Clinical uses of benzodiazepines. Focus on: benzodiazepines and anxiety disorders [Utilizzi clinici delle benzodiazepine Focus on: Benzodiazepine e disturbi d'ansia]
Benzodiazepines (BDZs) are usually safe and well-tolerated medications, which may be effectively prescribed in Anxiety Disorders and several other conditions requiring an anxiolytic and hypnotic therapeutic effect. Most often, BDZs should be considered as a complementary, rather than alternative, treatment to antidepressants such as SSRIs and SNRIs or to psychotherapies. Nonetheless, BDZs my be very useful tools both in the acute treatment phase, due to their ability to control anxious symptoms before the antidepressants start working, and in the continuation-maintenance treatment phase, as augmenting agents. BDZ have proven very effective in panic disorder, generalized anxiety and, even if to a lower degree, in social phobia. BDZ role in obsessive compulsive disorder and post traumatic stress disorder is not as strong but these medications may be useful for several patients with those diseases. BDZ are not devoid of side effects and risks such as tolerance and dependence. However, those risks may be reduced when BDZ are not used at high doses for long periods of time, when the choice of the specific compound is driven by an accurate evaluation of the specific pharmacokinetic properties and when patients at high risk of dependence are not prescribed these compounds, especially at high doses and for long periods of time. Al fine di rendere oggettivabile la sintomatologia riferita dal paziente, sono state costruite misurazioni di tipo psicologico, interviste cliniche strutturate e scale di autovalutazione, specifiche per i soggetti che hanno subito un evento traumatico. L’uso delle scale sintomatologiche, analizzate nel corso della trattazione, è finalizzato fondamentalmente alla valutazione della gravità riferita ai singoli sintomi e alla gravità globale del disturbo; tali strumenti permettono inoltre di seguire in modo oggettivo l’andamento della sintomatologia
al follow-up e di valutare la compromissione funzionale.
Gli studi degli ultimi anni hanno dimostrato come il Disturbo post-traumatico da stress (DPTS) rappresenti l’esempio estremo della capacità degli eventi esterni di produrre cambiamenti strutturali e funzionali di lunga durata. Tali alterazioni, che si riflettono anche in disfunzioni mnesiche e attentive, hanno reso sempre più evidente l’importanza di effettuare, tramite test neuropsicologici standardizzati una valutazione delle alterazioni delle funzioni cognitive.
La obiettivazione della diagnosi di DPTS e la rilevazione di specifici sintomi ascrivibili alla sfera cognitiva permettono infine un riconoscimento di questo disturbo in termini medico-legali

Questionnaire for the Assessment of Quality of Life in Healthy Subjects (VQV-ss): Elaboration of a new instrument for the assessment of quality of life in the general population [Questionario per la Valutazione della Qualità della Vita nei soggetti sani (VQV-ss): Elaborazione di un nuovo strumento per l'assessment della Qualità della Vita nella popolazione generale]
Objective: The aim of our study was the elaboration of a new questionnaire for the assessment of Quality of Life (QoL) in subjects not affected by any General Medical Condition or psychiatric disorder. Methods: We recruited 200 healthy subjects from the general population; they rated themselves on the Quality of Life Enjoyment and Satisfaction Questionnaire (Q-LES-Q) and the Questionnaire for the Assessment of Quality of Life in Healthy Subjects (VQV-ss), that we elaborated. Results: The VQV-ss showed a good inner consistency (0,8775). The Concurrent validity was estimated by means of the correlations between the VQV-ss and the Q-LES-Q total score (r = 0,566; p < 0,001). The correlation between the item 71 score and the total one of the VQV-ss, representative of the total subject's QoL (r = 0,674; p = 0,01), witnessed the overall sensitivity and specificity of the VQV-ss. Factorial analysis of the VQV-ss identified 6 main factors, able to explain, altogether, the 37,55% of the total variance. Conclusions: The VQV-ss appears to be sufficiently reliable and valid, since shows a good inner consistency and the scores obtained with this instrument correlate in significantly with those of the Q-LES-Q, a validated and widely used instrument

Off-Label Uses of Trazodone: a Review
Introduction: Trazodone is an antidepressant belonging to the class of serotonin receptor antagonists and reuptake inhibitors. It is approved by the FDA for the treatment of depression. Insomnia is the most frequent reason for prescription of trazodone. It has also been proven useful in the treatment of anxiety disorders. Other off-label uses include the treatment of bulimia, benzodiazepine/alcohol dependence, fibromyalgia, central nervous system degenerative diseases (behavioral disorders in dementia and other organic disorders), schizophrenia, chronic pain disease and diabetic neuropathy, sexual dysfunction. Areas covered: This paper evaluates trazodone's efficacy and safety in its off-label uses. It also discusses the possibility that a combination of trazodone with SSRIs may prevent or treat some of the SSRI side effects, such as anxiety, insomnia and sexual dysfunction, in addition to synergically increasing SSRIs' antidepressant activity. Expert opinion: Few clinical trials have been conducted to evaluate trazodone's efficacy in the treatment of the diseases and symptoms for which it is often used in clinical practice. More studies are necessary to investigate possible new therapeutic indications, and to scientifically demonstrate the risk/benefit ratio for the many conditions for which trazodone is used, but not approved by the FDA. © 2012 Informa UK, Ltd
