    Auerbach’s research, besides exploring the three main fields the author explicitly names (i.e. the succession of different and differently grounded forms of realism in literary fiction, the alternate splitting and melting of styles, the link between literary work and a Christian vision of the world), in various chapters of Mimesis, and particularly in chs. 2, 6, 12, 14 and 20, is also focused, albeit only implicitly, on the developments of a further distinctive feature in Western literature, which is the emergence of subjectivity in the representation of reality. Focusing mainly on the chapter on courtly romance (ch. 6) and on the Quijote (ch. 14), this essay will examine Auerbach’s interpreting work on texts that adopt, in different ways, a ‘subjective point of view’; this kind of interpretation is, in part, unexpectedly similar to the approach of other critics and theorists of literature contemporary to Auerbach, like Bachtin, Américo Castro and Viktor Šklovskij

    Several market studies have indicated that the roll-out of broadband has significant economic and social relevance. The investments in broadband infrastructures needed to realise these economic and social benefits are, however, high-risk. This is firstly caused by the fact that the roll-out of access networks is extremely costly and requires high initial investments. Moreover, the multidisciplinary environment and involvement of many actors with diverging objectives and interests cause high complexity. Moreover, the market for broadband is changing fast and continuously. Although broadband has been on the academic research agenda for quite some time, this has as yet not resulted in a structured insight into the decision-making process around the upgrading of broadband networks, seen from the perspective of private operators and taking into account the vast complexity of the surroundings in which this decision-making process takes place in. The objective of this research was to provide a unifying theory combining technical, economic, market-related and regulatory factors with regard to an evolutionary, flexible approach of network upgrading from a decision-maker perspective. The result of this research is a robust, explanatory, parsimonious model for the impact and outcomes of broadband roll-out. This model provides a certain degree of stability in this multidisciplinary, unpredictable and highly dynamic research field.Technology, Policy and Managemen

    Pseudohypoparathyroidism (PHP) exemplifies a quite unusual form of hormone resistance as the underlying molecular defect is a partial deficiency of the α subunit of the stimulatory G protein (Gsα), a key regulator of cAMP signaling pathway, rather than the hormone receptor itself. PHP, together with Albright hereditary osteodystrophy (AHO), is a rare disorder encompassing heterogeneous features, such as brachydactyly, ectopic ossifications, short stature, mental retardation, and endocrine deficiencies due to resistance to the action of different hormones, primarily PTH. The two main subtypes of PHP are caused by mutations and/or methylation defects within the imprinted GNAS cluster, whose main transcript is Gsα. Moreover, mutations in the PRKAR1A and PDE4D genes, both crucial as GNAS for cAMP-mediated signaling, have been demonstrated in patients with acrodysostosis, a disease of bone formation with characteristics similar to AHO, while small deletions of chromosome 2 may also lead to the AHO phenotype. The clinical and molecular overlap among these different but related disorders represents a challenge for endocrinologists as to differential diagnosis and genetic counselin

    In a randomized multicenter, double-blind, double-dummy, parallel group study a comparison of the efficacy and safety of 1 μg alfacalcidol to 880 IU vitamin D plus calcium carbonate (1 g calcium) once daily per os was performed on 148 postmenopausal osteoporotic Caucasian patients with normal vitamin D serum levels for 18 months. Bone mineral density (BMD) was measured at baseline, 12 and 18 months. Safety parameters were followed during the entire study period. Sixty-nine (90.8%) in the alfacalcidol group and 67 (93.1%) in the vitamin D group were included in the ITT analysis. Lumbar BMD in the alfacalcidol group increased by 0.017 g/cm2 (2.33%) and 0.021 g/cm2 (2.87%) from baseline (P<0.001) at 12 and 18 months, respectively, whereas in the vitamin D plus calcium group the increase was 0.005 g/cm2 (0.70%) from baseline (N.S.) at both 12 and 18 months. The higher changes from baseline in the alfacalcidol group, as compared to the changes in the vitamin D plus calcium group at both 12 and 18 months, were found to be statistically significant (P=0.018, 0.005). A small increase of mean femoral BMD was achieved in both groups (N.S.). Adverse events were similar in both groups. No significant differences were noted between the groups in serum calcium. In conclusion, alfacalcidol was found to be superior in significantly increasing lumbar BMD as compared to vitamin D plus calcium while safety characteristics were found to be similar in both treatments

    Pseudohypoparathyroidism (PHP), pseudo-PHP, acrodysostosis, and progressive osseous heteroplasia are heterogeneous disorders characterized by physical findings, differently associated in each subtype, including short bones, short stature, a stocky build, ectopic ossifications (features associated with Albright's hereditary osteodystrophy), as well as laboratory abnormalities consistent with hormone resistance, such as hypocalcemia, hyperphosphatemia, and elevated parathyroid hormone (PTH) and thyroid-stimulating hormone levels. All these disorders are caused by impairments in the cAMP-mediated signal transduction pathway and, in particular, in the PTH/PTHrP signaling pathway: The main subtypes of PHP and related disorders are caused by de novo or autosomal dominantly inherited inactivating genetic mutations, and/or epigenetic, sporadic, or genetic-based alterations within or upstream of GNAS, PRKAR1A, PDE4D, and PDE3A. Here we will review the impressive progress that has been made over the past 30 years on the pathophysiology of these diseases and will describe the recently proposed novel nomenclature and classification. The new term "inactivating PTH/PTHrP signaling disorder," iPPSD: (1) defines the common mechanism responsible for all diseases, (2) does not require a confirmed genetic defect, (3) avoids ambiguous terms like "pseudo," and (4) eliminates the clinical or molecular overlap between diseases
