Further visits in 1984 reinforced the author’s opinion, and testing through us of a metal detector was proposed. A grid was laid out on the site promptly and accurately by the crew of the SDHPT District 14 headquarters. The author secured permission to test the site from the property owner, Jay W. Barnes, Jr. Edward Sutherland, of District 14 coordinated the District activities. At the Highway Design Division Environmental and Community Factors Section, James W. Barr and Frank A. Weir coordinated with the District. Tom Gorman aided in testing the metal detector. Milton Bell, Jerry Henderson, Ann Irwin, and Marilyn Rotter of the SDHPT Archaeology subsection provided production, administrative, and secretarial support
